Основанием для выбора схемы химиотерапии в комбинации с лучевой терапией послужили две работы из Японии [1, 2], в которых были приведены высокие непосредственные результаты лечения при невысокой токсичности за счет пролонгированного введения цитостатиков. Авторы вводили неоадъювантно препарат 5-фторурацил в виде непрерывной инфузии в дозе 250 мг/м2, цисплатин 5 мг/м2 за 1 час до проведения лучевой терапии, лейковорин 30 мг/м2. После обследования с целью уточнения стадии заболевания и получения морфологического подтверждения рака грудного отдела пищевода перед началом лечения всем пациентам выполняют катетеризацию подключичной вены по Сельдингеру. Начинают проведение непрерывного (круглосуточного), длительного (на протяжении всего курса дистанционной лучевой терапии) внутривенного введение фторурацила в дозе 300 мг/м2 в сутки с помощью инфузионного насоса. Курсовая доза - 3600 мг/м2. Параллельно с введением фторурацила, один раз в день, ежедневно, внутривенно капельно, в течение 1 часа вводят цисплатин 6 мг/м2 на 400 мл 0,9% физиологического раствора. Курсовая доза - 72 мг/м2. Один раз в день, ежедневно (перед проведением лучевой терапии) внутривенно, струйно в течение 3-5 мин, вводят лейковорин 30 мг/м2. Курсовая доза - 360 мг/м2. Химиотерапию и дистанционную лучевую терапию начинают в один день. В течение 30-60 мин после окончания введения цитостатиков все больные получают дистанционную лучевую терапию на грудной отдел пищевода, параэзофагеальную клетчатку и лимфатические узлы средостения с разовой очаговой дозой 3 Гр, 5 раз в неделю до суммарной очаговой дозы 30 Гр. Через 7-10 суток после окончания лечения проводят контрольное обследование и выполняют хирургическое лечение в объеме субтотальной резекции пищевода. Within 30-60 minutes after the end of the administration of cytostatics, all patients receive remote radiation therapy to the thoracic esophagus, paraesophageal tissue and mediastinal lymph nodes with a single focal dose of 3 Gy, 5 times a week to a total focal dose of 30 Gy. 7-10 days after the end of treatment, a control examination is performed and surgical treatment is performed in the amount of subtotal resection of the esophagus. The total period of neoadjuvant treatment is 21-24 days.
Указанное лечение проведено нами у 27 пациентов раком грудного отдела пищевода, которым в неоадъювантном режиме проведено одновременное химиолучевое лечение предложенным способом, благодаря чему все больные получили лечение в полном объеме, без перерыва, токсических реакций III степени не отмечалось. Снижение токсичности сделало возможным проведение лечения у части пациентов с индексом Карновского = 70%. Сократились сроки лечения с 6-7 недель до 3 недель. Непосредственная эффективность лечения составила 70,4% с учетом стабилизации опухолевого процесса - 100%.The indicated treatment was carried out by us in 27 patients with cancer of the thoracic esophagus, who underwent simultaneous chemoradiotherapy in the neoadjuvant mode of the proposed method, due to which all patients received treatment in full, without interruption, there were no toxic reactions of the third degree. The immediate effectiveness of the treatment was 70.4%, taking into account the stabilization of the tumor process - 100%.

Пример 1. Больной П., 63 лет, клинический диагноз: Рак средней трети грудного отдела пищевода T3N0M0 IIA ст. Дисфагия III ст. Гистологическое заключение - плоскоклеточный рак умеренной степени дифференцировки. По данным обследования протяженность опухоли по пищеводу составила 3 см, мягкотканый компонент при рентгенологическом исследовании 2,5 см. В качестве неоадъювантного химиолучевого лечения пациенту выполнена лучевая терапия в суммарной ОД 30 Гр, параллельно проводилась инфузия цитостатиков: цисплатин 6 мг/м2, лейковорин 30 мг/м2 болюсно, фторурацил в дозе 300 мг/м2 в виде непрерывной инфузии на протяжении всего срока лучевой терапии (12 суток). Осложнение - тошнота II степени. Через 7 суток после окончания химиолучевой терапии выполнено контрольное рентгенологическое и эндоскопическое обследование, которое не выявило наличие опухоли в пищеводе. Еще через 3 дня больному была выполнена субтотальная резекция пищевода с одномоментным формированием пищеводно-желудочного анастомоза на шее. Гистологическое заключение - лечебный патоморфоз IV степени. Больной выписан в удовлетворительном состоянии.Example 1. As a neoadjuvant chemoradiation treatment, the patient underwent radiation therapy in a total OD of 30 Gy, and cytostatics were infused in parallel: cisplatin 6 mg / m 2 , leucovorin 30 mg / m 2 in a bolus, fluorouracil at a dose of 300 mg / m 2 in the form of a continuous infusion throughout the entire duration of radiation therapy (12 days). Complications - nausea of the II degree. 7 days after the end of chemoradiotherapy, a control x-ray and endoscopic examination was performed, which did not reveal the presence of a tumor in the esophagus. After another 3 days, the patient underwent subtotal resection of the esophagus with the simultaneous formation of an esophageal-gastric anastomosis on the neck. The histological conclusion is therapeutic pathomorphosis of the fourth degree. The patient was discharged in satisfactory condition.

Гистологическое заключение - плоскоклеточный рак умеренной степени дифференцировки. По данным обследования протяженность опухоли по пищеводу составила 5 см, мягкотканый компонент при рентгенологическом исследовании 3 см. В качестве неоадъювантного химиолучевого лечения пациенту выполнена лучевая терапия в суммарной ОД 30 Гр, параллельно проводилась инфузия цитостатиков: цисплатин 6 мг/м2, лейковорин 30 мг/м2 болюсно, фторурацил в дозе 300 мг/м2 в виде непрерывной инфузии на протяжении всего срока лучевой терапии (12 суток). Осложнения - тошнота I степени. Через 7 суток после окончания химиолучевой терапии проведено контрольное обследование, которое выявило резорбцию опухоли 80%. Через 3 дня больному была выполнена субтотальная резекция пищевода с одномоментным формированием пищеводно-желудочного анастомоза на шее. Гистологическое заключение - лечебный патоморфоз IV степени. The histological conclusion is therapeutic pathomorphosis of the fourth degree. The patient was discharged in good condition home.
