Известен способ аллопластики раневой поверхности печени с использованием аллотрансплантатов - нейлон, лавсан (Богославский Г.В. и др. Аллопластика при резекции печени. - Хирургия, 1964, №3, с.9). Суть способа (аналог) состоит в том, что ткань используется для укрепления гемостатического шва, проведенного через всю толщу паренхимы печени и аллотрансплантат. Шов глубиной 1,5-2 см. Швы множественные, на всю протяженность раны, располагаются не строго линейно, чаще в шахматном порядке. Это позволяет накладывать швы в неповрежденных раневым снарядом участках, распределять силу натяжения по всей поверхности печени. Швы накладываются на глубину 1,5-2 см, располагаются не строго линейно, в местах не травмированных воздействием ранящего снаряда. При наличии значительного раневого дефекта, когда после подшивания одного края сетки, не удается свести раневые края, дефект тампонируется большим сальником на ножке путем подведения к ране без, отсечения его от основной части органа; устанавливается трубчатый дренаж; края раны максимально сводятся; после чего второй край сетки подшивается множественными П-образными швами с противоположной стороны на расстоянии не менее 5 см от края раны. Производится фиксация тампонированного в ране сальника к синтетической ткани узловыми швами. Возможно использование вместо сальника биологического гемостатического препарата.The method is as follows. Рана тампонирована большим сальником на ножке, края раны сближены. Свободный край сетки подшит к диафрагмальной поверхности печени 4,3 сегментов П-образными швами не ближе 5 см к ране. Отдельными швами тампонированный сальник фиксирован к сетке. По достижении гемостаза печени произведена резекция желудка по типу Бильрот-1 с наложением гастродуоденоанастомоза, резекция тонкой кишки с наложением энтероэнтероанастомоза “конец в конец”. Послеоперационный период протекал без воспалительных и геморрагических осложнений. На 16-е сутки выписана на амбулаторное лечение.Example.
