Abstract
Description
Изобретение относится к медицине, в частности к способам лечения заикания. The invention relates to medicine, in particular to methods for treating stuttering.
From the prior art, there is a METHOD FOR TREATMENT OF STUTTER [RU2450835C1, publ. 05/20/2012], which includes self-regulation of the speech system through its connection with an external synchronizer, characterized in that at the first stage, a session of imperative suggestion and emotional stress therapy is carried out, after which patients are prescribed a silence mode outside of class for 5 days, then classes are conducted to form correct speech exhalation, voice delivery and voice leading, in addition, at the first stage, autogenic training is carried out, at the second stage, exercises are carried out in which patients are taught to synchronize speech with tactile irritation of the palms and fingers, in addition, at this stage, they conduct sessions of relaxing massage, self-massage , as well as sessions of group therapy to re-shape the attitude of patients to the speech defect, to increase the motivation for treatment, at the third stage, the skill of speech synchronization with tactile irritation of the palms and fingers formed in patients is used in artificially created difficult situations in the mode of "public speaking", graduallyreplacing the movements of two hands with the movement of one leading hand, and then, with speech, without its accompaniment by the movement of the leading hand.
Недостатком прототипа является использование в способе режима молчания на 5 дней, который практически невозможно выдержать детям. Ребенок воспринимает запрет на речь как наказание. У него возникает внутренний конфликт и протест, что приводит к дополнительному внутреннему напряжению и, как следствие, к усилению ступоров и запинок.The disadvantage of the prototype is the use of the silence mode for 5 days, which is almost impossible for children to withstand. The child perceives the ban on speech as a punishment. He has an internal conflict and protest, which leads to additional internal tension and, as a result, to increased stupor and stuttering.
Also known from the prior art is a METHOD FOR ELIMINATING STAMMERS [RU2556559C1, publ. 07/10/2015], which includes speech therapy sessions, characterized in that the treatment is carried out in three stages: preparatory, restorative and fixing, while at the preparatory stage, "pseudo-stuttering" is revealed, in contrast to "true" stuttering and is freed from "pseudo-stuttering", provide full-fledged intake of protein to the muscles of the body by correcting nutrition, taking into account the patient's blood group, eliminating desynchronosis by synchronizing the biorhythms of the patient's body by coordinating the frequency of the pulse beats per minute and the rhythm of speech exhalation with the performance in a multiple ratio to the given frequency of the rhythm of the fist: unclenching and squeezing the fingers hands in a fist and the rhythm of walking; as well as performing speech therapy exercises aimed at developing fluency of speech, exercises for coordination between mental and motor abilities, then, at the recovery stage, the causes of "true" stuttering are consistently eliminated by activating the respiratory, laryngeal and articulatory muscles: stuttering is eliminated in places of localization of respiratory spasms with the abdominal press , eliminate stuttering in places of localization of laryngeal spasms, connecting to the previous course of exercises with the larynx, which include contractions of the internal muscles of the larynx: swallowing saliva, singing, talking, reading aloud; further, stuttering is eliminated in places of localization of articulatory spasms, adding to the previous exercises exercises on the muscles of the articulatory apparatus with performing massage of the facial muscles, performing mimic exercises aimed at the mobility of the oropharyngeal and nasopharyngeal muscles; at the fixing stage, the developed speech skills are consolidated to natural speech, continuing to perform the techniques of the recovery stage.
Недостаток указанного способа заключается в том, что в этом случае формируется равнометричная искусственная речь, мало пригодная для общения. Кроме того, после отказа от использования метронома, заикание, как правило, возвращается.The disadvantage of this method is that in this case, uniform artificial speech is formed, which is not very suitable for communication. In addition, stuttering tends to return when you stop using the metronome.
From the prior art, there is a METHOD FOR TREATMENT OF STUTTER [RU2306153C2, publ. 09/20/2007], including the use of psychocorrection methods, speech therapy, characterized in that they conduct 5-7 courses of complex rehabilitation, 100 lessons each, while the complex rehabilitation includes the following 3 types of lessons: social and speech rehabilitation - 50 lessons, 45-90 min; adaptive physical rehabilitation - 25 sessions up to 45-180 minutes; psychological and pedagogical rehabilitation - 25 lessons for 45-90 minutes, each lesson is carried out with a generally accepted division into introductory, main and final parts; in the classroom for social and speech rehabilitation through deep disinhibition of speech and a metered load on auditory perception, pronunciation and psychoemotional sphere that increases from class to session, smooth and rhythmic speech is formed, for which the brain is first activated by stimulating the auditory and speech zones, while for excitation the auditory zone of the brain affects the patient's hearing with prolonged and / or loud sounds, speech and non-speech, and to excite the speech zone of the brain, the patient is offered to perform dosed speech loads in the form of prolonged and / or loud pronunciation of sounds, syllables, words, phrases, simple, complex sentences , monologue, in parallel they perform speech therapy generally accepted exercises for the development of smooth and rhythmic speech, but at the same time they perform them in the mode of a simulated psychoemotional load of a mini-stress nature, at the same time conduct training in public speaking using elements of dispute, argumentation and evidence of speech in interactive and polylogical modes; in classes on adaptive-physical rehabilitation, deep stretching of the muscles is carried out and for this, the main part of the lesson includes the performance of the gymnastic exercise "sitting bent over" in a static isometric mode, and during the exercise they strive to achieve the patient's position "legs together, chest lies on their feet", at which the angle formed by the patient's back and legs is zero, and to gradually achieve this state from session to session, the load is increased slightly beyond the tolerance limit so that the angle formed by the patient's back and legs decreases from session to session; in addition, at each lesson, the duration of the exercise is increased so that it is 30 seconds in the first lesson, increases by 0.5-1 minutes in each of the first 10 lessons and by 5-15 minutes in subsequent lessons, with the duration of the exercise being increased to the last lessons of the course up to 45-180 minutes, depending on individual indications; starting from the 10th lesson, when performing the exercise, an additional external load is exerted on the patient's back in the form of either pressing with the hands of the trainer, or imposing a load on the back, while the weight of the load is increased from session to session and gradually brought to a mass equal to half of the patient's body weight; in the classroom on psychological and pedagogical rehabilitation, stress resistance is developed by repeated repetition of stress loads that increase from class to session and their psychocorrection, for which at each lesson a stressful situation is simulated, aimed at causing a strong emotional response, both positive and predominantly negative, and then psychocorrection is carried out using the techniques of theatrical performance, gestalt therapy, body-oriented therapy with the evocation of expressed emotions, and the classes are carried out first individually, and then in a group of patients.
Недостатком данного аналога является большая длительность лечения заикания, что может привести в процессе коррекции к регрессу достигнутых результатов, ограниченность речевых нарушений, которые могут быть исправлены подобным образом и сильную зависимость достигаемых результатов от индивидуальных особенностей пациента.The disadvantage of this analogue is the long duration of stuttering treatment, which can lead to regression of the achieved results during the correction process, the limited speech disorders that can be corrected in this way, and the strong dependence of the achieved results on the individual characteristics of the patient.
Задачей изобретения является устранение недостатков прототипа. The objective of the invention is to eliminate the disadvantages of the prototype.
Техническим результатом изобретения является ускорение лечения хронической формы заикания, закрепление эффекта. The technical result of the invention is to accelerate the treatment of chronic forms of stuttering, consolidate the effect.
The specified technical result is achieved due to the fact that the method of treating the chronic form of stuttering in children over 5 years old, characterized by a cycle of therapeutic sessions aimed at the formation of muscle relaxation and emotional calm in the child at the beginning of a speech act, while each session is carried out in conditions of maximum muscle relaxation and begin with a series of special relaxing exercises: relaxation from pre-tension and stretching, supported by visual and mental images-oppositions, after which: in the first lessons, the child imitates a speech act with a long exhalation through relaxed lips, while using the technique of self-regulation of speech through its external synchronization with the movements of the fingers of the leading hand; in the following lessons, after the formation of the vowel pronunciation skill, the child adds consonants to the vowel sounds, combining them into syllables, while the vowel sounds are pronounced on a soft attack, with a smooth but quick transition to a full-sounding, strong pronunciation and the ability to rely on this sound before changing the speech deprivation by the mode of speech limitation; in the next lessons, the child, under the supervision of a trained adult, speaks in short speech segments to consolidate the developed speech skills in test situations, which include pronouncing memorized speech segments to strangers and talking with parents in the presence of strangers; at the final lessons, a new speech skill is consolidated that connects the beginning of speech with a feeling of muscle and emotional relaxation until overcoming logoophobia in all possible communication situations: school responses, everyday speech with loved ones, test situations, speech in public places, public speaking; when going through the entire cycle of classes for working out speech rules, psychological support of the child and his family is carried out to maintain a positive attitude of the child and his relatives to treatment, maintain a high level of motivation, and prevent emotional burnout and breakdowns in the treatment regimen.
В частности, перед проведением цикла занятий с ребенком и его близкими родственниками проводят профилактические беседы, направленные на повышение уровня мотивации и установление доверительных отношений между ребенком и специалистом.In particular, before the cycle of classes with the child and his close relatives, they conduct preventive conversations aimed at increasing the level of motivation and establishing a trusting relationship between the child and the specialist.
В частности, визуальными и мыслительными образами-противопоставлениями являются: «оловянный солдатик-тряпичная куколка», «сосулька на морозе-сосулька тает», «камень-подушка».In particular, visual and mental images-oppositions are: "a tin soldier-rag doll", "an icicle in the cold, an icicle melts", "a stone-pillow".
В частности, для детей от 5 до 12 лет цикл составляет 17 занятий. In particular, for children from 5 to 12 years old, the cycle is 17 lessons.
В частности, для детей старше 12 лет цикл составляет 10 занятий.In particular, for children over 12 years old, the cycle is 10 lessons.
Осуществление изобретения.Implementation of the invention.
Заявленное решение оптимальным образом подходит для лечения хронической формы заикания у детей старше 5 лет, при которой заикание возникло не менее чем за 6 месяцев до начала лечения.The claimed solution is optimal for the treatment of chronic stuttering in children over 5 years of age, in which stuttering occurred at least 6 months before the start of treatment.
Так, на первом этапе с ребёнком проводится профилактическая беседа, направленная на повышение уровня мотивации, веры в успешное лечение, установку доверительных отношений между пациентом и специалистом. Такая же беседа проводится с родителями и другими близкими родственниками ребёнка.So, at the first stage, a preventive conversation is held with the child, aimed at increasing the level of motivation, faith in successful treatment, and establishing a trusting relationship between the patient and the specialist. The same conversation is conducted with the parents and other close relatives of the child.
The next stage consists of 10 lessons for children and adolescents over 12 years old, and 17 lessons for children from 5 to 12 years old. The purpose of this stage is the formation of a new speech skill, which is based on a conditioned reflex that connects the onset of a speech act with a state of muscle relaxation and emotional calmness. For this, certain incentive materials are used. The word-signal for relaxation is "Tai", while the effect of relaxation is reinforced by a visual image. The child is shown a drawing depicting a melting icicle. At the first stages, classes are held in conditions of speech deprivation, and the child imitates a speech act with a long exhalation through relaxed lips. At this stage, the technique of self-regulation of speech is used by means of its external synchronization with the movements of the fingers of the leading hand. To achieve maximum muscle relaxation, special exercises are used: relaxation from pre-tension and stretching, supported by visual and mental images-oppositions: "tin soldier-rag doll", "icicle in the cold, icicle melts", "stone-pillow", etc.
This stage includes a set of activities aimed at mastering the child a new way of pronouncing vowels and consonants, combining them into syllables. The child is taught to easily pronounce consonants according to the principle of minimal muscle effort, this method is supported by visual images-oppositions "thorn-fluff", and is fixed in the patient's mind with the word image "Sound trace". Vowel sounds are pronounced on a soft attack, with a smooth but quick transition to a full-sounding, strong pronunciation and the ability to rely on this sound. This method of articulation and vocal delivery allows the patient to feel freedom from spasms, to believe in the possibility of safely pronouncing any sound and sound combination, and, most importantly, to get rid of the obsessive fear of certain sounds and words that is characteristic of most people suffering from chronic stuttering. Further, the sounds are combined into syllables, syllables into words, words into speech segments. During this period, speech deprivation is replaced by a speech restriction mode. The child is allowed to speak in short chunks under the supervision of a trained adult. At the same time, the child begins to consolidate the developed speech skills in test situations, which include the pronunciation of memorized speech segments to strangers and conversations with parents in the presence of strangers.
The third stage is aimed at consolidating a new pronunciation skill, a conditioned reflex that connects the start of speech with a feeling of muscle and emotional relaxation. An important goal of this stage is to overcome logoophobia by regularly testing speech skills in all possible communication situations: school responses, everyday speech with loved ones, test situations, speech in public places, public speaking. In parallel with classes on working out speech rules, psychological support of the patient and his family is carried out. The purpose of psychological support is to maintain a positive attitude of the child and those close to the treatment, maintain a high level of motivation, and prevent burnout and disruptions to the treatment regimen.
Let us dwell on these principles in more detail. The first visual image is introduced at the stage of developing a conditioned reflex that links the start of speech and the entire speech act with muscle and emotional relaxation. The child is shown an image of an ice icicle, they are asked to imagine a strong icicle in severe frost, to depict it, stretching out "in line" and straining all the muscles of the body. Then the specialist pronounces the word-signal for relaxation "Tai", asks the child to relax all the muscles, depicting a melting icicle, and shows another picture showing the icicle on which the bright sun shines. Moreover, the icicle hangs on a tree branch, and a sparrow sits on the branch below, on which drops fall from the melting icicle. This image evokes positive emotions in the child, the specialist, using this, invites the child to “bathe the sparrow”. Thanks to this playful moment, it becomes possible to perform multiple relaxation exercises from pre-tension, which is necessary to develop a conditioned reflex, while avoiding fatigue and loss of motivation in children of even the youngest age group.
The next visual and mental image is introduced at the stage of the formation of a new pronunciation skill of sounds according to the principle of minimum muscle efforts. The child is shown two contrasting pictures. "Evil thorn" symbolizes hard, tense pronunciation of sound, and "cute fuzz" is soft and relaxed. The specialist urges the child to never speak harshly, with tense articulation, so as not to let a thorn in, which can offend a kind fluff. The child trains to pronounce sounds softly, with relaxed lips, because the fluff loves so much. For additional motivation, the specialist informs that if the child tries to speak in the correct way, then the fluff will go to live at his home. This technique makes it easier for children to master the complex skill of correct, safe articulation, which helps to eliminate speech cramps.
