Abstract
Description
The invention relates to surgery, traumatology, combustiology, dermatology, to the treatment of skin diseases, burns and superficial as well as deep wounds.
Various preparations of gold are finding increasing medical application for the treatment of various diseases, including skin. Golden-Max gold-containing preparations are widely known, which, however, are not a drug and do not have proven efficacy in the treatment of skin diseases. At the heart of the drug Golden-Max - the so-called. “Colloidal gold” which is not organized into nanoparticles. Therefore, on Golden-Max one cannot extrapolate many medicinal properties studied for gold nanoparticles (positive effects of targeted delivery, effects on tumors, on the immune system and on skin fibroblasts, etc.), which are disadvantages of this technology.
Preparations of succinic acid have long been widely used in the treatment of a wide range of internal, skin and systemic diseases, being a means of adjuvant therapy and significantly reducing the cost of treatment (source: Mazina N. K, “A systematic approach to justifying the use of energy metabolism regulators in pharmacotherapy and health regimens” Abstract of thesis for the degree of Doctor of Medical Sciences Tomsk, 2007). For parenteral use preparations of succinic acid are widely used in the form of methyl glucamine mixtures. However, the properties of the combined use of preparations of succinic acid with nanoparticles of noble metals have not been studied.
Близким к предлагаемому является способ медицинского применения золотосодержащих наночастиц, полученных из растворов НАuCl4 с редукцией экстрактом Aloe vera (библиографическая ссылка: Chandran SP, Chaudhary М et al. Synthesis of gold nanotriangles and silver nanoparticles using Aloe vera plant extract. Biotechnol Prog. 2006; 22 (2): 577 - 83).Close to the present invention is a method for medical use of gold-containing nanoparticles obtained from HAuCl 4 solutions with reduction of Aloe vera extract (bibliographic reference: Chandran SP, Chaudhary M et al. Synthesis of gold nanotriangles and silver nanoparticles using Aloe vera plant extract. Biotechnol Prog. 2006; 22 (2): 577-83).
The disadvantage of this method of treatment is unsatisfactory indicators of the cost / effectiveness ratio due to the complexity and high cost of the production method, the need for expensive equipment that is not compensated for by the sufficient effectiveness of the money received. Because of this, the properties of the gold-containing nanoparticles thus obtained were not sufficiently studied in terms of clear indications of use for treating skin and wound pathological processes, in particular, in large areas of damage (extensive skin diseases, large area burns). The possibilities of adjuvant therapy (the use of adjuvants) have not yet been studied in order to reduce the amount of expensive gold-bearing drugs consumed per unit of clinical efficacy of treatment. Also in the available literature does not contain descriptions of combined patterns of the use of gold nanoparticles, gold-containing nanoparticles, together with regulators of energy metabolism such as succinates and similar substances.
The closest to the proposed is "Solid-phase method of obtaining ... bioactive composite based on ... citric acid ... and gold nanoparticles" (see RU patent for invention No. 2534789, M. CL. SW 08/07/08, publ. 2014 ). The disadvantage of this method is the complexity, resource intensity (need gold-hydrochloric acid) and the need for equipment to create a pressure of 50 MPa and above. Such resource-hardware costs of this method did not allow the authors to work out and study the combined schemes of using their gold-bearing product together with other known drugs and energy metabolism regulators for the treatment of skin and other diseases.
The technical result of the invention is to increase the effectiveness of treatment of skin diseases, burns, superficial and deep wounds.
The technical result is achieved by the fact that for the treatment of skin diseases, burns, superficial and deep wounds, consisting in applying to the skin or wound in the affected area of an aqueous micellar solution of nano-sized gold particles with regularity up to three times a day for up to ten days, simultaneously with this application produces intravenous drip infusion of a medicinal solution of succinic acid in the form of methyl glucamine mixtures or compounds at the rate of 5-6 g of succinic acid per 1 liter of infusion mixture.
Nanoscale gold particles are atomic gold agglomerates with sizes of 1-100 nm, the surface of which is surrounded by a layer of stabilizer molecules, which makes it possible to achieve water / stabilizer / nanoscale gold particles' lifetimes of at least 12 months. Preparations of succinic acid are well-studied drugs used in hypoxic conditions, mitochondrial dysfunctions, and as additive therapy in many areas of medicine.
Adv Mater. 2016 Oct; 28(39): 8567-8585).The properties of gold nanoparticles, such as flexible controllability of physicochemical and structural properties during creation, as well as unique surface properties, including local surface plasmon resonance, are of interest for treating a wide range of diseases. The surface properties make gold nanoparticles indispensable for controlling the release of drugs and passing them through the skin (thus, the pharmacokinetics of skin preparations can be adjusted) (source Chen W, Zhang S et al. Nanoparticles for Cancer Theranostics. Adv Mater. 2016 Oct; 28 (39): 8567-8585).
Они уменьшают аутоиммунное воспаление, уменьшают выработку цитокинов, тормозят созревание Т-лимфоцитов из Т1 в Т17, в то же время стимулируют созревание регуляторных Т-лимфоцитов (источник Nosratabadi R, Rastin М et al. Hyperforin-loaded gold nanoparticle alleviates experimental autoimmune encephalomyelitis by suppressing Th1 and Th17 cells and upregulating regulatory T cells. Nanomedicine. 2016 Oct; 12(7): 1961-1971).Immuno-modulating properties of gold nanoparticles matter. They reduce autoimmune inflammation, reduce the production of cytokines, inhibit the maturation of T-lymphocytes from T1 to T17, and at the same time stimulate the maturation of regulatory T-lymphocytes (source Nosratabadi R, Rastin M et al. Hyperforin-loaded gold nanoparticle alleviates experimental autoimmune encepharomyelome Th1 and Th17 cells and upregulating regulatory T cells. Nanomedicine. 2016 Oct; 12 (7): 1961-1971).
The proposed method of local medical use of nano-sized gold particles in combination with the systemic appointment of an energy metabolism regulator involves the use of nano-sized gold particles in liquid media to combine medical appointments of such particles for local use with succinic acid preparations, consists of 2 main operations:
1. Application of an aqueous solution of nanoscale gold particles to lesions located superficially (skin, wounds) in the form of a course of daily procedures (1-3 times per day) of different duration.
2. Or the simultaneous appointment of methylglucamine compounds of succinic acid in the form of intravenous drip infusions as part of balanced medicinal solutions.
This method of treatment was successfully used in the clinic, as can be seen from the following examples:
10%.Example 1. In the burn department of the Kirov Regional Clinical Budgetary Healthcare Institution (KOKBUZ) "Center for Traumatology, Orthopedics and Neurosurgery" 30 patients with burns of 2-3 severity degrees were treated. The prevalence of lesions - 20-30% of the body surface. By randomization, patients were divided into 2 groups. In the first group (N = 15 people), patients received standard treatment with the addition of a liquid micellar solution of nanosized gold particles locally when dressing 1-2 times per day for 10 days (control group). In the second group (N = 15 people), in addition to the specified combination (standard treatment + drugs with nano-sized gold particles), an intravenous drip infusion of a medicinal solution of succinic acid in the form of methyl glucamine mixtures was calculated at the rate of 5-6 g of succinic acid per 1 liter of infusion mixture 1 time per day (Yantar group). Both groups were compared in terms of the duration of hospitalization and the cost of treatment for the totality of prescribed standard drugs and the expenditure of disposable medical instruments for dressings. It turned out that in the Yantar group the duration of hospitalization was reduced by 15% compared with the control group. Also in the Yantar group, the use of expensive medicines and medical materials decreased, which reduced the cost of treatment by 10%.
20%.Example 2. Based on the Kirov Regional Clinical Budgetary Healthcare Institution (KOKBUZ) “Kirov Regional Clinical Skin and Venereal Disease Dispensary”, 30 patients with common eczema were treated. By randomization, patients were divided into 2 groups. In the first group (N = 15 people), patients received standard treatment with the addition of a liquid micellar solution of nanosized gold particles locally when dressing 1-2 times per day for 10 days (control group). In the second group (N = 15 people), in addition to the indicated combination (standard treatment + drugs with nano-sized gold particles), an intravenous drip infusion of a medicinal solution of succinic acid in the form of methyl glucamine mixtures at the rate of 5-6 g of succinic acid per 1 liter of infusion mixture 1 time per day (Yantar group). Both groups were compared in terms of duration of hospitalization and frequency / cost of using expensive manipulations (hemosorption, plasma exchange). It turned out that in the Yantar group the duration of hospitalization was reduced by 10% compared with the control group. Also, in the Yantar group, the costs of high-tech procedures decreased, which reduced the cost of treatment by 20%.
Improving the effectiveness of treatment of skin diseases, burns and superficial deep wounds, as well as reducing the cost of treatment are the advantage and advantage of the proposed method compared to the prototype. At the same time, the local effects of nano-sized gold particles in inflammatory foci are enhanced by systemically prescribed preparations of succinic acid, which reduce the hypoxic component of inflammation and contribute to the maturation of regulatory T-lymphocytes along with gold particles.
