Определив наружное отверстие пахового канала через кожу, в нее вводят иглу, которую продвигают через толщу круглой маточной связки вплоть до ее прикрепления к матке. Свободный конец микроирригатора, оставшегося в полости малого таза, продвигают по игле и выводят через наружное отверстие пахового канала таким образом, что один конец микроирригатора находится во влагалище, а второй выведен на кожу в области наружного отверстия пахового канала. Аналогичным образом с противоположной стороны происходит прокалывание стенки матки в области ее угла у места прикрепления круглой маточной связки до полости матки, куда игла как бы "проваливается". Вторым концом микроирригатор проводится через круглую маточную связку таким образом, что один конец микроирригатора выведен на кожу в паховой области, а второй расположен в полости матки. Конец микроирригатора, расположенного во влагалище, соединяют с запирающим приспособлением. Далее, через конец первого катетера, находящегося в паховой области, вводят противовоспалительные антибактериальные препараты. Противовоспалительный препарат попадает в обе маточные трубы и через них происходит гидротубация с профилактической целью для предупреждения слипчивых процессов маточных труб, бесплодия. При проведении длительного контактного лечения современные микроирригаторы могут находиться в полости матки так долго, как это необходимо для проведения всего комплекса лечения. Кроме того, замкнутое пространство полости матки, обеспеченное запирающим приспособлением, создает условие для концентрации препарата, исключает его истечение во влагалище. Пример конкретного выполнения "Способа дренирования полости матки"
Больная К. . . ист. болезни 2438/н, 29 лет поступила в гинекологическое отделение Ростовского НИИ онкологии с диагнозом: трубное бесплодие. Из анамнеза: роды - 0, искусственный аборт - 1 в возрасте 22 лет, хр. аднексит, аппендэктомия в возрасте 18 лет. Диагноз трубного бесплодия установлен на основании данных анамнеза, а также по данным гистеросальпингографии и лапароскопии с одновременной хромопертубацией. По классификации Hulka установлена II ст. спаечного процесса. Больной было выполнено хирургическое вмешательство в объеме фимбрио- и сальпинголизис, после чего полость матки была дренирована через круглые маточные связки по предлагаемому способу: стенка матки прокалывается в области ее угла у места прикрепления круглой маточной связки до полости, куда игла "проваливается" и это ощущение говорит о том, что цель достигнута. Далее игла проходит через цервикальный канал и через наружное отверстие цервикального канала выходит во влагалище. По игле проводится микроирригатор. Игла удаляется. Пальпаторно на коже определяют развилку круглой маточной связки. Определяют наружное отверстие пахового канала через кожу, в нее вводят иглу, которую продвигают через толщу круглой маточной связки вплоть до ее прикрепления к матке. Свободный конец микроирригатора, оставшегося в полости малого таза, продвигают по игле и выводят через наружное отверстие пахового канала таким образом, что один конец микроирригатора находится во влагалище, а второй выведен на кожу в области наружного отверстия пахового канала. Аналогичным образом с противоположной стороны происходит прокалывание стенки матки в области ее угла у места прикрепления круглой маточной связки до полости матки, куда игла как бы "проваливается". Далее в послеоперационном периоде проводились гидротубации с применением противовоспалительных препаратов, витаминов следующим образом: конец микроирригатора, расположенного во влагалище, соединяют с запирающим приспособлением конусовидной формы и подтягиванием за край микроирригатора, находящегося в паховой области, перекрывают наружное отверстие цервикального канала. Далее через конец первого микроирригатора, находящегося в паховой области, вводят противовоспалительные препараты в полость матки. Препарат попадает в обе маточные трубы и через них происходит гидротубация с профилактической целью для предупреждения слипчивых процессов маточных труб, бесплодия. Больная выписана в удовлетворительном состоянии, наблюдалась. Через 3 месяца больная забеременела. Течение беременности без осложнений.
