Abstract
Description
The invention relates to medicine, namely to dermatology and cosmetology, and can be used to treat atrophic skin scars that have arisen due to various reasons.
A known method of eliminating scars (RU 2030923 C1, 03.20.1995), which consists in exposing the scar tissue to laser radiation. Laser exposure is carried out by a focused beam of a holmium laser with a wavelength of 1.96-2.15 μm in a pulse-periodic mode with a pulse duration of 200-500 μs and an energy of 0.1-2.0 J. Its disadvantage is that it is very difficult to measure the penetration depth laser beam, subsequently, a demarcation line remains at the border of the treated and untreated sections, in addition, the implementation of this method requires the use of expensive equipment.
A known method of treating hypotrophic skin scars, which also includes the use of laser radiation (RU 2323021 C1, 04/27/2008). Scar tissue is subjected to a single exposure to laser radiation with a wavelength of 540 nm, a pulse energy of 50-150 mJ, a pulse repetition rate of 1-4 Hz, a pulse duration of 1-2 ns. This method also requires the use of expensive equipment.
A known method of treating normo-atrophic scars with mechanical dermabrasion using a combined collagen coating with an antimicrobial agent (RU 2230554 C2, 20.06.2004). A set of preoperative measures and dermabrasion is carried out, and the surgical area is exposed on an outpatient basis under sterile conditions in the small operating room, the skin surface is treated twice with 10 ml butanol using a gauze cloth, capturing more than 2-3 cm of the operating area, and local anesthesia is performed with lidocaine, then, using a rotary apparatus, dermabrasion is performed and the surface of the skin is treated by applying a gauze towel moistened previously with butene, the surgical wound is treated with butene twice 10-15 ml from a plastic syringe without a needle, and 3-5 minutes after the patient has no burning sensation, a gauze cloth is removed with a forceps, after dermabrasion, a collagen coating is applied to the surgical surface, which exceeds the surgical area by 1-2 cm, and the surgical wound is twice without breaks are treated with 10-15 ml butol from a plastic syringe without a needle and the operating surface is irradiated with low-power infrared radiation 5-8 times a day, and in the postoperative period, gauze is doused several times a day for 7-10 days dressings with butol, up to the healing of the epidermis and spontaneous removal of the scab. This method is relatively time consuming.
The objective of the invention is to provide a simpler highly effective method for the treatment of atrophic scars, which allows to obtain a pronounced effect in a short time.
The technical result is to reduce the complexity and cost, as well as to shorten the treatment time and improve functional results.
The technical result is achieved by a method of treating atrophic skin scars, characterized in that blood is taken from the peripheral vein of the patient into vacuum tubes, the blood is mixed with an anticoagulant, the blood mixture with anticoagulant is centrifuged for 4-6 minutes at a rotational speed of 700-900 rpm the top layer of the resulting platelet-rich plasma is removed from the tube into a separate tube, the platelet-rich plasma is activated by calcium ions by adding calcium gluconate and mixing them, activated platelet-rich plasma in a syringe and injected intradermally into the rumen, the injected area is heated by a radiofrequency device Surgitron ® electrodes GlideSafe or PelleFirm to 41-43 ° C for at least 4 minutes.
In certain embodiments of this method, blood is collected from a patient’s peripheral vein into 3-9 ml vacuum tubes; 3.8% sodium citrate solution is used as an anticoagulant; to mix blood with sodium citrate, the tubes are inverted 4-5 times; the upper layer of the resulting platelet-rich plasma is removed into a separate tube using a laboratory dispenser; calcium gluconate in platelet rich plasma is added at the rate of 50 μl of calcium gluconate per 1 ml of plasma; 1 ml insulin syringes are used for injection; 0.1-0.2 ml of platelet-rich plasma per 1 cm 2 of the scar is injected into the rumen; heating is performed using a Surgitron ® Surgitron S5 radio wave apparatus; when warming, the skin temperature of the heated area is measured by an infrared pyrometer; the procedure is repeated 5-7 times, 1 time per week.
The proposed method is as follows.
From the patient's peripheral vein, blood is drawn into vacuum tubes of 3–9 ml in volume (depending on the required plasma volume). Blood is mixed with an anticoagulant, which is used as a 3.8% solution of sodium citrate. To mix blood with citrate, the tubes are inverted 4-5 times.
Then the blood is centrifuged for 4-6 minutes with a centrifuge speed of 600-800 rpm. During rotation, the blood in the test tubes is divided into three layers: the lower is red blood cells, the middle is a thin layer of white blood cells, and the upper is plasma enriched with platelets.
Using a laboratory dispenser, the top layer of platelet-rich plasma (BTP) is removed into a separate tube. After this, platelet-rich plasma (BTP) is activated with calcium ions: 50 μl of calcium gluconate per 1 ml of plasma.
The fluid is mixed by carefully turning the tubes. Thus, BoTP is ready for use and should be used within 5-10 minutes. The plasma volume required for the procedure depends on the size of the skin defect.
2.All obtained BTP are collected in 1 ml insulin syringes. BOTP is injected intradermally into the scar at 0.1-0.2 ml per 1 cm 2 .
After BoTP is injected into the scar, it is necessary to warm this area with the Surgitron S5 radio wave apparatus using brand electrodes GlideSafe or PelleFirm models (depending on the area of the scar) up to 41-43 ° C for 5 minutes (the temperature is measured with an infrared pyrometer on the skin above the scar) to resume regeneration processes. The procedure is carried out 1 time per week 5-7 times.
The method was applied in clinical conditions in 22 patients with atrophic skin scars aged 20 to 50 years. The patient was treated by the proposed method. 12 ml of blood were taken from peripheral blood (in 4 vacuum tubes of 3 ml each). 3.8% sodium citrate solution was used as an anticoagulant. Then, the blood was centrifuged for 5 min with a centrifuge rotation speed of 800 rpm. Then, using the laboratory dispenser, the upper layer (BOT) was removed into a separate tube. Then, BOTP was activated with calcium ions: 50 μl of calcium gluconate per 1 ml of plasma. The liquid was mixed by carefully turning the tubes. Received 5 ml of BOTP. All obtained BTP were collected in 1 ml insulin syringes. BOTP was injected intradermally into the scar at 0.1-0.2 ml per 1 cm 2 .
After BOTP injection into the scar, this area was heated using a Surgitron S5 radio wave apparatus with a PelleFirm ™ electrode to 42 ° C for 5 min (the temperature was measured with an infrared pyrometer on the skin above the scar) to resume regeneration processes. The procedure was carried out 1 time per week 5 times. After 6 months there was a complete regression of striae. By the end of 2 months, a significant densification of scar tissue and alignment of the relief with the surrounding skin were noted. No adverse effects have been reported.
As shown by the application of the proposed method for the treatment of atrophic scars, the inventors have developed a new technology for the use of autologous platelet-rich plasma for the treatment of this disease.
The use of a single slow centrifugation mode with a centrifuge rotation speed of 700-900 rpm for 4-6 minutes allows you to maximize the integrity of platelets containing growth factors that, due to the very high centrifugation speed, can be mechanically injured and / or enter into chemical interactions after due to an increase in temperature, and all this leads to premature platelet degranulation, to the destruction of growth factors and, as a result, to the absence of a positive effect.
In addition, due to short-term centrifugation for 4-6 minutes, platelets do not have time to sink to the bottom of the tube and mix with the layer of leukocytes and red blood cells, but remain suspended in the upper part of the tube, evenly distributed in the plasma, which does not require separation of the plasma into layers or repeated centrifugation.
The short time required for the manufacture of platelet-rich plasma, and the simplicity of its preparation make it possible to use this technique in routine medical practice in institutions of any profile.
Due to the injection of plasma proposed by the authors, growth factors remain in the area of scar tissue for a long time, thus managing to realize their biological effect and attract fibroblasts, which subsequently synthesize collagen, necessary for the resumption of skin regeneration processes.
