Возможность консервативного лечения РП имеет большую привлекательность благодаря ее несомненной медико-социальной и экономической эффективности. The possibility of conservative treatment of RP is very attractive due to its undoubted medical, social and economic effectiveness. This prompts the search for new, more effective methods of chemoradiotherapy for locally advanced stages of RP.

При наличии выраженного опухолевою стеноза на первом этапе проводят 3-5 сеансов аргоноплазменной коагуляции новообразования пищевода (аргоноплазменный электрокоагулятор ERBE версии ICC ERBH VIO). После восстановления адекватного просвета пищевода, спустя 5-10 дней после стихания местных реакций и отторжения некротизированных участков опухоли начинают химнолучевое лечение. На первом этапе осуществляют внутрипросветную брахитерапию высокой мощности дозы (на аппарате контактной лучевой терапии Nucletron MICROSELECTRON HDR-Genie) с источником облучения Ir 192. Разовая очаговая доза составляет 7 Гр (референтная доза на глубине 10 мм от источника), 1 раз в неделю, в течение 3 недель. Внутрипросветную брахитерапию назначают и проводят в дни внутривенных инфузий карбоплатина и таксола (1-й, 8-й и 15 дни курса), что обеспечивает максимальное разрушение опухолевой массы. Используют комбинацию внутривенного введения в 1-й, 8-й и 15-й дни курса раствора карбоплатина (AUC 2) и раствора паклитаксела из расчета 45 мг/м2/сутки после предварительной премедикации, с использованием дексаметазона (8-12 мг внутривенно), циметидина гидрохлорида 300 мг, десенсибилизирующих препаратов (р-р супрастина 2% - 2,0 или р-р тавегила 2,0 внутривенно), седативных средств. В качестве противорвотных препаратов используют растворы эмесета (4-8 мг в/в), зофрана (8 мг в/в) или пероральный антиэметик эменд (175 мг в первые сутки, 80 мг во второй и третий дни). Затем проводят конформную дистанционную лучевую терапию на медицинских ускорителях электронов с энергией тормозного излучения 6-15 МэВ по стандартной трехпольной методике (РОД 1,8 Гр ежедневно, 5 фракций в неделю, суммарная очаговая доза 45 Гр). При рентгенографии пищевода и фиброэзофагогастроскопии опухоль протяженностью 7,5 см практически циркулярно охватывает стенки пищевода, суживая его просвет до 5-7 мм. Проведено 3 сеанса аргоноплазменной реканализации пищевода, диаметр стенозированного участка - 10-11 мм. Этап внутрипросветной лучевой терапии с одновременным (в 1-й, 8-й и 15-й дни лечения) введением карбоплатина (AUC 2) и раствора паклитаксела из расчета 45 мг/м2/сутки перенес удовлетворительно, отмечено развитие эзофагита II степени. При контрольной эзофагогастроскопии - умеренно выраженная гиперемия и отечность слизистой стенки пищевода, регресс опухоли более 50%. Клинически отмечена сохраняющаяся дисфагия 3 степени. С 25 дня курса начата дистанционная лучевая терапия по стандартной методике с внутривенными инфузиями 5-фторурацила 1000 мг/м2 еженедельно. На этом этапе зарегистрирована лейкопения I-II степени, показатели периферической крови восстановились после завершения лечения. При контрольном обследовании через 4 недели после окончания курса терапии - полный клинический регресс опухоли, подтвержденный результатами биопсии стенки пищевода. Спустя 3 месяца в связи с рубцовой стриктурой в зоне расположения исходной опухоли произведена баллонная дилатация пищевода. После одной процедуры аргоноплазменной коагуляции опухоли, восстановления просвета пищевода и устранения дисфагии проведено лечение по разработанной методике с использованием перорального приема капецитабина 600 мг/м2 в дни дистанционного облучения. Между проведением этапов внутриполостного и дистанционного облучения сделан недельный перерыв в связи с явлениями лучевого эзофагита II степени.
