Abstract
Description
The invention relates to medicine, namely to physiotherapy, and can be used for the rehabilitation of children with multiple sclerosis.
Multiple sclerosis (PC) includes many different symptoms and complications that reduce the quality of life of patients. It has been established that 7-10% of cases of PC debut occur precisely before the age of 18 years, while children have a remitting course of PC, in which there is an alternation of periods of worsening (exacerbation) and improvement (remission), therefore, it is necessary to constantly carry out rehabilitation treatment, even if the patient does not have an exacerbation of the disease.
The most effective way to treat diseases of the central and peripheral nervous system is reflex-segmental massage (Glezer O., Dalikho V.A. Segmentary massage. M., 1965, 1982; Dubrovsky V.I. Basics of segmental-reflex massage. M., 1982 ) in certain areas, sequences and directions. First, massage is performed in the area along the spine from the bottom up, then a massage of the reflex zone corresponding to the disease.
Известен способ лечения неврологических проявлений остеохондроза позвоночника путем воздействия вибрацией на симметричные биологически активные точки, при котором вибрацию осуществляют с частотой 10-15 Гц и одновременно воздействуют электростатическим полем, при этом в первые 3-4 процедуры воздействие осуществляют на точки общего и сегментарного действия отрицательным потенциалом 340-420 в течение 1-2 мин, в последующие процедуры воздействуют на сегментарные и дистальные точки положительным потенциалом 400-480 В, в течение 2-3 мин, в начале курса лечения воздействие осуществляют на 2-3 биологически активные точки, к середине лечения количество точек увеличивают до 5-6, а к концу лечения уменьшают до 3-4, курс лечения составляет 10-12 процедур (Патент РФ №2018295).There is a method of treating neurological manifestations of osteochondrosis of the spine by applying vibration to symmetrical biologically active points, at which the vibration is carried out with a frequency of 10-15 Hz and is simultaneously affected by an electrostatic field, while in the first 3-4 procedures, the effect is exerted on points of general and segmental action by negative potential 340-420 for 1-2 minutes, in the following procedures they act on the segmental and distal points with a positive potential of 400-480 V, for 2-3 minutes, at the beginning of the hens and treatment modality are administered for 2-3 biologically active points to the middle of the treatment the number of points is increased to 5-6 by the end of the treatment is reduced to 3-4, the treatment course is 10-12 procedures (RF Patent №2018295).
Известен способ лечения невралгий путем воздействия на болевые точки и зоны электростатическими потенциалами, вырабатываемыми трибоэлектретами, при котором на больной участок накладывают эластичную пленку с высокой степенью электризации, например полиэтиленовую, и подвергают его вибромассажному воздействию в течение 3-15 мин (Патент РФ №2080850).There is a method of treating neuralgia by acting on pain points and areas with electrostatic potentials generated by triboelectrets, in which an elastic film with a high degree of electrification, for example polyethylene, is applied to the affected area and subjected to vibration massage for 3-15 minutes (RF Patent No. 2080850) .
The methods are individual for each individual patient and are complicated by the layout of reflexogenic zones in a given disease.
СССР №349399).Known massager for exposure to the electrostatic field on the reflex zones of the human body - ebonite disk P.G. Goncharova When ironing the skin with such a disk, negative charges (electrons) pass from it into the body - an electric current of 5-8 μA is formed, which neutralizes the accumulation of positive charges in the body, massage stimulates cell functions, metabolism, has a restoring anti-inflammatory antispasmodic analgesic regenerative effect (Auth. St. USSR No. 349399).
The disadvantage of a disk massager is the low electrostatic potential created by ebonite when rubbing it on human skin.
The method we have chosen as a prototype.
Минздравсоцразвития, НЦ восстановительной медицины и курортологии, http://www.slav-med.ru/catalog/fizio/fiz_physiomed/fiz_hivamat_200.php).Known apparatus Hivamat 200 (Hivamat 200) by the abbreviation of the words in the expression: "hysto-logically variable manual lymph drainage technique", which is based on manual exposure directly over the affected area for pain of various origins. The principle of operation of the apparatus is based on the effect of a pulsating electrostatic field that arises between the patient and the manual applicator, creating a deep resonant vibration of the corresponding area of soft tissues. This is a high-voltage device with a large internal resistance, as a result of which the power of the direct current generated by it does not exceed several microamps. The current is supplied in the form of biphasic pulses. The presence of active discharge in the device is guaranteed to exclude the accumulation of static electricity on the patient’s skin. Therefore, the only and unique acting factor of the procedure is the reciprocating displacement of the entire thickness of the underlying tissues of the patient's body ("deep tissue oscillation" according to the terminology of German authors). They occur during current pulses only in the areas of movement of the applicator: the surface is attracted at the point of contact and released after being pulled to the side. Subjectively, the patient feels vibration. Recent studies have proven the analgesic, antispastic (detonating), decongestant, and trophic-regenerative effects of manual effects using the Khivamat-200 system in the prevention and treatment of various diseases and in the stationary phase of medical rehabilitation after heavy surgical interventions (“Application of the Khivamat 200 system in clinical practice. A manual for doctors ", approved by the Ministry of Health and Social Development, SC of restorative medicine and balneology, http://www.slav-med.ru/catalog/fizio/fiz_physiomed/fiz_hivamat_200.php).
We used this device to massage the reflex zones of the patient's body.
The objective of the invention is to develop a new method of rehabilitation of children with remitting multiple sclerosis.
The technical result of achieving this goal is to reduce the severity of residual neurological symptoms or complete compensation of these symptoms, the prevention of complications; maximum long-term preservation of the patient’s ability to the usual way of life.
The essence of the method of rehabilitation of children with remitting multiple sclerosis is that an electrostatic field is applied over the paravertebral region, sequentially treating the right and left sides with a manual applicator with longitudinal long stroke massage movements with a sequential change in frequencies of 160, 60 and 16 Hz for 3-5 minutes at each frequency, the duration of the procedure is 10 minutes with a 1: 1 mode, the intensity is 50%, stroking, rubbing and kneading.
The applicator is clamped between the index and middle or between the index and thumb and stroke massage movements are performed.
Irritation of skin receptors of reflexogenic zones causes a reaction of the autonomic apparatus associated with exposure to segments of the spinal cord and the corresponding part of the border sympathetic trunk and its autonomic ganglia.
High (80-200 Hz), middle (25-80 Hz) and low (5-25 Hz) frequencies are distinguished. It was experimentally established that high frequencies, perceived as vibration and rapid shaking, contribute to the resorption of seals in the patient’s tissues, anesthesia and activation of lymphatic drainage. These frequencies usually begin the procedure. Low frequencies cause a powerful pumping of fluid in the tissues, vasodilation, detonation of the respiratory tract, activation of venous and lymphatic drainage, stimulate tissue mobility. These frequencies usually complete the procedure, including manual lymphatic drainage. Mid frequencies can be used both to complete the procedure and to affect large areas of the body. They can cause significant detonation of muscles, including blood vessels, which leads to improved blood circulation. The intensity during the procedure is selected according to the principle of minimalism: the lowest value is applied, which still causes deep vibration in the tissues (usually 50-60% is enough). The duration of the procedure directly depends on the nature of the pathology. Chronic processes require the use of all three frequency ranges: 5 minutes - the average frequency (for the entire pathological focus), 5 minutes - high (for the "transition zone" to activate it), 5 minutes - low (for the entire focus for its "pumping" through transition zone).
To confirm the effectiveness of the treatment of children with multiple sclerosis by the claimed method, special metering cards were compiled, in which before and after treatment complaints, clinical manifestations of the disease, medical and physiological interpretation of HRV parameters and analysis of the anti-gravity system were reflected. Before and after the rehabilitation course, the following were assessed: neurological status, using the Kurtzke disability scale (EDSS) and stabilometry (analysis of the anti-gravity system), the state of the autonomic nervous system according to the dynamics of heart rate variability. The analysis of the anti-gravity system consists in identifying the features of the functioning of supraspinal, mainly cerebellar, anti-gravity (AGR) control systems. Under the anti-gravity function is usually understood as a set of mechanisms that ensure the vertical orientation of the human body at rest and when walking. The main “working” mechanism of the anti-gravity system is large, mainly tonic muscles of the legs and back, straightening the spine and regulating the position of the center of mass within the area of the support (feet). The effector control of these muscles (in relation to their AHR functions) is carried out mainly through the cerebellar-spinal paths. If the anti-gravity control system works efficiently, then the time between the appearance of a spontaneous deviation of the patient’s center of mass in the horizontal and sagittal planes is minimal. Therefore, the main parameters characterizing the quality of work of the AGR of the brain system are the median, which characterizes the most probable magnitude of the amplitude of deviations of the human CM and the standard deviation, which describes the spread in the magnitudes of the amplitudes of the CM oscillations. The improvement in the state of work of the AHR of the human system is characterized by a gradual decrease in these values.
A clinical example of the method.
50%.Example 1. Patient O., 16 years old, diagnosis: Multiple sclerosis, remitting course, stage of remission. The patient was treated in accordance with the claimed method, namely 10 procedures for exposing the pulsating electrostatic field over the paravertebral region with long hand strokes (stroking and rubbing) with a manual applicator, with a frequency of 160, 60 and 16 Hz, for 3-5 minutes at each frequency, the duration of the procedure is 10 minutes, with a 1: 1 mode, an intensity of 50%.
In the second or third procedure, a decrease in pain was noted. An increase in the total power of the spectrum of neurohumoral modulation characterizes an improvement in the functional state of the organism.
Thus, the claimed method of rehabilitation of children with multiple sclerosis provides a high therapeutic effect while reducing treatment time from 15-17 to 10-12 days. The method is well tolerated by patients, simple and affordable in rehabilitation centers.
