Abstract
Description
The invention relates to medicine, namely to otorhinolaryngology, and for the treatment of exudative otitis media.
Е. и соавт., 2003; Авдеева CH., 2006; Paradise J.L. et al., 2000; Antenius J.L., 2007).Exudative otitis media is one of the most common diseases of the middle ear. Hearing loss, which is one of the most persistent symptoms of ESO, often causes social maladaptation of patients (Zagoryanskaya M.E. et al., 2003; Avdeeva CH., 2006; Paradise J.L. et al., 2000; Antenius J.L., 2007).
Exudative otitis media is characterized by the presence of exudate in the tympanic cavity, accompanied by hearing loss in patients with no pain and preserved integrity of the eardrum. The basis of the pathogenesis of exudative otitis media is a combination of tubular dysfunction and sluggish inflammation of the mucous membrane of the middle ear.
Conservative treatment of the acute stage of exudative otitis media includes the use of vasoconstrictor, anti-inflammatory, antisecretory and hypersensitizing drugs in combination with physiotherapy. With the ineffectiveness of conservative therapy for more than 2 weeks, as well as in the treatment of the chronic stage of exudative otitis media, when a viscous exudate forms in the tympanic cavity, a surgical procedure is performed - myringotomy or tympanostomy - shunting of the tympanic cavity. In the late stages of exudative otitis media with an outcome in tympanofibrosis or tympanosclerosis, multi-stage reconstructive surgery of the middle ear is necessary.
The low efficiency of conservative and surgical treatment of the chronic form of exudative otitis media necessitates rational treatment in the early stages.
Н.А. - С. 382].A known method of treating exudative otitis media by electrophoresis in the pathological focus of enzymes hyaluronidase and proteolytic action. For this purpose, a funnel with a carbon electrode can be used for endaural electrophoresis [Soldatov IB, Preobrazhensky NA, Patyakina O.K. Conservative treatment of hearing loss. // Hearing loss. Ed. ON. Preobrazhensky. - M .: Medicine, 1978. - S. 382].
A known method of treating exudative otitis media by conducting endaural ultrasound therapy (RU 2539426, IPC A61N 7/00, publ. 20/01/2015), including ultrasonic treatment of the middle ear tissue using a solution of thiamphenicol glycinate acetylcysteinate, with amplitude oscillations of 40 microns with a frequency of 26.5 kHz for a duration of 15 s, a course of 7 procedures conducted daily.
The disadvantages of these methods include insufficiently deep penetration of the drug into the middle ear and in sufficient concentration.
A known method for the endaural administration of the Derinat preparation (RU 2358743, IPC A61K 31/711, published on 06/20/2009), when patients are injected with dexamethasone and epinephrine, proteolytic enzymes (trypsin), transtimpanic, aminotransferase, antitumor, and probolytic enzymes (trypsin). the tympanic cavity is injected with 0.5 ml of a 1.5% solution of sodium deoxyribonucleate (Derinat) with an interval of 48 hours, a course of 4 injections. This method is invasive and does not affect all links in the pathogenesis of exudative otitis media.
There is a method of treating exudative otitis media with hirudotherapy (Kuzmina MB abstract of the dissertation "Hirudotherapy of exudative otitis media in children" /, 2006, S. 5-8), in which leeches are applied to the area of the mastoid process, the seventh cervical vertebra, sacrum and anterior-ear zone. The disadvantages of the method include its complexity, a possible allergy to the secret of leeches and the threat of bleeding, which limits its use in widespread practice.
One of the effective means of treating exudative otitis media is reflexology, which allows you to influence the main links of the pathogenesis of the disease.
- С. 135).In case of ear diseases, an inhibitory effect on corporal points is proposed: TR17 (i-feng), TR21 (jiao-sun), IG4 (wan-gu), IG17 (tien-rong), VB2 (ting-hui), GI4 (he -gu), E36 (tsu-san-li), P7 (le-tsue) (Yarotskaya EP Reflexotherapy of diseases of internal organs, Kharkov, 1994. - S. 135).
Closest to the proposed one is the pharmacopuncture method using courses of cerebrolysin and actovegin at points IG19 (ti-gun), IG16 (tien-guan), VB2 (ting-hui), VB3 (shan-guan), VB10 (fu-bai), E41 (jie-si), E3 (jiu-liao), GI20 (ying-xiang), E6 (jia-che), TR16 (tian-yu); to the auricular points AP9, AP20 and general action points GI4 (he-gu), E36 (tsu-san-li), IG3 (hou-si), TR3 (chjun-chu), TR5 (wai-guan) (Farkhutdinova L. V., Sukhareva M.L., conference proceedings, Suzdal, February 28 - March 1, 2006, p. 98) The disadvantages of the method include the possible development of an allergic reaction to the injected drugs, in addition, this technique allows you to stop the symptoms of acute exudative secondary otitis due to improved ventilation function of the auditory tube, but does not affect the inflammatory processes in the middle ear cavity.
20.09.2002 г.).Recent studies have revealed a violation of local immunity in the middle ear cavity with exudative otitis media, which consists in increasing the number of lymphocytes and reducing the functional activity of neutrophils and macrophages (A. Knipenberg. Abstract of the dissertation "Clinical effectiveness of using local immunocorrection in the treatment of exudative otitis media" , 2009 - S. 7-8). Therefore, in the development of new approaches to the treatment of exudative otitis media, the authors used the well-known properties of the Alloplant biomaterial, powdered to a powder state (dispersed form), which consist in attracting and concentrating macrophage cells and stimulating their maturation, which makes it possible to correct fibrotic and degenerative dystrophic changes in tissues (RU No. 2189257, IPC A61L 27/00, A01N 1/00, publ. September 20, 2002).
It is known pharmacopuncture introduction of dispersed Alloplant biomaterial for the treatment and prevention of diseases (RU No. 2238076, IPC A61H 39/08, publ. 20.10.2004), when the drug is injected into biologically active points and scalp zones.
Currently, there is no data on the use of the pharmacopuncture use of Alloplant for the treatment of exudative otitis media.
The objective of the invention is to propose a method for the treatment of exudative otitis media, combining all the advantages of reflexology, as well as the immunocorrecting and reparative effects of the pharmacopuncture administration of Alloplant biomaterial, which will significantly increase the effectiveness of treatment and prevent the chronization of the process.

The proposed method improves the treatment of exudative otitis media due to the following factors:
- выбор точек фармакопунктуры обеспечивает оптимальный нейрорегуляторный эффект, что положительным образом сказывается на вентиляционной функции слуховой трубы;- the choice of pharmacopuncture points provides an optimal neuroregulatory effect, which positively affects the ventilation function of the auditory tube;
- впервые неспецифическое иммунокорригирующее действие биоматериала Аллоплант используется для нормализации локального иммунитета с целью предотвращения образования вязкого экссудата и спаечного процесса в полости среднего уха.- for the first time, the non-specific immunocorrecting effect of Alloplant biomaterial is used to normalize local immunity in order to prevent the formation of viscous exudate and adhesions in the middle ear cavity.
Clinical examples.
1. Patient Α., 46 years old, complained of congestion of the right ear, hearing loss, sensation of transfusion of fluid in the ear - more than two weeks. In the anamnesis - periodically there were similar symptoms that stopped on the background of conservative therapy with vasoconstrictive, secretolytic and anti-inflammatory drugs; in this case, the therapy was ineffective. Otoscopy of the right ear: The skin of the ear canal is not changed, the ear canal is free, the eardrum is gray, the light cone is shortened, a lunar strip of viscous secretion and a drop of exudate are visible. The mastoid process is painless. Otoscopy of the left ear - without features. Hearing: whispering in the right ear up to 1 m, conversational 5 m; the left ear is the norm. Audiometry: on the right violation of sound conduction up to 22 dB. Acoustic impedanometry: tympanogram type B on the right, the acoustic muscular reflex is not recorded.
In the smear reprint from the pharyngeal orifice of the auditory tube, a high content of degraded neutrophils and phagocytes is noted. Diagnosed with Right-sided exudative otitis media, secretory phase, subacute stage.
