В настоящее время в странах Европы, а также в России наблюдается рост заболеваемости раком прямой кишки при сохранении достаточно высокого уровня летальности пациентов. Пятилетняя выживаемость при опухолях данной локализации не превышает 60%, частота местных рецидивов составляет 24-32% (при I стадии заболевания 10-15%, при II стадии - 20-30%, при III - до 50%). Снижение частоты местно-регионарных рецидивов, увеличение радикальности выполняемых операций при повышении возможностей сфинктерсохраняющих вмешательств являются важными аспектами лечения рака прямой кишки. В связи с этим возрастающее значение приобретают методики химиолучевого лечения (ХЛТ) рака прямой кишки. При местно-распространенных опухолях прямой кишки проведение неоадъювантной ХЛТ достоверно вызывает уменьшение объема опухолей (в среднем на 58%), улучшение стадии заболевания по системе TNM, в 15-28% случаев - достижение полных патологических регрессов опухоли, причем имеется достоверная корреляция между степенью регресса в ходе химиолучевого лечения и общей выживаемостью (Janian N, Crane С., Feig В. et al. Частота местно-региональных метастазов была вдвое меньше у пациентов, получавших предоперационную ХЛТ (р<0.006), количество сфинктерсохраняющих операций (для больных с исходно планируемой брюшно-промежностной экстирпацией) - примерно вдвое больше (р<0.004). Последнее обстоятельство особенно важно при низких локализациях опухолей - при расположении в средне- и нижнеампулярном отделах прямой кишки и анальном канале или на высоте не более 6-8 см от наружного сфинктера. Такая локализация с неизбежностью предполагает в большинстве случаев выполнение травматичной и калечащей операции - брюшно-промежностной экстирпации прямой кишки, вызывающей длительное существенное снижение качества жизни пациентов. Использование химиолучевого лечения (ХЛТ) повышает шансы на выполнение сфинктерсохраняющих операций у части больных, а в ряде случаев (при небольших размерах опухоли, отказе пациента от брюшно-промежностной экстирпации или соматическом состоянии больного, не позволяющем произвести оперативное вмешательство) дает возможность ограничиться консервативным лечением (Habr-Gama A., Perez R.O., Nadalin W. et al. Operative versus nonoperative treatment for stage 0 distal rectal cancer following chemoradiation therapy: long-term results. - Ann. Surg., 2004. - v.240, № 4. - p.711-717). В результате у 122 из 361 пациентов (33,8%), получавших комбинацию лекарственных препаратов (5-Фу+лейковорин) с одновременным проведением дистанционного облучения в режиме обычного фракционирования (СОД 50.4 Гр), был достигнут полный клинический регресс. Данная группа больных не подвергалась хирургическому лечению, при динамическом наблюдении безрецидивное течение на протяжении 12 месяцев сохранялся у 99 больных, после чего авторы отметили развитие эндоректальных рецидивов у 5% больных. Очевидно, что степень местного ответа опухоли можно увеличить повышением дозы облучения опухоли, однако при стандартном дистанционном облучении это существенно усиливает лучевые реакции и повреждения смежных органов и тканей. Предлагаемый метод отличается от прототипа применением конформной внутрипросветной гамма-терапии высокой мощности дозы (брахитерапии) в сочетании с дистанционным облучением очага и внутритазовых лимфоузлов (сочетанной лучевой терапии) и одновременным назначением химиотерапевтического лечения, что обеспечивает более эффективное воздействие при отсутствии увеличения реакций и осложнений лечения.Currently, in Europe, as well as in Russia, there is an increase in the incidence of colorectal cancer while maintaining a fairly high mortality rate for patients. Such localization inevitably involves in most cases traumatic and crippling surgery - abdominal-perineal extirpation of the rectum, causing a long-term significant decrease in the quality of life of patients. После стандартного клинико-диагностического обследования, гистологической верификации заболевания проводили стадирования процесса при помощи магнитно-резонансной томографии, эндоректального ультразвукового исследования. Одновременно начинали лучевое лечение и химиотерапию капецитабином (с 1-го по 50-52 дни курса ежедневно, из расчета 600 мг/м2 в сутки, утром, за 1-1,5 часа до планируемого сеанса облучения, и вечером). Курсовая доза капецитабина составляла 60-70 г капецитабина.Description of the method. С 1-го по 34 дни курса осуществляли дистанционное облучение на медицинских ускорителях электронов SL 75-5 Ех 6 МэВ, ЛУЭВ 15 Ml Ex 15 МэВ, РОД 1,8 Гр, 5 фракций в неделю, всего 25 фракций, суммарная очаговая доза 45 Гр. С 35 по 50-52 дни курса проводили конформную внутрипросветную брахитерапию с источником 192Ir высокой (8-10 Кюри) мощности дозы на аппарате Microselectron Nucletron HDR. В зависимости от протяженности и конфигурации опухоли по данным фиброколоноскопии и магнитно-резонансной томографии производилось дозиметрическое планирование брахитерапии с использованием пакета прикладных программ планирующей системы «Plato Nucletron». Длина траектории источника излучения с шагом 5 мм рассчитывалась, исходя из протяженности поражения кишечной стенки и визуально неизмененной стенки на расстоянии 1,0-2,0 см от дистального и проксимального краев опухоли и определяемого при помощи МРТ и ТРУЗИ опухолевого объема. Разовая доза составляла 3,5 Гр на глубине 10 мм от источника, 3 фракции в неделю через день, всего 4-6 сеансов лечения. A single dose was 3.5 Gy at a depth of 10 mm from the source, 3 fractions per week every other day, a total of 4-6 treatment sessions. The total dose to the focus after completion of the combined exposure was 62-68 Gy.
Способ применен у 17 пациентов с опухолями прямой кишки и анального канала (8 - рак нижнеампулярного отдела прямой кишки T2-4N0M0, 9 - рак анального канала Т2-4N0-1M0). Данная методика лечения хорошо переносилась пациентами, токсических реакций III-IV степени отмечено не было. Осложнений в процессе ХЛТ, требующих прекращения курса или перерыва в лечении, не наблюдалось. Клинический ответ на проводимую по данной методике ХЛТ (частичный+полный) был отмечен в 76,4% наблюдений, в том числе полный клинический регресс у 6 больных (35,3%). Оперативному лечению подверглись 9 пациентов, в среднем спустя 6,5 недель после завершения ХЛТ. При этом в двух случаях, когда эффект лечения клинически расценивался как частичный регресс или стабилизация (у 2 больных), исследование операционного материала показало более существенное изменение клинической стадии заболевания (R1 в обоих случаях). Подведение высоких локальных доз радиационного воздействия за счет присоединения конформной брахитерапии способствует достижению более выраженного уровня регресса опухоли, в том числе - полного клинического регресса новообразования у трети больных. Развитие полного клинического регресса опухоли в ходе химиолучевого лечения является предиктивным фактором, достоверно влияющим на общую и безрецидивную выживаемость пациентов (Roh M.S., Colangelo L., Wieand S. et al. Увеличение суммарной очаговой дозы за счет брахитерапии не сопровождается ростом лучевых реакций и осложнений со стороны органов и тканей малого таза. Одновременное каждодневное использование капецитабина, обладающего собственно цитостатическим и радиосенсибилизирующим действием, усиливает локальный ответ и способствует уменьшению рисков развития отдаленных метастазов. При опухолях анального канала методика ХЛТ с применением сочетанного лучевого воздействия вызывала клиническую ремиссию у 76,3% больных, при этом длительность безрецидивного периода составила в среднем 22 месяца при медиане наблюдения 24,5 месяца.Summing up high local doses of radiation exposure due to the addition of conformal brachytherapy helps to achieve a more pronounced level of tumor regression, including complete clinical regression of the tumor in one third of patients. Больная Б., 51 года, диагноз: рак нижне-ампулярного отдела прямой кишки, T3N0M0, гистологическое исследование биоптата, полученного при фиброколоноскопии, - умеренно дифференцированная аденокарцинома. Проведено химиолучевое лечение: с первого дня курса - прием капецитабина 1050 мг в сутки, дистанционное облучение очага и лимфоузлов малого таза, РОД 1,8 Гр, 5 фракций в неделю (1-29 дни). К концу дистанционного этапа сочетанной лучевой терапии отмечены явления эпителиита II степени, диарея II степени. Лучевые реакции купировались после проведения коррегирующей терапии. С 37 по 52 дни лечения проведена внутрипросветная брахитерапия, разовая доза на глубине 10 мм от источника 3,5 Гр. Суммарная доза на опухоль с учетом предшествующей дистанционной лучевой терапии 62.2-61,0 Гр. Спустя 8 недель после завершения химиолучевого лечения по предлагаемой схеме произведено оперативное лечение (низкая передняя резекция прямой кишки). Окончательное гистологическое заключение: выраженный лечебный патоморфоз, среди полей фиброза отдельные комплексы опухолевых клеток, без метастазов в лимфатических узлах. Патологическая стадия - R1N0. Послеоперационное течение гладкое. В анамнезе: ишемическая болезнь сердца, стенокардия напряжения 2 ф.к., острый инфаркт миокарда в 2005 г., постинфарктный кардиосклероз, мерцательная аритмия, НК 1 ст. Ввиду выраженной патологии сердечно-сосудистой деятельности оперативное вмешательство в объеме брюшно-промежностной экстирпации прямой кишки признано рискованным. Проведено химиолучевое лечение по данной методике, в результате чего достигнут частичный регресс опухоли (уменьшение объема опухоли на 65%). Наблюдался на протяжении 11 месяцев, в течение которых признаков прогрессирования заболевания не отмечалось. Дальнейший контроль утрачен из-за развившегося у больного острого нарушения мозгового кровообращения.Patient P., 67 years old, diagnosis: adenocarcinoma of the lower ampulla of the rectum T3N0M0. Ha фоне перорального ежедневного приема капецитабина (1000 мг/сут) проведено сочетанное облучение (суммарная доза на очаг за счет дистанционного и внутриполостного компонентов лечения составила 66,4-65,2 Гр). В процессе проведения химиолучевого лечения отмечена лейкопения I степени, пальмарно-плантарный синдром I степени. Реакции самостоятельно купировались спустя 3 недели после завершения лечения. Достигнут полный клинический регресс опухоли, подтверждаемый данными МРТ малого таза, биопсией стенки анального канала. Ремиссия сохраняется в течение 21 месяца.Patient K., 63 years old.
