Известен способ фиксации катетера у больных с окклюзией желчных протоков путем нанесения на дистальной части катетера углового разреза стенки, фиксированного с помощью лески. При перемещении с помощью остроконечного зажима узелка в сторону проксимального отверстия происходит подъем выреза стенки в дистальной части катетера. Через иглу соответствующего диаметра в просвет желчных путей вводят ангиографический проводник. По игле и проводнику устанавливают катетер. Иглу и проводник извлекают, в катетер помещают цилиндр диаметром 3 мм, длина которого зависит от диаметра желчного протока, катетер с цилиндром продвигают к зоне обструкции в дистальном направлении, выводят из катетера в полость желчного протока. Цилиндр имеет две нити - одна фиксирует центр цилиндра, вторая - один из периферических концов. Обе нити через катетер выводятся наружу. Под рентгенологическим контролем, убедившись, что цилиндр полностью находится в протоке, осуществляют подтягивание центральной части и устанавливают его перпендикулярно катетеру и желчному протоку. Концы нитей фиксируют на наружной части катетера. По истечении необходимости катетеризации удаляют цилиндр, освобождая обе нити от фиксации, ослабляют центральную нить. Оценивают топографию внутрипеченочных трубчатых структур, взаиморасположение внутрипеченочных желчных протоков и ветвей портальной и печеночной вен, определяют положение желчного пузыря, правого плеврального синуса, толстой кишки. Выясняют уровень обструкции, причину блока желчных путей. Выбирают оптимальную траекторию пункции желчных путей, проходящую через кожу в 7-8-9 межреберьях. Выполняют пункцию и контрастирование желчных протоков, оценивают уровень блока и внутренний диаметр желчевыводящих путей. Через пункционную иглу вводят ангиографический проводник. По игле и проводнику устанавливают катетер. Иглу и проводник извлекают, в катетер помещают цилиндр диаметром 3 мм, длина которого зависит от диаметра желчного протока, катетер с цилиндром продвигают как можно ближе к зоне обструкции и выводят цилиндр в просвет желчного протока. Цилиндр имеет две нити - одна фиксирует центр цилиндра, вторая - один из периферических концов. Обе нити через катетер выводятся наружу. Выполнив рентгеновское исследование и убедившись, что цилиндр полностью находится в протоке, осуществляют подтягивание нити, фиксированной к центральной части, устанавливая его перпендикулярно катетеру и желчному протоку. Нити фиксируются к проксимальногй части катетера. После истечения необходимости дренирования или необходимости изменения положения катетера цилиндр удаляют путем освобождения обеих нитей от фиксации, ослабляют центральную нить. Под контролем рентгена осуществляют тракцию периферической нити, цилиндр удаляют из просвета протока. Иглу и проводник извлекли, в катетер поместили цилиндр диаметром 3 мм, длиной 8 мм, катетер с цилиндром передвинули к зоне обструкции в дистальном направлении, вывели из катетера в полость желчного протока. Цилиндр имеет две нити - одна фиксирует центр цилиндра, вторая - один из периферических концов. Обе нити через катетер вывели наружу. Под рентгенологическим контролем, убедившись, что цилиндр полностью находится в протоке, осуществили подтягивание центральной части и установили его перпендикулярно катетеру и желчному протоку. Концы нитей фиксировали на наружной части катетера.
