Neurological status: pupils symmetrical, photoreactions preserved, eyeball movements in full, intermittent spontaneous nystagmus, double vision when looking to the right; pharyngeal reflex is not caused; movements in the limbs preserved, sufficient strength, hypotension and hyporeflexia on the right, in the Romberg position deviates to the right and back. Spiral computed tomography of the brain was performed in axial, sagittal and frontal projections, a volumetric formation of the right hemisphere and cerebellar worm was discovered with dislocation and wedging of stem structures, occlusal internal triventricular hydrocephalus. The right cerebellar hemisphere is implanted 2.6 mm above the tentorial foramen. When conducting color duplex scanning, an increase in peripheral resistance indices was revealed with a Purselo resistance index RI = 0.82 and a Gosling pulsation index PI = 1.9. The degree of cerebellar-tentorial infringement of the brain stem is defined as moderate. In this regard, resection trepanation of the posterior cranial fossa, subtotal removal of the worm tumor and the right cerebellar hemisphere were performed as planned. Histological diagnosis: medulloblastoma with moderate cell polymorphism, rare mitoses, colic necrosis, without capillary endothelial proliferation.
Течение послеоперационного периода без осложнений, заживление раны первичным натяжением. Контрольная магнитно-резонансная томография показала, что опухоль удалена субтотально, явления ущемления ствола мозга на тенториальном уровне ликвидировано. Отмечает значительное улучшение. На 13 сутки после операции в удовлетворительном состоянии выписана под наблюдение невролога поликлиники.The course of the postoperative period without complications, wound healing by primary intention. В представленном наблюдении у пациентки молодого возраста имела место субтенториальная злокачественная опухоль в области задней черепной ямки с развитием мозжечково-тенториального ущемления ствола мозга умеренной степени. В связи с этим проведенная в плановом порядке операция с субтотальным удалением опухоли позволила ликвидировать ущемление ствола мозга, стабилизировать состояние и сохранить жизнь пациентке.In the presented observation, a young patient had a subtentorial malignant tumor in the posterior cranial fossa with a moderate degree of cerebellar-tentorial infringement of the brain stem. Пример 2. Пострадавший Ч. 59 лет, доставлен бригадой скорой помощи в травмоцентр в тяжелом состоянии через 40 минут после ДТП. Интубирован, число дыханий 20 в 1 минуту. Сознание утрачено, сопор. Пульс 64 удара в 1 минуту, ритмичный, напряженный. Артериальное давление 100 и 60 мм рт.ст. В экстренном порядке сделана спиральная компьютерная томография головного мозга в аксиальной, сагиттальной и фронтальной проекциях, обнаружена внутримозговая гематомы левого полушария мозжечка объемом 25 мл с ущемлением ствола. Левое полушарие мозжечка выбухает над вырезкой намета мозжечка на 5,7 мм. При цветовом дуплексном сканировании выявлено повышение индексов периферического сопротивления с индексом резистентности Пурсело RI=0,92 и индексом пульсативности Гослинга PI=2,8, снижение диастолической и средней скоростей кровотока. На основании проведенного исследования установили выраженную степень мозжечково-тенториального ущемления ствола головного мозга. В экстренном порядке проведена операция: трепанация задней черепной ямки, опорожнение и дренирование внутримозговой гематомы объемом 25 мл в виде сгустков крови. Послеоперационное течение тяжелое, проводилась искусственная вентиляция легких на протяжении 5 суток, нарушение сознания около 9 суток. При контрольных компьютерно-томографических исследованиях внутримозговая гематома удалена полностью, явления мозжечково-тенториального ущемления ствола регрессировали. В представленном наблюдении у пострадавшего с тяжелой черепно-мозговой травмой имела место внутримозговая гематома полушария мозжечка с развитием мозжечково-тенториального ущемления ствола мозга выраженной степени. Состояние при поступлении тяжелое, сознание нарушено по типу глубокого оглушения, в легких жесткое дыхание, хрипов нет, частота дыханий 24 в 1 минуту, тоны сердца приглушены, ритмичные, пульс 86 удара в 1 минуту, артериальное давление 120 и 80 мм рт.ст., умеренный гемипарез справа (3 балла), гемиплегия слева, тонус мышц резко повышен слева, глубокие рефлексы выше слева. Больна на протяжении 3 лет, начало заболевания - с появления приступов судорог в левой руке без потери сознания, наблюдалась с диагнозом «эпилепсия». За 4 месяца до поступления появились жалобы на слабость, сонливость, утомляемость, неловкость в левой руке, снижение памяти и критики к собственному состоянию. За 2 суток до поступления резкое ухудшение - перестала ходить, нарушилось сознание, появилось недержание мочи.Example 3. Проведена магнитно-резонансная томография головного мозга в аксиальной, сагиттальной и фронтальной проекциях, выявлена опухоль размерами 98×79×68 мм субтенториально в области слияния синусов по средней линии, смещение обоих полушарий мозжечка, больше слева в аксиальной проекции (фиг. 1а), в сагиттальной проекции (фиг. 1б) и фронтальной проекции (фиг. 1в) правое полушарие мозжечка выстоит над мозжечковым наметом на 19 мм. При цветовом дуплексном сканировании левой позвоночной артерии на экстра- и интракраниальном уровнях обнаружено повышение индексов периферического сопротивление до RI=1,0 и PI=3,13 (фиг. 2). В день поступления в экстренном порядке по жизненным показаниям выполнена операция - резекционная трепанация задней черепной ямки, субтотальное удаление внемозговой опухоли с локализацией в области слияния синусов. В ближайшем послеоперационном периоде состояние пациентки с положительной динамикой: сознание ясное, доступна элементарному контакту, регрессировал правосторонний гемипарез, слева - гемипарез 2-3 балла. Гистологическое заключение: менингиома. В представленном наблюдении у пациентки среднего возраста имела место субтенториальная доброкачественная опухоль в области задней черепной ямки больших размеров с развитием мозжечково-тенториального ущемления ствола мозга значительной степени, представляющего угрозу жизни. В связи с этим своевременная операция с радикальным удалением опухоли позволила стабилизировать состояние и сохранить жизнь пациентке.In the presented observation, a middle-aged patient had a subtentorial benign tumor in the region of the posterior cranial fossa of large sizes with the development of cerebellar-tentorial infringement of the brain stem to a significant extent, which posed a threat to life.
