Abstract
Description
The invention relates to medicine, in particular to dermatology, cosmetology, medical cosmetics and clinical pharmacology, and is intended for intradermal injections with the aim of discoloring a skin area stained due to bruising.
Известен способ обкалывания постинъекционного медикаментозного инфильтрата, вызываемого введением в клетчатку гиперосмотического раствора лекарственного средства, включающий определение объема введенного раствора, оставление иглы на месте инъекции, отсоединение от нее шприца, введение в клетчатку вторым шприцом раствора антидота и прикладывание пузыря со льдом на период не менее 30 минут, отличающийся тем, что предварительно определяют показатель осмотической активности раствора, незамедлительно определяют локализацию и размер инфильтрата, в качестве антидота используют охлажденную до 0°C воду для инъекции в дозе, обеспечивающей нормализацию осмотического давления введенного раствора, вводят немедленно первоначально половину дозы в виде последовательных инъекций по периферии инфильтрата вплоть до создания полного (замкнутого) инфильтрационного кольца, а затем через оставленную иглу в его центральную часть (RU 2333001 С1).A known method of chipping a post-injection drug infiltrate caused by introducing a hyperosmotic solution of a drug into the fiber, including determining the volume of the injected solution, leaving the needle at the injection site, disconnecting the syringe from it, injecting the antidote solution into the fiber with the second syringe and applying an ice bladder for a period of at least 30 minutes, characterized in that pre-determine the indicator of the osmotic activity of the solution, immediately determine the localization and size in of the filtrate, water cooled to 0 ° C is used as an antidote for injection in a dose that ensures the normalization of the osmotic pressure of the injected solution; immediately half the dose is administered immediately as sequential injections around the periphery of the infiltrate until a complete (closed) infiltration ring is created, and then through the left a needle in its central part (RU 2333001 C1).
The disadvantage of this method is low accuracy, safety, effectiveness and a narrow scope, since the method eliminates selective injection infiltration of the skin in the bruising area, water for injection does not have isotonic activity, and the temperature of the drug does not provide the optimal temperature regime in the tissues necessary for their effective bleaching and rinsing. The fact is that the method provides for the introduction of only water for injection and is intended for injection of the drug only into the fiber, saturated with hyperosmotic solutions of drugs. In addition, the method is not intended to determine the location and size of the bruise, as well as for the selective infiltration of skin-impregnated skin with blood and red blood cell remnants. At the same time, chipping fiber and other tissues with water for injection does not exclude osmotic damage to cells, including red blood cells. Therefore, injection of water for injection into the bruising area can cause hemolysis and additional staining of the skin in the bruising area due to rupture of red blood cells and the release of hemoglobin from them.
31.12.2014).There is a known procedure for performing an intradermal allergic test, in which hands are thoroughly washed with soap and warm running water; without wiping with a towel, so as not to violate the relative sterility, wipe them well with alcohol; put on sterile gloves and also treat them with a sterile cotton ball soaked in a 70% alcohol solution, draw the prescribed amount of a medicinal solution into the syringe, ask the patient to take a comfortable position (sit down or lie down) and free the injection site from clothes, treat the injection site with a sterile cotton ball, dipped in a 70% alcohol solution, make movements in one direction from top to bottom, wait until the skin dries at the injection site, then grasp the patient’s forearm with the left hand and fix the skin, right hand the needle is inserted into the skin with the cut upwards in the direction from the bottom up at an angle of 15 ° to the skin surface to the length of only the cut of the needle so that the cut is visible through the skin without removing the needle, slightly lifting the skin with the cut of the needle (forming a "tent"), transfer the left hand on the piston of the syringe and, pressing on the piston, inject the drug, remove the needle with a quick movement, put the used syringe, needles into the tray; used cotton balls are placed in a container with a disinfectant solution (http://www.provizor-online.ru/2011/02/vnutrikozhnaya-inekciya. Found on the Internet on December 31, 2014).
The disadvantage of this method is the low accuracy, safety, effectiveness and narrow scope, since the method excludes selective injection of skin infiltration in the bruising area, the temperature of the drug does not provide the optimal temperature regime in the tissues, necessary for their effective bleaching and “washing”, and the drug itself the drug does not have isosmotic activity. In this regard, intradermal injection of a drug with a solution does not exclude hyper- or hypoosmotic damage to cells, including red blood cells. Therefore, the injection of such drugs into the bruising area can cause hemolysis or wrinkling of red blood cells, which contributes to additional staining of the skin in the bruising area due to the destruction of red blood cells and the release of hemoglobin from them.
In addition, the method is not intended to determine the location and size of the bruise, for selective infiltration with a drug of the skin soaked in blood and the remnants of red blood cells.
The objective of the invention is to improve the accuracy, safety, efficacy and expand the scope of application due to intradermal injection of a warm isotonic solution up to its complete infiltration of the entire skin area in the bruising area with the appearance of a colorless "lemon peel".
The technical result is bleaching of the skin in the bruising area under conditions that exclude local post-injection hypoosmotic tissue damage, cell swelling, erythrocyte hemolysis, and hemoglobin staining of the skin.
The essence of the method of bleaching the skin in the area of the bruising, including determining the localization and size of the lesion site, chipping it according to the general rules with a solution of a drug with a certain temperature by means of successive injections, initially around the periphery of the damaged area, until a complete (closed) infiltration ring is created, and then the inside, lies in the fact that they illuminate a selected part of the body with a stream of white scattered light, determine the localization and size of the eye by eye a water leak in the skin, the drug is injected into the skin by intradermal injection, an isotonic solution of 0.9% sodium chloride is used as a medicine at a temperature of 37-42 ° C, the skin of the inside of the bruise is punctured until a complete infiltration field is created and merges with the infiltration ring with the formation of a single intradermal infiltrate, over which the skin takes the form of a colorless lemon peel.
At the same time, illumination of a selected part of the body with a stream of white scattered light increases the accuracy, efficiency and safety of determining the localization and size of the bruise in the skin by the eye, since it provides complete identification and detailed determination of the localization, size and shape of the bruise in the skin in conditions of full and sufficient light. Such detailing is necessary for proper skin infiltration with an isotonic solution of 0.9% sodium chloride in order to completely “wash” it with the entire thickness of the skin tissue that is “stained” with blood.
The use of isotonic solution of 0.9% sodium chloride at 37-42 ° C for interstitial skin washing increases the accuracy, efficiency, safety of the method and expands its scope, since such a tool is devoid of local irritating effect due to the correspondence of its osmotic activity to the level of physiological osmotic skin tissue activity. Therefore, such a tool does not cause osmotic damage to cells, including red blood cells, and does not additionally stain tissues with hemoglobin, since it remains inside whole red blood cells and does not go out of them. Using the drug warm at a temperature of 37-42 ° C improves in accordance with the Arrhenius law C. skin impregnation, absorption from the skin into it and “washing” of the skin from red blood cells and the remains of destroyed red blood cells. The indicated temperature range of the drug is optimal.
Chipping the inside of the bruise until the complete infiltration field is created and merges with the infiltration ring with the formation of a single intradermal infiltrate with the skin above it in the form of a colorless lemon peel increases the accuracy, efficiency, safety of the method and expands the scope, since only in this way can “rinse” the entire area of the skin from red shaped blood cells and hemoglobin with extensive bruising. The fact is that bruises in the skin can have different sizes: with an area from a few square millimeters to several hundred square centimeters. Therefore, with a single puncture of the skin in the very center of the bruise, it is not always possible to create an infiltrate that fills the entire field inside the formed infiltration ring. Only the complete fusion of the internal infiltrate with the external infiltration ring and the formation of a single intradermal infiltrate ensures complete washing of the entire skin area “stained” with blood in the bruising area. At the same time, the creation of a general infiltrate by infiltrating the skin with an isotonic solution of 0.9% sodium chloride up to the appearance of the skin as a colorless lemon peel provides maximum interstitial administration of the “washing” solution and its subsequent effective washing of the skin from red blood cells and hemoglobin when the solution is absorbed into the blood. The fact is that an isotonic solution of 0.9% sodium chloride with such a “saturation” of the skin with itself initially (during the injection infiltration) “squeezes” red blood cells and hemoglobin residues from it, and subsequently (during absorption into the blood) “captures” them, leaving the skin. At the same time, the creation of an injection infiltrate to the criterion of giving the skin a “colorless lemon peel” over the bruise ensures the maximum efficiency and accuracy of the method, since it thus guarantees complete discoloration of the skin already at the stage of injection infiltration. Moreover, the high efficiency and safety of the method contributes to the fact that the method is carried out in conditions of illumination of the bruising area with scattered white light. Such lighting provides, firstly, high accuracy of skin infiltration, as it creates the conditions for eye control to impregnate the entire "desired" area of the skin in the bruising area, and secondly, high efficiency of skin impregnation with a solution, since infiltration is carried out under conditions of maximum skin saturation washing solution, since it is carried out before giving the skin a colorless lemon peel over the infiltrate.
The method is as follows. If you suspect a bruise, the skin is illuminated on a selected area of the body with a stream of white scattered light and localization and size of the bruise are determined by eye. Then, as an agent, an isotonic solution of 0.9% sodium chloride is used at a temperature of 37-42 ° C, which is injected into the skin by intradermal injection. The skin of the inside of the bruise is punctured until a complete infiltration field is created, it merges with the infiltration ring and a single intradermal infiltrate forms with the skin above it in the form of a colorless lemon peel.
Thus, the proposed method due to the intradermal injection of a warm isotonic solution up to its complete infiltration of the entire skin area in the bruise area with the appearance of a colorless “lemon peel” to the skin allows us to expand the scope, increase the accuracy, safety and effectiveness of the method of bleaching the skin in the bruising area
