Предлагаемое изобретение относится к травматологии и ортопедии и может быть использовано при лечении асептического некроза средних и задних отделов стопы, обусловленного патологической функциональной перестройкой костной ткани. Лечение начальных стадий асептического остеонекроза, различных локализаций, в особенности средних и задних отделов стопы, является весьма трудной задачей, которая требует длительного времени. Проявления асептического остеонекроза на стопе заключаются в выраженном отеке, болях и значительным, вплоть до полной потери, снижении функции. Асептические некрозы условно подразделяются на два типа: вторичные асептические остеонекрозы, как следствие:The proposed invention relates to traumatology and orthopedics and can be used in the treatment of aseptic necrosis of the middle and hind parts of the foot caused by pathological functional rearrangement of bone tissue. Преимущества данного способа лечения начального асептического остеонекроза средних и задних отделов стопы, обусловленного патологической функциональной перестройкой костной ткани заключается в том, что туннельные каналы в костях стопы создают условия для иеоангиогенеза, а в туннельные каналы и фасциальные футляры стопы плазмы, богатой тромбоцитами (PRP-терапия (Platelet- Rich Plasma)) стимулирует регенерационные процессы за счет цитокинов и факторов роста, секретируемые из α-гранул тромбоцитов. Цитокины тромбоцитов, содержат TGF-β (продуцирующий фактор роста β), PDGF (тромбоцитарный фактор роста), инсулиноподобные факторы роста IGF-I и IGF-II, FGF (фактор роста фибробластов), VEGF (фактор роста сосудистого эндотелия), фактор роста эпидермальных тканей и фактор роста клеток эндотелия. Данные технические приемы позволяют ликвидировать процессы гипертензии в костной ткани, уменьшить зону отека и ишемии, обеспечив процессы восстановления (регенерации) костной ткани; снижают риски, связанные с развитием инфекционных и неинфекционных осложнений; аутоплазма не приводит к аллергическим реакциям со стороны организма; малая травматичность процедуры и малый объем плазмы обеспечивают минимальные проявления болевого синдрома; не требуют госпитализации больных, технически простое исполнение позволяет весь курс лечения проводить в амбулаторных условиях, уменьшая общие расходы на лечение пациента.The advantages of this method of treatment of initial aseptic osteonecrosis of the middle and hind parts of the foot, caused by pathological functional rearrangement of bone tissue, is that the tunnel canals in the bones of the foot create conditions for ioangiogenesis, and platelet-rich plasma into the tunnel canals and fascial cases of the foot (PRP-therapy (Platelet-Rich Plasma)) stimulates regeneration processes due to cytokines and growth factors secreted from platelet α-granules. These techniques make it possible to eliminate the processes of hypertension in the bone tissue, to reduce the area of edema and ischemia, ensuring the processes of restoration (regeneration) of the bone tissue; reduce the risks associated with the development of infectious and non-infectious complications; autoplasma does not lead to allergic reactions from the body; low invasiveness of the procedure and a small volume of plasma provide minimal manifestations of pain syndrome; do not require hospitalization of patients, technically simple execution allows the entire course of treatment to be carried out on an outpatient basis, reducing the overall cost of treating a patient.
Сущность изобретения заключается в том, что активируют процесс васкуляризации, при этом после местной или внутривенной анестезии выполняют туннелизацию пораженного участка стопы и вводят в полученные каналы аутоплазму в объеме до 5 мл, обогащенную тромбоцитами, затем, через 2 дня, а потом через каждые 5 дней 2-3 раза вводят аутоплазму в фасциальные футляры стопы в том же объеме в смеси с анестетиком в соотношении 1:1, после чего выполняют ортопедическую коррекцию стопы.The essence of the invention lies in the fact that the process of vascularization is activated, while after local or intravenous anesthesia, tunnelization of the affected area of the foot is performed and autoplasma in a volume of up to 5 ml, enriched with platelets, is injected into the resulting channels, then, after 2 days, and then every 5 days Autoplasm is injected 2-3 times into the fascial cases of the foot in the same volume mixed with anesthetic in a 1: 1 ratio, after which orthopedic correction of the foot is performed.

Под внутривенной или местной инфильтрационной анестезией в зонах асептического остеонекроза (определяют по средствам МРТ) однократно выполняют туннелизацию пораженных участков стопы (зон отека, ишемии, трабекулярного некроза костного мозга - на рис., п. 1), с целью снижения внутрикостного давления при отеке костного мозга, создания каналов в губчатой кости для обеспечения возможности прорастания сосудов в аваскулярные зоны, доставки непосредственно в зону асептического некроза стимуляторов регенерации костной ткани.1. Клинический пример. Больной Н., 41 г. Заболел после выполнения несвойственных образу жизни работ, связанных с подъемом и переносом тяжестей. В течение 2-х месяцев проведено лечение в муниципальном травпункте с диагнозами: плантарный фасцит, ахиллобурсит. Эффекта от лечения получено не было. Сохранялся болевой синдром. Передвигался с помощью костылей. На основании выполненной у нас МРТ и измерения давления в фасциальных фулярах стопы был сформулирован диагноз: полая стопа, перегрузка левой стопы, субхондральный отек-ишемия костного мозга пяточной кости, трабекулярный некроз пяточной кости (незавершенный стресс-перелом), гипертензия в фасциальных футлярах стопы. Под внутривенной анестезией выполнена туннелизация средне-задних отделов стопы (таранной, пяточной кости), в каналы введена богатая тромбоцитами плазма (PRP) и аспират аутологичного костного мозга 5 мл. Спустя два дня в футлярные пространства стопы введено 10 мл смеси обогащенной плазмы крови (PRP) - 5 мл и наропина - 5 мл. Данную процедуру повторяли каждые 5 дней - всего 3 процедуры. Через 1 мес после начала лечения пациент начал самостоятельно передвигаться без использования костылей в ортопедически откорректированной обуви. Контрольная МРТ через 1,5 месяца после начала лечения: положительная динамика, реваскуляризация зоны незавершенного стресс перелома, зоны трабекуллярного отека не определяется.Clinical example. Tunnelization of the mid-posterior parts of the foot (talus, calcaneus) was performed under intravenous anesthesia, platelet-rich plasma (PRP) and autologous bone marrow aspirate 5 ml were introduced into the canals. Two days later, 10 ml of a mixture of enriched blood plasma (PRP) - 5 ml and naropin - 5 ml were injected into the case spaces of the foot. This procedure was repeated every 5 days - a total of 3 procedures. One month after the start of treatment, the patient began to walk independently without using crutches in orthopedically adjusted shoes. Control MRI 1.5 months after the start of treatment: positive dynamics, revascularization of the zone of incomplete stress fracture, the zone of trabecular edema is not determined.
