Изменения ногтевых пластинок негрибковой и грибковой этиологии являются одной из наиболее частых причин обращаемости пациентов к врачу-дерматологу. По ряду вопросов патогенеза, дифференциальной диагностики и лечения ониходистрофий нет единого мнения специалистов различного профиля. Дифференциальная диагностика достаточно сложна и, в первую очередь, необходимо исключить микологическую природу заболевания. Отсутствие лечения, а также неправильная тактика фармакотерапии при ониходистрофии нередко приводит к прогрессированию процесса, значительно снижает качество жизни пациентов.Changes in the nail plates of non-fungal and fungal etiology are one of the most common reasons for patients to contact a dermatologist. В терапии ониходистрофий используется массаж, гидропроцедуры, аппликации масляного раствора ретинола ацетата 3,44%, внутрикожные инъекции триамцинолона в ногтевые валики, локальная ПУВА-терапия, длительный прием ретиноидов, цитостатиков (Фицпатрик Т. и соавт. «Дерматология, атлас-справочник», McGraw-Hill - Практика, 1999, с.89). Необходимость использования данных средств системной терапии псориаза определяется тяжестью процесса, а также приводит к развитию ряда побочных явлений и осложнений (повышение артериального давления, нарушения функции почек, повышение концентрации калия и мочевой кислоты в организме и др.) и характеризуется высокой стоимостью курса терапии (Добрица В.П. и соавт. «Современные иммуномодуляторы для клинического применения», 2001, с.236-238, Кубанова А.А. «Дерматовенерология. Известен также способ лечения одного из вариантов псориатической ониходистрофии - онихолизиса аппликацией кортикостероидных мазей после массажа пораженных пальцев (В.С.Новоселов. - Справочник «Кожные и венерические болезни» под ред. О.Л.Иванова. М.: Медицина, 1997 г., с.181). Доказано иммуномодулирующее действие кальципотриола за счет снижения синтеза интерлейкинов - 1, 2, 6, 8, профильных для активированных при псориазе Т-лимфоцитов. Кальципотриол уменьшает воспалительную реакцию в псориатических бляшках, что выражается существенным уменьшением дермального околососудистого инфильтрата. Околоногтевые валики не изменены. Анамнез: страдает псориатической ониходистрофией с 22 лет с осенне-весенними обострениями. Традиционное лечение с незначительным положительным эффектом. Длительность данного обострения 4 месяца. Проведено традиционное параклиническое обследование, PASI - 23,2, BSA - 45%, дерматологический индекс качества жизни (ДИКЖ) - 19, клиническая оценка зуда - слабо умеренные нарушения, визуально выражен сосудистый компонент высыпаний. Больная К., 39 лет, поступила в стационар с жалобами на распространенные высыпания, сухость кожи, умеренный зуд в течение дня и незначительное шелушение. Анамнез: впервые заболела 11 лет назад после перенесенного психоэмоционального стресса, в последующем ежегодные обострения осенью и зимой. Стационарное лечение проводилось ежегодно, с последующей ремиссией около 2 месяцев. Данное обострение 2-3 месяца, самостоятельное лечение наружными средствами с незначительной положительной динамикой. Проведено традиционное лабораторное обследование, PASI - 22,1, BSA - 38%, ДИКЖ - 20, уровень тревоги средний - 22 балла, в значительной степени обусловленный наличием дистрофической патологии ногтевых пластинок кистей и стоп. Предлагаемый курс лечения позволяет достичь более выраженной клинической эффективности терапии псориатической ониходистрофии, сократить срок пребывания пациента в стационаре, добиться удлинения срока ремиссии за счет воздействия на патогенетически значимый фактор псориаза. Кроме того, предлагаемый способ лечения позволяет проводить лечение больных псориазом как в стационарных, так и в амбулаторных условиях, с явным клиническим эффектом, подтверждаемым клиническими показателями, предотвращая осложнения, сокращая сроки пребывания больных в стационаре и увеличивая сроки ремиссии по сравнению с существующими методами лечения.The proposed course of treatment allows to achieve a more pronounced clinical efficacy of treatment of psoriatic onychodystrophy, to shorten the patient’s stay in hospital, to achieve an extension of the period of remission due to the impact on the pathogenetically significant psoriasis factor.
