В России ежегодно регистрируется свыше 63000 новых случаев рака легкого, из них 53000 - у мужчин, что в структуре общей онкологической смертности составляет 14%, а среди мужского населения - 22, 8%. Большинство больных раком легкого к моменту установления диагноза, в силу распространенности опухолевого процесса или серьезных сопутствующих заболеваний, являются неоперабельными. Лучевая терапия, а в последние годы синхронная химиолучевая терапия, являются основными методами лечения больных с местно-распространенными формами немелкоклеточного рака легкого.In Russia, over 63,000 new cases of lung cancer are registered annually, of which 53,000 are in men, which makes up 14% of the total oncological mortality, and 22.8% among the male population. Величина СОД недостаточна для достижения высокой эффективности лечения, а механическое увеличение дозы невозможно из-за возникающих осложнений, в т.ч. острого эзофагита. Кроме того, количество укладок при использовании данного режима облучения резко возрастает, что увеличивает длительность и сложность проведения лучевой терапии, осложняет работу медперсонала.The disadvantage of this method is that it does not ensure the achievement of the required total focal dose (SOD) supplied to the focus. Задачей предлагаемого изобретения является устранение указанных недостатков, повышение эффективности проводимого лечения, уменьшение частоты возникновения острого лучевого эзофагита (ОЛЭ), воспалений орофарингеальной зоны, возможность увеличения суммарной очаговой дозы радикального курса химиолучевой терапии больных местно-распространенным раком легкого. Также ставится задача по уменьшению степени выраженности лучевой реакции и отсрочки времени ее наступления. Кроме того, должны обеспечиваться возможность использования пациентом в своем ежедневном рационе более обширного набора продуктов (так же у них в меньшей степени возникает «страх приема пищи» в виду отсутствия выраженного болевого синдрома) и уменьшение сроков пребывания пациентов в стационаре.The objective of the invention is to eliminate these drawbacks, increase the effectiveness of treatment, reduce the incidence of acute radiation esophagitis (OLE), inflammation of the oropharyngeal zone, the possibility of increasing the total focal dose of a radical course of chemoradiotherapy of patients with locally advanced lung cancer. По предлагаемому способу пролечено 30 больных. Эффективность результатов профилактики и лечения острого лучевого эзофагита оценивалась по частоте ОЛЭ, степени тяжести, дозе возникновения симптомов, а так же данных фиброгастродуоденоскопии (на «высоте» симптомов). У пациентов, получавших «Колетекс-гель-ДНК-Л» и «Колетекс-гель-ДНК», не было отмечено острых лучевых эзофагитов 4 степени (характеризуется невозможностью приема пищи в виду выраженной болезненности в области пищевода, потери массы тела, обезвоживанием, изъязвлением пищевода в области, подвергшейся облучению по данным ФГДС). Так же всем больным лучевая терапия проводилась в полном объеме без перерывов в лечении, что повышает ее эффективность.The proposed method treated 30 patients. У больных РЛ проводилась ДЛТ в режиме динамического фракционирования (РОД 4 Гр - 2 фракции, РОД 3 Гр - 4 фракции, РОД 2 Гр - 15 фракций). Суммарная очаговая доза на лимфатические узлы средостения составляла 50 Гр (эквивалентно 55 Гр обычного фракционирования согласно линейно-квадратичной модели). Затем проводилось переформирование полей с уменьшением объема облучения до размеров первичного очага с полным исключением из поля облучения спинного мозга и, по возможности, других критических органов. Суммарная очаговая доза доводилась до изоэффективной 60-70 Гр (РОД 1,8 Гр, 5-7 фракций).In patients with RL, DLT was performed in the dynamic fractionation mode (ROD 4 Gy - 2 fractions, ROD 3 Gy - 4 fractions, ROD 2 Gy - 15 fractions). Выявлено значимое улучшение после проведенного лечения: средняя сумма баллов по шкале FACT-G до лечения составила 23,0±0,8, после - 16,4±0,3, p<0,05. Использование лучевой терапии в режиме динамического фракционирования позволило уменьшить сроки пребывания пациентов в стационаре.
