Решение проблемы хирургического лечения больных с разнообразной патологией позвоночника тесно связано с разработкой рациональных доступов. В этом отношении наиболее сложными являются смежные отделы позвоночного столба, к числу которых относится и пояснично-крестцовый. Трудность оперативного доступа к последнему обусловлена его глубиной, а также расположением на уровне крестцового мыса бифуркации крупных сосудов - аорты, нижней полой вены и их ветвей. Несмотря на то, что первая операция переднебоковым забрюшинным доступом выполнена в 1931 г. В.Д. Чаклиным и существует много модификаций забрюшинных доступов к вышележащим поясничным позвонкам, проблема оперативных подходов к пояснично-крестцовому сегменту остается не решенной. Хирургические доступы, применяемые при операции на пояснично-крестцовом отделе позвоночника, не создают оптимальных условий для декомпрессии корешков конского хвоста на этом уровне. A semi-oval incision of the skin, subcutaneous tissue and superficial fascia with a bulge down is performed along the intended line. The spread of the skin incision beyond the lateral edges of the rectus abdominis muscles is not necessary, because skin easily shifts. After exposure of the aponeurosis, it is dissected in the transverse direction so that the incision does not exceed 2 cm to the right and left of the white line of the abdomen. Next, the aponeurosis is separated in a blunt way to both sides of the underlying rectus abdominis muscles. Dissection of the aponeurosis is prolonged with a semi-oval incision to the right and left, respectively, of the skin incision line with the approach under the skin flap vertically upwards by 3-4 cm, because the skin in this area is very mobile. This allows you to further create maximum surgical access to the lumbosacral spine. In an acute way, the aponeurosis is cut off along the midline of the abdomen from the transverse fascia. The aponeurosis thus cut off has a wedge shape with a rounded end and a base 2-3 cm below the umbilical ring. The rectus abdominis muscle on the right or left stupidly exfoliates from the white line of the abdomen and the underlying transverse fascia along with the lower epigastric artery and vein, if the pathological process (tumor, inflammation, bone proliferation) affects the lumbosacral vertebrae on one side, then only this side. Then, the rectus abdominis muscle along with the lower epigastric artery and vein is pulled laterally and anteriorly. From beneath it, focusing on the lateral iliac portions, the fibers of the transverse fascia are bluntly pushed apart in the area of the lateral portions of the iliac fossa and access is made into the retroperitoneal space. Along the anteromedial edge of the large lumbar muscle and the external iliac artery and vein, an approach is made to the lumbosacral spine. The parietal peritoneum is exfoliated beyond the midline and, together with the ureter, is displaced medially, together with the peritoneum, the midline rectum is displaced. The spermatic cord and its neurovascular bundle in men and the round ligament of the uterus in women are displaced downward and medially. Pelvic organs do not interfere with manipulations on the spine. After displacement of the peritoneal sac, an area of the sacral cape becomes covered with an intraperitoneal fascia and fatty tissue. Between the right and left common iliac arteries and veins in the projection of the L5-S1 disk, the intra-abdominal fascia and adipose tissue are bluntly stratified until a longitudinal ligament of the spine appears. As a result of this, the area bounded by bifurcations of the aorta and inferior vena cava above, the left common iliac vein and the right common iliac artery on the sides is exposed. The lower half of the L5 vertebral body, the L5-S1 disk and the anterior surface of the sacrum up to the 3rd sacral vertebra become clearly visible. The median sacral vessels, the artery and the two veins accompanying it, are usually small in diameter, they cross, and in the future they do not interfere with manipulations on the spine. Dissect the anterior longitudinal ligament along the axis of the spine and separate it from the bodies of the underlying vertebrae to the sides, which leads to additional mobilization of the left iliac vein, flaps of the separated ligament provide protection of nearby vessels from damage by instruments. To decompress the cauda equina roots, a hole is drilled to the spinal canal with a diameter of 20-40 mm. Compressing substrate is removed with bone spoons, an audit is made of the contents of the spinal canal. An implant is inserted between the bodies of the L5 and S1 vertebrae.

Больной Н., 28 лет, поступил в нейрохирургическую клинику ГКБ 29 г. Новокузнецка 15.10.01 г. по поводу спондилолистеза тела L5 позвонка 3 степени. Patient N., 28 years old, was admitted to the neurosurgical clinic of the City Clinical Hospital 29, Novokuznetsk 10/15/01, about spondylolisthesis of the body L5 vertebra 3 degrees.
При клиническом исследовании выявлен выраженный поясничный лордоз с ограничением движений и болезненностью в поясничном отделе позвоночника, снижение правого ахилова рефлекса и гипостезия в дерматоме S1 справа. 17.10.01 г. произведена операция: в положении больного на спине с валиком под пояснично-крестцовым отделом позвоночника после обработки кожи антисептиком по намеченной линии скальпелем выполнили в надлобковой области полуовальный поперечный разрез кожи, подкожной клетчатки и поверхностной фасции. Апоневроз рассекли в поперечном направлении вправо и влево от белой линии живота на 2 см, затем тупым путем его отсепаровали в обе стороны от подлежащих прямых мышц живота. Рассечение апоневроза продлили полуовальным разрезом вправо и влево соответственно линии кожного разреза с заходом под кожный лоскут вертикально вверх на 3-4 см. Ножницами апоневроз отсекли по средней линии живота от поперечной фасции. Отсеченный таким образом апоневроз имел форму клина с закругленным концом и основанием, расположенным на 2-3 см ниже пупочного кольца. Прямую мышцу живота справа тупо тупфером отслоили от белой линии живота и подлежащей поперечной фасции вверх и вниз вместе с нижней надчревной артерией и веной. Затем правую прямую мышцу живота вместе с нижней надчревной артерией и веной крючками оттянули латерально и кпереди. Из под нее, ориентируясь по направлению к боковым отделам подвздошной кости, тупо тупфером раздвинули волокна поперечной фасции в области латеральных отделов подвздошной ямки и осуществили доступ в забрюшинное пространство. По переднемедиальному краю большой поясничной мышцы и наружной подвздошной артерии и вены осуществили подход к пояснично-крестцовому отделу позвоночника. Париетальную брюшину отслоили за среднюю линию и вместе с мочеточником сместили медиально, при этом вместе с брюшинной сместили срединнорасположенную прямую кишку. Семенной канатик и его сосудисто-нервный пучок сместили вниз и медиально. После смещения брюшинного мешка стала видна область крестцового мыса. Между правыми и левыми общими подвздошными артериями и венами в проекции диска L5-S1 тупо тупфером расслоили внутрибрюшную фасцию и жировую клетчатку до появления продольной связки позвоночника. Срединные крестцовые сосуды, артерию и сопровождающие ее две вены, пересекли. Рассекли скальпелем переднюю продольную связку по оси позвоночника и распатором отделили ее от тел подлежащих позвонков в стороны. Это привело к дополнительной мобилизации правой подвздошной вены, лоскуты отделенной связки обеспечили защиту близлежащих сосудов от повреждения инструментами. Скальпелем рассекли и кюреткой удалили диск L5-S1, корончатой фрезой с надежным ограничителем ее погружения, в смежных отделах тел L5 и S1 позвонков сформировали паз до позвоночного канала. Костной ложкой под визуальным контролем удалили грыжу диска. Корешок и дуральный мешок расправились. В межтеловой паз вкрутили пористый имплантат из никелида титана. Тщательный гемостаз. На края прямых мышц живота наложили кетгутовые швы. Разрез апоневроза зашили отдельными лавсановыми швами. Подкожную клетчатку и кожу ушили отдельными узловыми швами. Then, the right rectus abdominis muscle along with the inferior epigastric artery and vein was hooked laterally and anteriorly. From underneath, orienting towards the lateral ilium, the fibers of the transverse fascia in the area of the lateral portions of the iliac fossa were bluntly tuffed apart and accessed into the retroperitoneal space. Along the anteromedial edge of the large lumbar muscle and the external iliac artery and vein, an approach was made to the lumbosacral spine. The parietal peritoneum was exfoliated beyond the midline and, together with the ureter, was displaced medially, while the midline rectum was displaced along with the peritoneum. The spermatic cord and its neurovascular bundle were displaced downward and medially. After displacement of the peritoneal sac, the area of the sacral cape became visible. Between the right and left common iliac arteries and veins in the projection of the L5-S1 disk, the intra-abdominal fascia and adipose tissue stupidly stratified until the longitudinal ligament of the spine appeared. The median sacral vessels, the artery and the two veins accompanying it, crossed. We dissected the anterior longitudinal ligament with a scalpel along the axis of the spine and separated it from the bodies of the underlying vertebrae with a raspator. This led to additional mobilization of the right iliac vein, the flaps of the separated ligament provided protection of nearby vessels from damage by instruments. They dissected with a scalpel and removed the L5-S1 disk with a curette, with a crown milling cutter with a reliable limiter for its immersion, in the adjacent sections of the L5 and S1 vertebral bodies formed a groove to the spinal canal. A bone hernia was removed with a bone spoon under visual control. The spine and the dural sac were dealt with. A porous titanium nickelide implant was screwed into the interbody groove. Careful hemostasis. Catgut sutures were placed on the edges of the rectus abdominis muscles. The aponeurosis incision was sutured with separate lavsan sutures. Subcutaneous tissue and skin were sutured with separate interrupted sutures.
Операция прошла без осложнений. Кровопотеря незначительная. Послеоперационное течение гладкое. На следующий день больной поднят на ноги, самостоятельно передвигался. Боли в поясничном отделе позвоночника и в правой ноге не беспокоили. Швы сняты на 7 день. Выписан из клиники на 8 день после операции. The operation went without complications. Blood loss is negligible. The postoperative course is smooth. The next day, the patient raised to his feet, moved independently. Pain in the lumbar spine and in the right leg did not bother. Sutures were removed on day 7. Discharged from the clinic on the 8th day after the operation.
Объективные оперативные критерии разработанного нами переднего надлобкового забрюшиннго доступа и его прототипа (переднебокового забрюшинного доступа) представлены в таблице. Из таблицы видно, что разработанный нами передний надлобковый забрюшинный доступ в позвоночный канал на пояснично-крестцовом уровне имеет существенные преимущества перед распространенным переднебоковым забрюшинным доступом. В предложенном нами доступе статистически достоверно стала меньше глубина раны (7,5 см вместо 10 см), увеличился угол наклонения оси операционного действия почти до прямого, что значительно облегчило манипуляции в позвоночном канале. Длина раны значительно уменьшилась с 18 до 11,5 см, что свидетельствует об уменьшении травматичности разработанного нами доступа. The length of the wound decreased significantly from 18 to 11.5 cm, which indicates a decrease in the morbidity of the access developed by us.
С применением данного способа в клинике выполнено 16 операций у взрослых людей женского и мужского пола в возрасте от 21 до 42 лет. У 11 больных со спондилолистезом тела L5 позвонка, у 5 с грыжей диска L5-S1. Все пациенты выписаны с выздоровлением. Using this method, the clinic performed 16 operations in adult women and men, aged 21 to 42 years. In 11 patients with spondylolisthesis of the L5 vertebral body, in 5 with L5-S1 disc herniation. All patients were discharged with recovery.
Таким образом, оперативное лечение патологии пояснично-крестцового отдела позвоночника у больных с выполнением полуовального переднего надлобкового забрюшинного доступа позволяет осуществить спондилодез сегмента L5-S1 или декомпрессивно-стабилизирующую операцию на этом уровне, уменьшить операционную травму, рано активизировать пациента и сократить сроки пребывания пациента в стационаре.
