Abstract
Description
The invention relates to surgery, traumatology, combustiology, dermatology, to the treatment of skin diseases, burns and superficial, as well as deep wounds.
Preparations of gold are finding increasing medical use for the treatment of various diseases, including skin ones. Golden-Max gold-containing preparations are well known, but they are not a medicine and do not have proven efficacy in treating skin diseases. At the heart of the drug Golden-Max - the so-called. “Colloidal gold” which is not organized into nanoparticles. Therefore, on Golden-Max one cannot extrapolate many medicinal properties studied for gold nanoparticles (positive effects of targeted delivery, effects on tumors, on the immune system and on skin fibroblasts, etc.), which are disadvantages of this technology.
The preparations of the group of flavonoids are widely used as part of combination drugs for treating conditions associated with chronic lympho-venous insufficiency (diosmin, hesperidin), chronic inflammatory and dystrophic processes (rutoside as part of drugs wobenzym and flogenzyme). However, the properties of the combined use of preparations of the flavonoid group with nanoparticles of noble metals have not been studied enough, and the optimal combination has not yet been developed.
Close to the present invention is a method of medical use of gold-containing nanoparticles, described in patent RU 2500381 C1 (dated 10/01/2012, class IPC А61Л 8/04, publ. 10.12.2013, authors Khairullin RM, Terentyuk GS, etc. ), in which gold nanoparticles with a diameter of 140 nm are applied in combination with dimethyl sulfoxide. Dosage form applied to the skin - gel base. Immediately after application to the skin, application of 1 MHz ultrasound for 2 minutes is required.
The disadvantage of this method of treatment is the high cost of the soft form (gel base requires agar, which is less accessible and more expensive than vaseline, glycerols, silicones and other variants of the bases for soft dosage forms). Additional ultrasound treatment of low frequencies, in addition to the obvious resource costs, is not always safe for health. It is known, for example, that exceeding the thermal and mechanical indexes of the effect of ultrasound on tissues can pose a danger to the fetus during pregnancy, which is why it is not recommended to conduct too frequent examinations during this period of a woman’s life.
Наиболее близким к предлагаемому является способ лечения ран кожи с помощью наночастиц золота в оболочке фитохимических веществ из группы флавоноидов (источник: Lee J, Kim J, Go J, Lee JH et al. Transdermal treatment of the surgical and burned wound skin via phytochemical-capped gold nanoparticles. Colloids Surf В Biointerfaces. 2015 Nov 1; 135:166-74). The closest to the present invention is a method of treating skin wounds using gold nanoparticles in the shell of phytochemicals from the group of flavonoids (source: Lee J, Kim J, Go J, Lee JH et al.) Wth skin via phytochemical-capped gold nanoparticles. Colloids Surf At Biointerfaces. 2015 Nov 1; 135: 166-74). This method was tested only in experiments on animals and consists in applying gold nanoparticles coated with isoflavone gallic acid and (second version) gold nanoparticles coated with isoflavone protocatechic acid. Both variants of the method led to the acceleration of epithelialization of experimental wounds of the back of rodents and the best indicators of a number of biomarkers. The disadvantage of the method is the laboriousness and resource intensity of obtaining gold nanoparticles precisely in the variant of their coating with isoflavone compounds of gallic and protocatechic acid. The also described isoflavone components act only locally, targeted, and do not affect the systemic mechanisms of protection, as well as regional lymphatic and microvascular networks, the condition of which is just as important for wound healing.
The technical result of the invention is to increase the effectiveness of treatment of skin diseases, burns, superficial and deep wounds.
The technical result is achieved by the fact that for the treatment of skin diseases, burns, superficial and deep wounds, a treatment method is used which consists in applying on the skin or wound to the affected area of an aqueous micellar solution of nano-sized gold particles with regularity up to three times a day for up to ten days simultaneously with this application, oral administration of tablets of flavonoid group preparations is made: hesperidin (50–200 mg daily for 2 doses after meals), diosmin (450–900 mg daily for 2 doses after meals) ) And rutoside (50-200 mg per day in 2 divided doses after meals) in singly or in combinations with each other.
Nanoscale gold particles are atomic gold agglomerates with sizes of 1-100 nm, the surface of which is surrounded by a layer of stabilizer molecules, which makes it possible to achieve water / stabilizer / nanoscale gold particles' lifetimes of at least 12 months. The preparations of the flavonoid group (hesperidin, diosmin, rutoside) are well-studied medicinal substances that are used for microcirculatory disorders, lympho-venous insufficiency, tissue trophism disorders and as additive therapy in many areas of medicine.
Adv Mater. 2016 Oct; 28(39): 8567-8585).The properties of gold nanoparticles, such as flexible controllability of physicochemical and structural properties during creation, as well as unique surface properties, including local surface plasmon resonance, are of interest for treating a wide range of diseases. The surface properties make gold nanoparticles indispensable for controlling the release of drugs and passing them through the skin (thus, the pharmacokinetics of skin preparations can be adjusted) (source Chen W, Zhang S et al. Nanoparticles for Cancer Theranostics. Adv Mater. 2016 Oct; 28 (39): 8567-8585).
Они уменьшают аутоиммунное воспаление, уменьшают выработку цитокинов, тормозят созревание Т-лимфоцитов из Т1 в Т17, в то же время стимулируют созревание регуляторных Т-лимфоцитов (источник Nosratabadi R, Rastin М et al. Hyperforin-loaded gold nanoparticle alleviates experimental autoimmune encephalomyelitis by suppressing Th1 and Th17 cells and upregulating regulatory T cells. Nanomedicine. 2016 Oct; 12(7): 1961-1971).Immuno-modulating properties of gold nanoparticles matter. They reduce autoimmune inflammation, reduce the production of cytokines, inhibit the maturation of T-lymphocytes from T1 to T17, and at the same time stimulate the maturation of regulatory T-lymphocytes (source Nosratabadi R, Rastin M et al. Hyperforin-loaded gold nanoparticle alleviates experimental autoimmune encepharomyelome Th1 and Th17 cells and upregulating regulatory T cells. Nanomedicine. 2016 Oct; 12 (7): 1961-1971).
The proposed new method of local medical use of nano-sized gold particles in combination with the systemic administration of the preparations of the flavonoid group (hesperidin, diosmin, rutoside) involves the use of nano-sized gold particles in liquid media to combine medical appointments of such particles for topical use with the preparations of the flavonoid group (hesperidin, Diosmin, Rutozid), consists of 2 main operations:
1. Application of an aqueous solution of nanoscale gold particles to lesions located superficially (skin, wounds) in the form of a course of daily procedures (1-3 times per day) of different duration.
2. Or the simultaneous administration of oral tablets of the flavonoid group: hesperidin (50–200 mg per day for 2 doses after a meal), diosmin (450–900 mg per day for 2 doses after a meal), and ruthoside (50–200 mg per day for 2 meals after meals) alone or in combination with each other.
This method of treatment was successfully used in the clinic, as can be seen from the following examples:
Example 1. In the outpatient department of the Kirov Regional Clinical Budgetary Healthcare Institution (KOKBUZ) "Center for Traumatology, Orthopedics and Neurosurgery" 30 patients with burns of 1–2 degrees of severity were treated. The prevalence of lesions - 5-7% of the body surface. By randomization, patients were divided into 2 groups. In the first group (N = 15 people), patients received standard treatment with the addition of a liquid micellar solution of nanosized gold particles locally when dressing 1-2 times per day for 3-4 days (control group). In the second group (N = 15 people), in addition to the indicated combination (standard treatment + drugs with nano-sized gold particles), oral administration of a tablet drug Venarus, containing diosmin and hesperidin in dosages of 450 and 50 mg per tablet, was prescribed orally. Reception was made 2 times a day after meals (group "Flavonoids"). Both groups were compared in terms of the rate of epithelialization of skin defects and the frequency of complications of the wound process (infection of the wound, gross cosmetic defects). It turned out that in the group "Flavonoids" epithelialization occurred 8-9% faster compared with the control group. Also in the “Flavonoids” group, there were no purulent complications of the wound process and no cases of formation of cosmetologically unacceptable defects (pigmentation spots), while in the control group 4 such cases took place at once.
Example 2. Based on the Kirov Regional Clinical Budgetary Healthcare Institution (KOKBUZ) “Kirov Regional Clinical Dermatovenerologic Dispensary”, 30 patients with atopic dermatitis / neurodermatitis were treated. By randomization, patients were divided into 2 groups. In the first group (N = 15 people), patients received standard treatment with the addition of a liquid micellar solution of nano-sized gold particles locally with dressings 1-2 times a day for 10 days (control group). Both groups were compared in terms of the duration of the frequency and duration of the use of hormonal ointments in cases of insufficient efficacy of other agents (the steroid hormones that are part of such ointments can have a resorptive effect and cause severe side effects). It turned out that in the group "Flavonoids" the frequency of use of hormonal ointments decreased by 8% compared with the control group. Also in the group "Flavonoids" significantly decreased the duration of the use of hormonal ointments in cases of their appointment.
Improving the effectiveness of treatment of skin diseases, burns and superficial / deep wounds, as well as reducing the cost of treatment are the advantage and advantage of the proposed method compared with the prototype. At the same time, local effects of nano-sized gold particles in inflammation foci are enhanced by orally prescribed preparations of the flavonoid group (hesperidin, diosmin, rutoside), which improve microcirculation and lymph flow in the wound or inflammation area, as well as in the adjacent anatomical areas, reducing the likelihood of the spread of the pathological process . Thus, flavonoid preparations also reduce the hypoxic component of inflammation and contribute to the viability of local nonspecific defense mechanisms.
