Различные препараты золота находят все большее медицинское применение для лечения различных заболеваний, в том числе кожных. Широко известны золотосодержащие препараты Golden-Max, которые, однако, лекарственным средством не являются и не обладают доказанной эффективностью при лечении кожных заболеваний. В основе препарата Golden-Max - т.н. «коллоидное золото», которое не организовано в наночастицы. Следовательно, на Golden-Max нельзя экстраполировать многие лечебные свойства, изученные для наночастиц золота (позитивные эффекты адресной доставки, воздействия на опухоли, на иммунную систему и на фибробласты кожи и др.), что является недостатками этой технологии.Various gold preparations are increasingly used for medical treatment of various diseases, including skin ones. Препараты янтарной кислоты давно и широко используются в лечении обширного круга внутренних, кожных и системных заболеваний, являясь средством адьювантной терапии и существенно снижая стоимость лечения (источник: Мазина Н.К, «Системный подход к обоснованию применения регуляторов энергетического метаболизма в схемах фармакотерапии и оздоровления». Автореферат дисс. на соискание уч. степени доктора медицинских наук Томск, 2007). Для парентерального применения препараты янтарной кислоты широко применяются в виде метилглюкаминовых смесей. Близким к предлагаемому является способ медицинского применения золотосодержащих наночастиц, полученных из растворов HAuCl4 с редукцией экстрактом Aloe vera (библиографическая ссылка: Chandran SP, Chaudhary М et al. Synthesis of gold nanotriangles and silver nanoparticles using Aloe vera plant extract. Недостатком этого способа лечения являются неудовлетворительные показатели коэффициента «затраты/ эффективность» из-за сложности и дороговизны метода производства, необходимости в дорогостоящем оборудовании, который не компенсируется достаточной действенностью полученного средства. Из-за этого свойства получаемых таким образом золотосодержащих наночастиц не были достаточным образом изучены в плане четких показаний применения для лечения кожных и раневых патологических процессов, в частности, при больших площадях поражения (обширные кожные заболевания, ожоги большой площади). Не были до сих пор изучены возможности адьювантной терапии (применения вспомогательных средств) для того, чтобы снизить количество дорогостоящих золотосодержащих препаратов, расходуемых на единицу клинической эффективности лечения. Так же в доступной литературе не содержится описаний комбинированных схем применения наночастиц золота, золотосодержащих наночастиц вместе с регуляторами энергетического метаболизма типа сукцинатов и сходных веществ.The disadvantage of this method of treatment is the unsatisfactory indicators of the coefficient "cost / effectiveness" due to the complexity and high cost of the production method, the need for expensive equipment, which is not compensated by the sufficient effectiveness of the funds received. Наиболее близким к предлагаемому является «Твердофазный способ получения… биоактивного композита на основе… лимонной кислоты… и наночастиц золота» (см. патент RU на изобретение №2534789, М. Кл. С08В 37/08, опубл. 2014). Недостатком указанного способа является трудоемкость, ресурсоемкость (нужна золото-хлористоводородная кислота) и необходимость в оборудовании для создания давления от 50 МПа и выше. Такие ресурсно-аппаратные издержки данного способа не позволили авторам отработать и изучить комбинированные схемы применения их золотосодержащего продукта совместно с другими известными препаратами и регуляторами энергетического метаболизма для лечения кожных и других заболеваний.Closest to the proposed is the "Solid-phase method for producing ... bioactive composite based on ... citric acid ... and gold nanoparticles" (see patent RU for invention No. 2534789, M. Пример 1. В ожоговом отделении Кировского Областного клинического бюджетного учреждения здравоохранения (КОКБУЗ) «Центр травматологии, ортопедии и нейрохирургии» лечение проходили 30 пациентов с ожогами 2-3 степени тяжести. Распространенность поражения - 20-30% поверхности тела. Посредством рандомизации пациентов разбили на 2 группы. В первой группе (N=15 человек) больные получали стандартное лечение с добавлением жидкого мицеллярного раствора наноразмерных частиц золота местно при перевязках 1-2 раза в сутки в течение 10 дней (контрольная группа). Во второй группе (N=15 человек) больным кроме указанной комбинации (стандартное лечение + препараты с наноразмерными частицами золота) назначали внутривенное капельное вливание лекарственного раствора янтарной кислоты в виде метилглюкаминовых смесей из расчета 5-6 г янтарной кислоты на 1 литр инфузионной смеси 1 раз в сутки (группа «Янтарь»). Обе группы сравнивались по показателям длительности госпитализации и стоимости лечения по совокупности назначенных стандартных лекарственных препаратов и расходования одноразового медицинского инструментария при перевязках. Оказалось, что в группе «Янтарь» длительность госпитализации уменьшалась на 15% по сравнению с контрольной группой. Так же в группе «Янтарь» уменьшалось расходование дорогостоящих медикаментов и медицинских материалов, что уменьшало стоимость лечения на 10%.Example 1. Во второй группе (N=15 человек) больным кроме указанной комбинации (стандартное лечение + препараты с наноразмерными частицами золота) назначали внутривенное капельное вливание лекарственного раствора янтарной кислоты в виде метилглюкаминовых смесей из расчета 5-6 г янтарной кислоты на 1 литр инфузионной смеси 1 раз в сутки (группа «Янтарь»), Обе группы сравнивались по показателям длительности госпитализации и частоты/ стоимости применения дорогостоящих манипуляций (гемосорбция, плазмоферез). Оказалось, что в группе «Янтарь» длительность госпитализации уменьшалась на 10% по сравнению с контрольной группой. Так же в группе «Янтарь» уменьшались расходы на высокотехнологичные процедуры, что уменьшало стоимость лечения на 20%.Example 2. Повышение эффективности лечения кожных заболеваний, ожогов и поверхностных глубоких ран, а так же уменьшение расходов на лечение являются достоинством и преимуществом предлагаемого способа по сравнению с прототипом.
