Abstract
Description
The invention relates to medicine, in particular to pediatric orthopedics, neurology, namely to the clinical diagnosis of flat-valgus deformity of the feet and flat feet in children and adolescents by determining the level of serotonin in the blood serum, and can be used to objectively predict the development and progression of flat feet valgus deformity of the feet and flat feet. Serotonin is a monoamine neurotransmitter and hormone involved in the formation and regulation of various body functions, primarily the central nervous system, heart, and smooth muscles. The spectrum of the biological action of serotonin is not fully understood, therefore, further study of its biological properties continues.
The search for objective prognostic criteria for determining the likelihood of progression of flat-valgus deformity of the feet and flat feet in children and adolescents is one of the urgent tasks of pediatric orthopedics. As a result of the weakness of the ligamentous-muscular apparatus of the foot, there is a violation of the biomechanics of the foot with a flattening of its arches and the development of flat feet. With the progression of the disease, pain occurs in the joints of the lower extremities and back, which leads to asymmetric tension of the pelvic muscles, to its distortion and, as a result, the formation of scoliosis, and subsequently the development of osteochondrosis and osteoarthritis of the joints of the extremities. All this leads to a deterioration in the quality of life of patients.
Of the instrumental methods known:
1) способ диагностики плоскостопия, основанный на оценке результатов отпечатков опорной поверхности стопы с помощью чернил или других красящих веществ, выполненных на плантографе. По отпечаткам вычисляют отношения нагружаемой и не нагружаемой частей стопы по методу К. Годунову. Недостаток данного способа в том, что у детей до 3 лет выражена подкожная жировая клетчатка на подошвенной поверхности стопы, поэтому сложно оценить ее состояние;1) a method for diagnosing flat feet, based on the evaluation of the results of prints of the supporting surface of the foot using ink or other dyes made on a plantograph. According to the prints, the ratio of the loaded and unloaded parts of the foot is calculated according to the method of K. Godunov. The disadvantage of this method is that in children under 3 years of age, subcutaneous fatty tissue is expressed on the plantar surface of the foot, so it is difficult to assess its condition;
2) способ определения высоты свода стопы и величины угла между ладьевидной и пяточной костями на рентгенограмме в положении стоя является самым точным при определении степени плоскостопия. Недостатками способа является: лучевая нагрузка на растущий организм, необходимость квалифицированного специалиста для интерпретации рентгеновских снимков и специализированного оборудования, а также невозможность применения при массовых осмотрах;2) the method for determining the height of the arch of the foot and the angle between the scaphoid and calcaneus on the radiograph in the standing position is the most accurate in determining the degree of flat feet. The disadvantages of this method are: radiation exposure to a growing organism, the need for a qualified specialist to interpret x-rays and specialized equipment, as well as the inability to use for mass examinations;
3) a method for diagnosing the state of the foot sections (patent RU 2 253 363 C1 21.04.2004 Gavrikov K.V. “Method for diagnosing the state of the foot sections”) is based on taking prints of the plantar surface of the feet with their subsequent processing by a computer program and determining the degree of flat feet. The method is modern, technological, but its use in clinical practice is limited by the need for expensive equipment, which is not available in clinics.
A known method for predicting the development of plano-valgus foot deformity in children at an early age according to the state of muscle tone and reflexes. The method allows early prediction of the possible development of functional disorders of the musculoskeletal system in children (plano-valgus deformity of the feet and flat feet) and to carry out preventive measures. (A method for predicting functional disorders of the musculoskeletal system in children Pogosyan I.A. patent for invention RU 2 299 002 C1, 29.08. 2005). The disadvantage of this method is inaccuracy, since only visual inspection is used without instrumental and laboratory confirmation.
Also known are methods for diagnosing diseases of the skeletal system, based on the analysis of deviations of laboratory parameters from the norm. In the patent RU 2 670 769 23.11.2017 Zaytseva N. V. “Method of substantiating the occurrence in children of a negative effect in the form of a disease of the skeletal system associated with aerogenic exposure to fluorine compounds”, the effect of fluorine on the skeletal system of children with the development of scoliosis and other dorsopathies is indicated.
From patent RU 2 541 756 C2 Iomdina E.N. "Method for diagnosing disorders of the supporting function of the sclera in myopia in children and adolescents" it is known that in the presence of hypermobility of the joints and / or flat feet and / or curvature of the spine in children and adolescents, the value of the Kerdo index is more than 10% and the level of serum cortisol is less than 250 nmol / l diagnosing a violation of the supporting function of the sclera.
The main disadvantage of all the above methods is the lack of determining the likelihood of progression of flat feet and plano-valgus deformity of the feet in children in the early stages of development.
The objective of the invention is the possibility of predicting the progression of plano-valgus deformity of the feet and flat feet in children and adolescents aged 3 to 15 years in the early stages with high accuracy.
A method for predicting the progression of plano-valgus deformity of the feet and flat feet in children and adolescents aged 3 to 15 years is achieved by determining serotonin in the blood serum of children and adolescents in the age periods from 3 to 15 years. To confirm the accuracy of this method, radiography of the feet was performed in a lateral projection and the angle of pronation of the calcaneus was determined.
When the value of serum serotonin in children is up to 270 ng/ml, favorable formation of the arches of the foot is considered, with a value of serotonin above 270 ng/ml, a high risk of progression of plano-valgus deformity of the feet and flat feet is considered.
The method is carried out as follows.
2 days before the examination, foods containing serotonin are excluded from the diet: chocolate, walnuts, bananas, peaches, plums, melons, etc.
Required reagents: Serotonin ELISA Fast Track enzyme immunoassay kit (LDN, Germany) for the quantitative determination of serotonin in blood serum, urine and platelets.
In the examined child, 5 ml of blood is taken from a vein into a glass centrifuge tube in the morning on an empty stomach and centrifuged to obtain serum. Separated serum can be stored for 3 hours at room temperature, up to 12 hours at 4-8 degrees, and for extended periods at -20 degrees and below. Select 20 µl of blood serum and carry out its acylation. Then, by the method of competitive ELISA based on the binding of antigens to specific antibodies immobilized on the solid phase, the concentration of serotonin in the test sample is determined depending on the optical density at a wavelength of 450 nm. The concentration of serotonin in blood serum is calculated by extrapolating the obtained data to a calibration graph built according to the results of the analysis with a series of Standards and expressing the results in ng/ml. With a serotonin value in children up to 270 ng/ml, favorable formation of the arches of the foot is considered, with a serotonin value above 270 ng/ml, a high risk of progression of plano-valgus deformity of the feet and flat feet is considered.
We examined 76 children and adolescents aged 3 to 15 years with flat-valgus deformity of the feet and flat feet, of which 42 were boys and 34 were girls.
The control group included 15 children and adolescents aged 3-15 years who did not have signs of plano-valgus deformity of the feet and flat feet.
The advantage of the method is the accuracy of predicting the development and progression of flat-valgus deformity of the feet and flat feet in children and adolescents aged 3 to 15 years in the early stages of occurrence, which makes it possible to take timely preventive and therapeutic measures to prevent the development and progression of flat-valgus deformity stop and flat feet.

Example 3. Patient E., 9 years old. Objectively: flattening of the longitudinal arch of the feet, valgus position of the calcaneus relative to the axis of the tibia on the right 8° and on the left 9°. On the radiograph of the feet: the angle of height of the longitudinal arch of the foot was on the right 136°, on the left - 138°; arch height on the right 27 mm, on the left 25 mm, signs of longitudinal flat feet of both feet of the 1st degree. The serum serotonin content of patient E. was 250 ng/ml. Diagnosis: Longitudinal flat feet of the 1st degree. There is no risk of flatfoot progression.
Follow-up was carried out for 2 years. Patient E. periodically wore orthopedic insoles, not regularly. At 10 years, the content of serum serotonin was 230 ng/ml, at 11 years 260 ng/ml. Objectively at the age of 11: flattening of the longitudinal arch of the feet, valgus position of the calcaneus relative to the axis of the tibia on both sides on the right 7° and on the left 7°. On the radiograph of the feet: the height angle of the longitudinal arch of the foot was 134° on the right, 135° on the left; arch height on the right 29 mm, on the left 28 mm, signs of longitudinal flat feet of both feet of the 1st degree. Diagnosis: Longitudinal flat feet of the 1st degree, non-progressive course.
