Известны способы лечения хронических субдуральных гематом и ликворных гидром путем краниотомии и костно-пластической трепанации черепа. Однако эти вмешательства травматичны и вызывают необходимость значительного количества (до 37%) послеоперационных повторных операций [1, 2].Known methods of treating chronic subdural hematomas and cerebrospinal fluid by craniotomy and osteoplastic craniotomy. Нередко в этой системе происходит закупорка клапана, что приводит к прекращению дренажной функции и требует повторного хирургического вмешательства. Способ осуществляют следующим образом.The method is as follows.
Начиная от проекции локализации патологического очага под местной анестезией или общим обезболиванием осуществляют линейный разрез кожи до скуловой дуги. В кости над очагом фрезой просверливают отверстие, одновременно вскрывают твердую мозговую оболочку и подлежащую капсулу. Проникают в подооболочечное пространство, в которое, в зависимости от количества жидкостных камер, вводят от двух и более эластичных биоинертных дренажных трубок, например субстернальных катетеров. Среднюю часть трубок располагают в силиконовой муфте, которую швами фиксируют к надкостнице в области фрезевого отверстия и скуловой дуги. Дистальные отделы трубок через микроинцизии фасции жевательной мышцы (m.masseter) микропинцетом вводят в толщу указанной мышцы. В зависимости от количества периферических трубок их распределяют как в поверхностном, так и в глубоком слоях мышцы. При госпитализации: сопорозное состояние, двигательное возбуждение, правосторонняя пирамидная недостаточность, элементы моторной афазии. В область гематомы введены две дренажные трубки, средняя часть которых размещена в силиконовую муфту. Муфта швами фиксирована к надкостнице в области фрезевого отверстия и скуловой дуги. Дистальные отделы трубок через микроинцизии фасции жевательной мышцы введены в ее толщу. Швы на рану.Operation: General anesthesia. На РКТ через 5 суток после операции - данных за рецидив гематомы нет. Общее состояние удовлетворительное. Регресс общемозговой и очаговой симптоматики.
