SUBSTANCE: invention refers to medicine and may be used in retrograde X-ray endoscopic diagnostic and therapeutic procedures. A device consists of a catheter and a built-in flexible rod. The catheter and rod are mounted in a working channel of the endoscope. The endoscopic catheterisation of the deformed sinuate bile ducts is enabled through a duodenum and accompanied by transpapillary X-ray diagnostic and therapeutic interventions with using a catheter of a graduated internal diameter to be lowered in one point provided with an elastic cuff peripherally fixed in a lowering point and having a hole formed in the centre of lowering. The provided flexible rod having on its distal side is equipped with a calibrated rod, a drill and a cone to cover the hole in the graduated internal diameter of the catheter with making a contrast substance flowing into the hole onto the catheter in the centre of lowering of the extended cuff to an internal diameter of a choledoch duct by pressure of the contrast substance and providing the contrast substance outflow closing into the duodenum. Further, the flexible rod opens the contrast substance passage by the cone through the catheter under pressure into the deformed bile duct. From the proximal side of the catheter, there is a control handle with a port, the in-built rod with a handle with an attached drill on the distal end for the purpose of flexibility.
EFFECT: facilitated contrast of the deformed bile ducts in transpapillary X-ray diagnostic and therapeutic interventions, provided optimal conditions for catheterisation of the deformed duct strictures; reducing a radiation dose for the patient and medical personnel, enhancing minimally invasive surgery; ensuring economic effect due to contrast substance saving.

Развитие малоинвазивных рентгенэндоскопических методик диагностики и лечения, в том числе при неопластических поражениях органов гепатопанкреатодуоденальной зоны, что часто сопряжено с развитием деформации и сужением желчных протоковых структур, не позволяет выполнять эндоскопические вмешательства, направленные на внутреннюю декомпрессию желчных протоков. При затруднении проходимости контрастного вещества через деформированные структуры и суженный просвет желчных протоков, особенно при выполненной у больного эндоскопической папиллосфинктеротомии, возникает сброс контрастного вещества в двенадцатиперстную кишку, тем самым затрудняется визуализация под контролем рентгеновского излучения, что препятствует безопасной катетеризации извилистых стриктур струной направителем, а также требует повышенного расхода контрастных веществ и значительно увеличивает лучевую нагрузку на пациента и медицинский персонал. Сущностью предлагаемого изобретения «Устройство для эндоскопической катетеризации и контрастирования деформированных желчных протоков при выполнении чрезпапиллярных рентгендиагностических и лечебных вмешательств» заключается в том, что катетер, расположенный в канале эндоскопа, выполнен с занижением диаметра и в одном месте с калиброванным внутренним диаметром. По центру занижения диаметра в катетере предусмотрено отверстие. В месте занижения по наружному краю катетера закреплен эластичный охватывающий манжет. Расположенный внутри катетера гибкий стержень на дистальном конце оснащен буравчиком, конусом и калиброванным штоком с канавкой для прохода контрастного вещества (КВ); при затруднении прохода КВ в деформированных извилистых структурах желчных протоков и сбросе КВ в двенадцатиперстную кишку конусом на гибком стержне перекрывается калиброванный внутренний диаметр катетера и КВ через отверстие в катетере поступает в манжет, который при этом увеличивается до внутреннего диаметра холедоха, препятствуя сбросу КВ в двенадцатиперстную кишку, затем калиброванным штоком перекрывается выход КВ из манжета, открывается конус и КВ с повышенным давлением поступает в деформированный желчный проток, путем вращения рукоятки управления с портом для введения КВ, расположенной с проксимального конца катетера, а также вращения расположенного в основании рукоятки штока с закрепленным к нему гибким стержнем для обеспечения маневренности на дистальном конце буравчика облегчается прохождение КВ и проводников в деформированных и извилистых структурах желчных протоков.The essence of the invention “Device for endoscopic catheterization and contrasting of the deformed bile ducts when performing extra-capillary X-ray diagnostic and therapeutic interventions” lies in the fact that the catheter located in the channel of the endoscope is made with underestimation of diameter and in one place with a calibrated inner diameter. К проксимальному концу катетера-1 в месте-18 крепления рукоятки-15 с основанием-17, портом подвода КВ в катетер-1, в основание-17 устанавливается шток-19 с рукояткой-21 и винтом-20 крепится проксимальный конец гибкого стержня. По каналу эндоскопа-2 катетер-1 через двенадцатиперстную кишку-13 вводится в холедох-14, катетер-1 с рукояткой-15 проводится в проток структуры желчных путей-22. При выполнении рентгена диагностики подается КВ. При затруднении прохождения КВ и сбросе КВ в двенадцатиперстную кишку-13 посредством рукоятки-21 закрепленный гибкий стержень совместно со штоком-19 выполняет перекрытие конусом-10 калиброванного внутреннего диаметра-4, катетер-1 перекрывает прохождение КВ и через отверстие-5 КВ поступает в эластичный манжет-7, увеличивает его до размера внутреннего диаметра холедоха-7. Калиброванным штоком-11 посредством рукоятки-21 перекрывается отверстие-5. При этом повышается давление KB в катетере-1, что способствует прохождению KB по протоку структуры желчных путей-22. Устройство для эндоскопической катетеризации и контрастирования деформированных желчных протоков при выполнении чрезпапиллярных рентгендиагностических и лечебных вмешательств состоит из установленных в рабочий канал эндоскопа катетера и расположенного внутри него гибкого стержня, отличающееся тем, что для выполнения через двенадцатиперстную кишку эндоскопической катетеризации деформированных извилистых структур желчных протоков с выполнением чрезпапиллярных рентгендиагностических и лечебных вмешательств катетер выполнен с калиброванным внутренним диаметром, с занижением диаметра в одном месте, с охватывающим эластичным манжетом, закрепленным по краям в месте занижения с отверстием по центру занижения, и предусмотрен гибкий стержень, на дистальной стороне оснащенный калиброванным штоком, буравчиком и конусом для закрытия отверстия в калиброванном внутреннем диаметре катетера, обеспечением при этом прохода контрастирующего вещества KB в отверстие на катетер по центру занижения, расширения манжета до диаметра внутреннего диаметра холедоха давлением KB, обеспечением закрытия оттока KB в двенадцатиперстную кишку, в последующем гибким стержнем посредством конуса открывается проход KB через катетер под давлением в деформированный желчный проток, а с проксимальной стороны катетера предусмотрена рукоятка управления с портом, расположенным внутри нее штоком с рукояткой с закрепленным к нему гибким стержнем для обеспечения маневренности, на дистальном конце буравчиком.
