Оссифицирующий периостит (пяточная шпора) в области прикрепления подошвенного апоневроза к пяточному бугру чаще встречается у лиц среднего возраста, страдающих продольным плоскостопием, и проявляется болями по медиально-подошвенной поверхности пяточного бугра вследствие воспаления окружающих шпору мягких тканей, травмируемых костным экзостозом при нагрузке на стопу (околошпорная сумка, ткани подошвенного апоневроза, фасции, надкостница и т. д.) [1]. Аналогами предлагаемому нами методу служат традиционные способы лечения пяточной шпоры - ограничение физической нагрузки, физиопроцедуры, тепло. При неэффективности этих видов лечения назначается рентгенотерапия, местное введение стероидов [1, 2]. Однако на фоне такой терапии болевой синдром может сохраняться много месяцев, а иногда и лет, что резко ухудшает качество жизни пациентов. Прототипом методики, предложенной авторами, является закрытая компрессия пяточной шпоры [3] , когда под внутривенным наркозом производится закрытая компрессия пяточной шпоры тремя-четырьмя ударами молотка по металлической насадке (диаметр на конце 2 см), которая плотно прижимается к коже в болезненной точке в положении максимального подошвенного сгибания и разгибания пальцев. После манипуляции на рентгенограмме обнаруживается перелом шпоры, более часто у острого конца ее. Однако подобное вмешательство травматично. Ввиду того, что манипуляция производится закрыто, происходит разможжение мягких тканей находящихся между шпорой и металлической насадкой, а также повреждение костной структуры шпоры (перелом), что увеличивает сроки восстановительного лечения. Предлагаемый нами способ менее травматичен т.к. манипуляция производится под местной анестезией, не травмируя мягкие ткани, ввиду того, что пункционно-перфоративное и термическое воздействие оказывается непосредственно на патологический очаг в течение 10 секунд, через световод, проходящий внутри иглы. Очаг воспаления находится пальпаторно, по месту наибольшей болезненности соответствующей пяточной шпоры. Кроме того, во время термодеструкции гибнут чувствительные нервные окончания, что приводит к исчезновению болей в области шпоры сразу же после манипуляции. Назначение обезболивающих средств поэтому не производится.
