Кишечные швы, особенно при формировании многорядного анастомоза, сами по себе представляются "интрамуральными сосудистыми жгутами", значительно снижающими кровоснабжение, соответственно и дальнейшую репарацию тканей (Сигал З.М. с соавт., 2000).The invention relates to medicine, namely to surgery. В процессе постнатального онтогенеза организм приобретает наиболее совершенный механизм компенсации нарушенного кровоснабжения тканей в виде коллатерального кровотока. Высокий тонус сосудов, обусловленный стрессовой активацией гипофизарно-надпочечниковой системы, носит, хотя и кратковременный, компенсаторно-приспособительный характер. Он направлен на увеличение внутрисосудистого перфузионного давления для преодоления критического давления замыкания капилляров и обеспечения адекватной перфузии тканей (Оппель В.А., 1928). В условиях метаболического ацидоза, обусловленного недостаточной васкуляризацией, в тканях накапливаются неспецифические метаболиты, местные гормоны. Они регулируют транскапиллярный кровоток, стимулируя гладкую мускулатуру, расширяя мелкие сосуды и улучшая "шунтирующие пути кровотока" (Шабанов А.Н., Котельников В.П., 1983; Гонджиашвили В.Г., 1992; Пучков К.И. с соавт., 97; Lewis G.P. 1960; Russe R, 1978; Richard H.С., 1984).In the process of postnatal ontogenesis, the body acquires the most advanced mechanism for compensating for impaired blood supply to tissues in the form of collateral blood flow. Поставленная цель достигается при помощи аппарата по авторскому свидетельству №1442194 от 08.08.88 г. Под контролем пульса, А/Д и дыхания за 5 дней до операции начинают передавливание брюшной аорты в проекции выше места бифуркации аорты; в первые 2 дня - частично с сохранением пульсации бедренных артерий, начиная с 3 дня, - до исчезновения их пульсации. В послеоперационном периоде в течение 5-7 дней перелавливают одновременно обе бедренные артерии на фоне эластического бинтования нижних конечностей. В день проводятся по 4 сеанса через равные промежутки времени, каждый сеанс состоит из 6 процедур. Эффективность предлагаемого способа интенсификации гемоциркуляции шовной полосы для защиты межкишечного анастомоза определялась его апробацией на экспериментах на животных с моделью толстокишечной непроходимости по результатам динамических морфофункциональных исследований стенки кишки в области анастамоза (витальная биомикроскопия, ангиотензиометрия, токопроводимость, гистологическое исследование).
