При наличии условий для доступа к собственной сети сосудов капсулы хронической субдуральной гематомы - диаметр сосудов сопоставим с размером микрокатетера для проведения эмболизации, в сосуды капсулы максимально возможно дистально заводят микропроводник, а по нему DMSO-совместимый микрокатетер. При выполнении мультиспиральной компьютерной томографии (МСКТ) головного мозга через 6 месяцев после травмы выявлены хронические субдуральные гематомы с двух сторон. Выполнено оперативное лечение - двухстороннее дренирование субдуральных гематом из фрезевых отверстий. В послеоперационном периоде регресс неврологической симптоматики не отмечен, сохранялись постоянные головные боли. На контрольной ангиографии данных за функционирование собственной сосудистой сети капсулы не получено. Затем направляющий катетер 7f Envoy перемещен в правую наружную сонную артерию. Микрокатетер Sonic при помощи микропроводника Asahi Chikai 10 заведен в левую среднюю оболочечную артерию, затем при помощи микропродника Hybrid 007J осуществлена максимально возможная дистальная катетеризация артерий собственной сосудистой сети капсулы гематомы, введено 3 мл неадгезивной композиции SQUID 18. На контрольной ангиографии данных за функционирование собственной сосудистой сети капсулы не получено. Васкуляризация капсул гематомы отсутствует. Сразу после операции больная экстубирована, неврологический статус: уровень сознания - ясное, речевых нарушений нет, высшие корковые функции сохранены, парезов, нарушений чувствительности нет. Через сутки после операции выполнена МСКТ головного мозга, при которой не выявлено увеличения размеров гематом (толщина 8 мм). В течение 2 недель пациентке проводилась реабилитационная терапия: электростимуляция глазодвигательных нервов, массаж. Через 15 дней после операции выполнена МСКТ головного мозга, продемонстрировавшая положительную динамику в виде уменьшения толщины гематом с 8 мм до 4 мм. For 2 weeks, the patient underwent rehabilitation therapy: electrical stimulation of the oculomotor nerves, massage. MSCT of the brain was performed 15 days after the operation, which showed positive dynamics in the form of a decrease in the thickness of hematomas from 8 mm to 4 mm. There was also a positive dynamics of neurological symptoms in the form of a decrease in meningeal-vascular headaches, regression of diplopia.

Заявляемый способ прошел клинические испытания при лечении 11 больных, из них 3 пациента были с рецидивами хронических субдуральных гематом после хирургического удаления. Во всех случаях удалось добиться полного выключения собственной сосудистой сети капсулы хронической субдуральной гематомы. Нарастания очаговой неврологической симптоматики не было. В ближайшем послеоперационном периоде отмечено уменьшение в размерах и затем полное рассасывание хронической субдуральной гематомы и регресс неврологической симптоматики.
