Изобретение относится к области медицины, а именно к патологической анатомии и судебной медицине, связано с разработкой способа определения давности наступления смерти путем проведения посмертного лучевого исследования. Наличие сведений о времени наступления смерти и соответственно о длительности периода с момента ее наступления до исследования трупа существенным образом облегчает патологоанатомическую дифференциальную диагностику прижизненных поражений и посмертных изменений и способствует выяснению звеньев танатогенеза. В случае проведения судебно-медицинской экспертизы время наступления смерти имеет доказательное значение в раскрытии преступлений против жизни и здоровья граждан. В этой связи в ряде случаев компьютерная томография и/или магнитно-резонансная томография является основным этапом минимального инвазивного вскрытия [5]. Более того, ряд исследователей считает даже возможным замену традиционного аутопсийного исследования посмертным лучевым исследованием [6]. При помощи посмертной компьютерной томографии было показано увеличение количества жидкости в просвете бронхов больных, умерших в возрасте от 18 лет до 98 лет, в первые 15 часов после смерти [7]. На основании результатов двух повторных посмертных компьютерно-томографических исследований установлено увеличение объема свободной жидкости в плевральной полости 12 больных, умерших в возрасте от 27 лет до 81 года [8]. Интервал времени между компьютерно-томографическими исследованиями варьировал от 4 часов до 164 часов (средний интервал составлял 30 часов, минимальный – 18 часов), что не позволило авторам провести четкий анализ зависимости объема плевральной жидкости от давности наступления смерти. Кроме того, по заявлению самих авторов [8], существенным ограничением проведенного исследования явилось отсутствие наблюдений и соответственно данных раннего посмертного периода. In connection with the development of pneumothorax, thoracocentesis and drainage of the left pleural cavity were performed. Через 18 часов 28 минут после констатации смерти до аутопсии проводят магнитно-резонансное томографическое исследование в стандартных режимах Т1- и Т2 регистрации импульсных последовательностей в сагиттальной проекции в положении лежа на спине. На полученных Т1- и Т2-взвешенных изображениях в сагиттальной проекции определяют интенсивность сигнала ткани в верхней части печени (Т1верх = 492, Т2верх = 63) и в нижней части печени (Т1низ = 857, Т2низ = 181) умершего новорожденного. По формуле рассчитывают давность наступления смерти: t = 27,86 - = 27,86 - = 18,59 (часа). Следовательно, рассчитанное по формуле значение давности наступления смерти (18 часов 35 минут) на 7 минут больше интервала времени с момента констатации смерти, указанного в истории болезни, до момента проведения посмертной магнитно-резонансной томографии, указанного в протоколе посмертной магнитно-резонансной томографии. 18 hours and 28 minutes after death is confirmed before autopsy, magnetic resonance imaging is performed in standard T1 and T2 modes of recording pulse sequences in a sagittal projection in the supine position. On the obtained T1- and T2-weighted images in the sagittal projection, the intensity of the tissue signal in the upper part of the liver (T1 up = 492, T2 up = 63) and in the lower part of the liver (T1 low = 857, T2 low = 181) of the deceased newborn is determined. The formula calculates the prescription of death: t = 27.86 - = 27.86 - = 18.59 (hours). Consequently, the value of the prescription of death (18 hours 35 minutes) calculated by the formula is 7 minutes longer than the time interval from the moment of ascertaining death indicated in the medical history until the moment of postmortem magnetic resonance imaging indicated in the protocol of postmortem magnetic resonance imaging.

Предлагаемый способ определения давности наступления смерти, отличаясь объективностью, высокой информативностью и простотой, позволяет провести неинвазивную количественную оценку давности наступления смерти в раннем посмертном периоде, способствовать дифференциальной диагностике прижизненных патологических процессов с посмертными неспецифическими изменениями и соответственно выяснению причины смерти. Достоинством способа является возможность повторного многократного, в том числе, дистанционного анализа полученных томограмм, а также визуализации имеющихся патологических процессов.The proposed method for determining the prescription of death, differing in objectivity, high information content and simplicity, makes it possible to conduct a non-invasive quantitative assessment of the prescription of death in the early postmortem period, to contribute to the differential diagnosis of vital pathological processes with postmortem nonspecific changes and, accordingly, to clarify the cause of death. The advantage of the method is the possibility of repeated multiple, including remote analysis of the obtained tomograms, as well as visualization of existing pathological processes.
Диагностические возможности предлагаемого способа были проверены при сопоставлении результатов посмертной магнитно-резонансной томографии с данными историй болезни и патологоанатомического вскрытия тел 63 новорожденных и младенцев, умерших в возрасте 2 часа - 36 дней. Давность посмертного периода составляла от 2 часов 18 минут до 70 часов 20 минут. На основании проведенных сопоставлений установлено, что данный способ позволяет достаточно четко определить давность наступления смерти в раннем посмертном периоде (в пределах 36 часов). Рассчитанный коэффициент корреляции между рассчитанным по представленному способу значением давности наступления смерти и промежутком времени, прошедшим после констатации смерти, указанным в истории болезни, и проведением посмертной магнитно-резонансной томографии, указанным в протоколе исследования, составил 0,81 (р=0,0000082).The diagnostic capabilities of the proposed method were tested by comparing the results of postmortem magnetic resonance imaging with the data of case histories and postmortem autopsy of the bodies of 63 newborns and infants who died at the age of 2 hours - 36 days. The postmortem period ranged from 2 hours 18 minutes to 70 hours 20 minutes. On the basis of the comparisons carried out, it was found that this method makes it possible to quite clearly determine the prescription of death in the early postmortem period (within 36 hours).
