Известны хирургические способы реконструкции первого пальца кисти при полидактилии - врожденном пороке развития кисти, на которой вместо одного имеется два первых пальца, ограничивающих функцию и нарушающих косметику. Коррекция такого порока осуществляется путем ампутации менее функционального пальца или продольной промежуточной резекции каждого из них с последующим совмещением половинок и формированием, таким образом, полноценного первого пальца (Травматология и ортопедия: Руководство для врачей / Под ред. Н.В. Корнилова: в 4-х томах. - СПб.: Гиппократ, 2004. - Т.2: Травмы и заболевания плечевого пояса и верхней конечности / Под ред. Н.В. Корнилова и Э.Г. Недостатком первого способа является ампутация первого пальца, части которого, могут быть использованы при его реконструкции, а другой способ является сложным в техническом исполнении, тем более если функциональный палец расположен в первом межпальцевом промежутке и его не удается соединить с половинкой другого, т.е. операция исключена технически, или сопряжена с длительным заживлением и возможным неудовлетворительным итоговым результатом. Общим недостатком рассмотренных способов является то, что реконструкция первого пальца может не удовлетворять функциональным и эстетическим запросам.The disadvantage of the first method is the amputation of the first finger, parts of which can be used during its reconstruction, and the other method is difficult in technical execution, especially if the functional finger is located in the first interdigital gap and cannot be connected to the other half, i.e. the operation is technically excluded, or is associated with prolonged healing and a possible unsatisfactory final result. Технический результат поставленной задачи достигается тем, что для реконструкции первого пальца при врожденной полидактилии выполняют иссечение костных фрагментов удаляемого малофункционального добавочного наружного пальца с сохранением его питающихся кожных покровов, а затем производят транспозицию более функционального добавочного внутреннего пальца с фиксацией его на дистальный отдел первой пястной кости и увеличивают его до нормальных размеров за счет пластики сохраненными кожными покровами с удаленного малофункционального добавочного наружного пальца. Фиксацию выполняют осевой спицей до полного сращения костных фрагментов.The technical result of the task is achieved by the fact that for the reconstruction of the first finger with congenital polydactyly, the bone fragments of the removed non-functional additional external finger are excised while maintaining its feeding skin, and then the more functional additional internal finger is transposed with its fixation on the distal first metacarpal bone and increase it to normal sizes due to the plasticity of the saved skin integuments from a remote additional external pin. Fixation is performed with an axial spoke to complete fusion of bone fragments.

Первоначально, через фигурный разрез в области боковой поверхности малофункционального и предполагаемого к удалению наружного добавочного пальца осуществляется доступ к его костным фрагментам, которые полностью удаляются. Затем выполняется транспозиция более функционального внутреннего добавочного пальца, костные фрагменты которого располагаются над уровнем дистальной части первой пястной кости и фиксируются к ней посредством осевой спицы до полного сращения костных фрагментов. Реконструированный первый палец дополнительно формируется из мягких тканей удаленного малофункционального наружного добавочного пальца, за счет чего обеспечивается функциональный и эстетический результат лечения. После месячного периода, необходимого для сращивания костных фрагментов, спица извлекается, и пациент тренирует функцию схвата.Initially, through a figured incision in the region of the lateral surface of a non-functional and supposed to be removed external extra finger, bone fragments are accessed that are completely removed.
