Способ осуществляют следующим образом. Пациента укладывают в положение "на спине". Больную ногу укладывают на валики в положение наружной ротации 10-15°, коленный сустав сгибают до 160-165°. Обрабатывают операционное поле и производят разрез из точки 5 см краниальнее внутренней лодыжки и отступя 2 см дорсально. Рассекают мягкие ткани до кости, огибая внутреннюю лодыжку и отступя от ее полюса 2 см, после чего разрез направляют к ладьевидной кости, где и заканчивают. Рассекают дельтовидную связку, локализуют сухожилия, нервы, экзостоз и фиксированные к нему анатомические образования.The method is as follows. The patient is placed in the “on the back” position. При ревизии выявляют экзостоз медиального отдела пяточной кости и фиксированные к нему рубцами задний большеберцовый нерв и сухожилие длинного сгибателя 1-го пальца стопы. Отмечают степень внедрения костной ткани в ткань нерва и сухожилия. Попеременно тупым и острым путем осуществляют отделение нерва и сухожилия от экзостоза, по показаниям проводят тенолиз сухожилия или невролиз заднего большеберцового нерва. Следующим этапом удаляют экзостоз, основание экзостоза шлифуют и затирают воском. Освобожденные сухожилие и нерв укладываются в анатомически правильное положение. Больной Б., 25 лет, история болезни №1572/02, при падении с высоты 2 метров получил оскольчатый чрессуставной перелом правой пяточной кости со смещением фрагментов 15.07.01. Получал лечение скелетным вытяжением в течение 2 недель, затем методом внешней иммобилизации гипсовой повязкой до 2-х месяцев. Осмотрен на амбулаторном приеме 05.10.01 с жалобами на боль, деформацию заднего отдела стопы, нарушения чувствительности подошвенной части стопы, невозможность разгибания 1-го пальца (фиг.1).An example of a specific application. При клинико-рентгенологическом обследовании у пациента выявлен консолидированный в порочном положении перелом пяточной кости, разгибательная контрактура 1 пальца правой стопы в положении порочной фиксации пальца 10° подошвенной флексии, полная анестезия подошвы. В глубине тканей медиального отдела пяточной кости пальпировалось образование костной плотности 0,5×0,5 см. Отмечено, что традиционная рентгенография была неэффективна для диагностики морфологического субстрата данного повреждения (фиг.2), и пациенту проводилась компьютерная томография, с помощью которой выявлен морфологический субстрат патологии: медиальный проминирующий фрагмент левой пяточной кости, перелом внутренней стенки пяточной кости справа без смещения фрагментов (фиг.3).Clinical and radiological examination revealed a fractured heel bone consolidated in a vicious position, extensor contracture of 1 toe of the right foot in a position of vicious fixation of a finger of 10 ° plantar flexion, complete anesthesia of the sole. После проведения диагностических мероприятий и уточнения диагноза больной был оперирован. Пациенту был проведен корригирующий подтаранный артродез по Carr, внутренняя фиксация винтами типа А\О из внутреннего доступа. Под общим обезболиванием проведен разрез мягких тканей, был визуализирован экзостоз, осуществлено отделение нерва и сухожилия от него и удален медиальный проминирующий экзостоз величиной 2×3×1 см, проведены тенолиз длинного сгибателя 1-го пальца и невролиз заднего большеберцового нерва. Основание экзостоза зашлифовано и обрабтано воском. Освобожденные сухожилие и нерв уложены в анатомически правильном положении. Рана ушита послойно. Заживление первичным натяжением к 10-му дню после операции. После снятия швов Больному наложена циркулярная гипсовая повязка в среднефизиологическом положении стопы, до коленного сустава. Начиная с 3-го дня после операции пациент отметил отчетливое улучшение чувствительности подошвенной части стопы и увеличение объема пассивных и активных движений 1-м пальцем оперированной стопы. Клинические признаки восстановления утраченных в результате травмы функций нарастали, и при осмотре через 8 месяцев после операции отмечалось восстановление чувствительности подошвенной части стопы и активная тыльная флексия пальца до 13°. По данным контрольных рентгенограмм от 24.04.02 артифициальный подтаранный костный анкилоз в правильном положении, перестройка трансплантатов, незначительные дегенеративные изменения в костях и суставах стопы. Starting from the 3rd day after the operation, the patient noted a clear improvement in the sensitivity of the sole of the foot and an increase in the volume of passive and active movements with the 1st finger of the operated foot. Clinical signs of the restoration of the functions lost as a result of the trauma increased, and upon examination 8 months after the operation, restoration of the sensitivity of the sole of the foot and active back flexion of the finger to 13 ° were noted. According to the control radiographs dated 04.24.02, artifactal subtalar bone ankylosis in the correct position, transplant remodeling, minor degenerative changes in the bones and joints of the foot. The result is regarded as good.
