Известен способ уретропластики, включающий выполнение вентральной и дорзальной аугментации передней уретры с формированием трансплантатов, основного и дополнительного, при выполнении дорзальной аугментации трансплантат формируют из слизистой щеки, в котором дорзальную аугментацию передней уретры осуществляют на всем протяжении ее сужения, отделяя уретру от кавернозных тел, а вентральную - в месте ее максимального сужения, увеличивая разрез уретры проксимально и дистально до появления подвижности предварительно введенного в просвет уретры катетера, дорзальный трансплантат используют в качестве основного, а вентральный - дополнительного, причем дополнительный трансплантат формируют из слизистой языка (патент РФ №2 301 024, А61В 17/00, опубл. 20.06.2007). После этого в уретру устанавливают уретеральный катетер №16 по Шарьеру. Измеряют общую протяженность дефекта стенки уретры. According to the generally accepted method, an autograft of the mucous membrane of the cheek is oval.

При поступлении предъявлял жалобы на ухудшение мочеиспускания. Из анамнеза известно, что болеет в течение последних 10 лет. Пациенту неоднократно выполняли бужирование уретры с временным положительным эффектом. В августе 2016 г. впервые диагностирована протяженная стриктура бульбозного отдела. Госпитализирован в клинику урологии ВМедА в плановом порядке для оперативного лечения. По данным урофлоуметрии - объем мочеиспускания 324 мл, максимальная скорость потока 7,0 мл/сек, средняя скорость потока 4,6 мл/сек. Восходящая уретрография - стриктура бульбозного отдела уретры протяженностью 3,0 см. Лечение - 19.01.2017 г. выполнено оперативное вмешательство уретропластика бульбозного отдела лоскутом из слизистой рта с использованием обогащенной тромбоцитами аутоплазмы.Example 1. После этого с помощью калиброванного дозатора из каждой пробирки удалялось по 3,0 мл верхнего слоя плазмы. В результате получалось в 2-х пробирках около 2,0 мл обогащенной тромбоцитами аутоплазмы с последующим добавлением 0,2 мл 10% раствора кальция хлорида. Указанным способом при информированном согласии пролечено 7 пациентов с протяженными стриктурами уретры. Таким образом, результаты клинического исследования свидетельствуют о том, что предложенный метод достаточно эффективен при лечении протяженных стриктур передней уретры у мужчин. Преимуществом данного способа является низкая стоимость, простота применения, высокая результативность, отсутствие осложнений и побочных эффектов, что позволяет успешно применять его в урологии.In this way, with informed consent, 7 patients with extended urethral strictures were treated.
