Несостоятельность эзофагогастроанастомоза после резекции пищевода и желудка с эзофагогастропластикой является одним из наиболее частых и грозных осложнений. При несостоятельности эзофагогастроанастомоза после проксимальной резекции желудка с резекцией нижней трети пищевода лапаротомным доступом с расположением анастомоза в средостении, адекватное дренирование трубкой, установленной во время операции к анастомозу, обеспечивает ограничение воспалительного процесса в средостении. Для заживления свища через дренажную трубку, установленную во время операции через правую люмбодорзальную область, подпеченочное пространство и располагающуюся около дефекта в анастомозе в средостении, проводят через дополнительно сделанное в дренажной трубке на расстоянии 3-4 см от кожи, под контролем одновременно выполняемой фиброэзофагогастроскопии, катетер Фогарти. Конец катетера Фогарти с баллоном выводят за конец дренажной трубки в свищевой ход. Дренажную трубку подтягивают так, чтобы баллон катетера, выходя за пределы трубки, располагался на расстоянии 0,5-1,0 см от дефекта анастомоза. После этого баллон катетера раздувают, полностью перекрывая свищевой ход. Катетер фиксируют в этом положении к коже. При этом прекращается поступление содержимого пищевода по свищу, края дефекта смыкаются, отверстие в анастомозе закрывается грануляциями и рубцовой соединительной тканью в течение 2-3 недель. Положение катетера контролируется фиброэзофагогастроскопиями с интервалом 5-7 дней. При смещении раздутого баллона близко к дефекту в анастомозе, препятствующего смыканию его краев, катетер подтягивают для достижения необходимого расстояния в 0,5-1,0 см от линии несостоятельности. Свободное смыкание краев дефекта является необходимым условием его заживления. Питание осуществляется через назогастроеюнальный зонд, установленный во время резекции пищевода и желудка или при фиброэзофагогастроскопии. После заживления дефекта в анастомозе, подтвержденного рентгеновским исследованием с водорастворимым контрастом и фиброэзофагогастроскопией, жидкость из баллона удаляют и катетер Фогарти извлекают. Пример 1. Больной К., 65 л, поступил в торакальное отделение Пермской краевой клинической больницы с диагнозом: Аденокарцинома кардиального отдела желудка с переходом на пищевод. Выполнена операция лапаротомия, субтотальная проксимальная резекция желудка и нижней трети пищевода, спленэктомия, дренирование брюшной полости. Лимфодиссекция D2. На 3-и сутки по дренажу из брюшной полости появилось мутное отделяемое. Перитонеальных симптомов не определялось. Затем по данным компьютерной томографии появились признаки острого панкреатита, несостоятельности эзофагогастроанастомоза. Учитывая отсутствие клиники перитонита, ограниченность процесса продолжена консервативная терапия. Через 10 дней в связи с отрицательной динамикой выполнена релапаротомия, рассечение спаек, вскрытие поддиафрагмального абсцесса слева. На 4-е стуки после операции по дренажу появилось мутное отделяемое. Ежедневно выделялось по 250-350 мл. При эзофагоскопии эзофагогастроанастомоз в 37 см от резцов, на правой стенке дефект около 1 см, через который виден дренаж, прилежащий практически вплотную. Через инструментальный канал эндоскопа в двенадцатиперстную кишку проведен микроирригатор для питания. Дренаж подтянут на 1 см, через его стенку со стороны брюшной стенки проведен катетер Фогарти так, что его конец располагался в свище на 0,5 см от дефекта в анастомозе. После раздувания баллона просвет свища перекрыт полностью. Через 7 и 14 дней выполнена повторная эзофагоскопия. На 20-е сутки дефект в стенке пищевода полностью зарубцевался. Катетер Фогарти удален, через двое суток удален дренаж из брюшной полости. Больной переведен для дальнейшего лечения в торакальное отделение.Example 1. After balloon inflating, the lumen of the fistula is completely blocked. After 7 and 14 days, repeated esophagoscopy was performed. On the 20th day, the defect in the wall of the esophagus completely healed. The Fogarty catheter was removed, and drainage from the abdominal cavity was removed after two days. The patient was transferred to the thoracic department for further treatment.

Лимфодиссекция D2. На третьи сутки клиника несостоятельности эзофагогастроанастомоза, выполнена релапаротомия, ушивание дефекта эзофагогастроанастомоза, дренирование брюшной полости. Через 5 дней вновь клиника несостоятельности анастомоза. Перитонеальных симптомов нет, дренаж располагается около свища. При эзофагоскопии дефект в анастомозе 1 см, к нему предлежит дренажная трубка. Дренаж подтянут на 1 см. Через дренаж со стороны брюшной стенки проведен катетер Фогарти так, что его конец располагался на 0,5 см от дефекта в анастомозе, не препятствуя смыканию его краев. После раздувания баллона просвет свища перекрыт полностью. Проводилось зондовое питание. Отделяемое по дренажу сократилось до 20-30 мл в сутки. Положение катетера Фогарти трижды контролировалось при эзофагоскопии. Свищ закрылся в течение 3 недель полностью. При рентгеноскопии пищевода затеков контрастного вещества не определяется. Катетер Фогарти удален. При эзофагоскопии в области эзофагогастроанастомоза язвенный дефект до 1 см без признаков несостоятельности. Дренажи из брюшной полости и назоеюнальный зонд удалены. Выписан в удовлетворительном состоянии на амбулаторное лечение.Example 2. На 5-е сутки при рентгенконтрастном исследовании пищевода выявлен затек контрастного вещества. Выполнена релапаротомия, ушивание дефекта эзофагогастроанастомоза, дренирование брюшной полости. На 6-е сутки вновь мутное отделяемое по дренажу. Перитонеальных симптомов нет, содержимое свища полностью эвакуируется по дренажу. В течение 10 дней по дренажу по 200-300 мл отделяемого в сутки. Выполнена фиброэзофагоскопия, дефект в анастомозе 1 см. Через дренаж со стороны брюшной стенки проведен катетер Фогарти. Раздутый баллон катетера располагался на 0,8 см от дефекта в анастомозе, не препятствуя смыканию его краев. После раздувания баллона просвет свища перекрыт полностью. Проводилось зондовое питание через назогастроеюнальный зонд. Отделяемое по дренажу сократилось до 10 мл в сутки через 2,5 недели, при рентгеновском исследовании затек контраста отсутствует. При эзофагоскопии на месте несостоятельности эзофагогастроанастомоза язвенный дефект 1 см, покрытый фибрином, без признаков несостоятельности. Удалены катетер Фогарти, назоеюнальный зонд и дренаж из брюшной полости. При контрольной эзофагоскопии, на месте несостоятельности эзофагогастроанастомоза язвенный дефект, слизистая отечна. С проксимального края язвенного дефекта дивертиикулоподобное выпячивание до 3 мм, без признаков несостоятельности.Example 3.
