Известны способы хирургического лечения плоско-вальгусной деформации стоп у детей с детским церебральным параличом (ДЦП) открытым вправлением таранной кости с внедрением костного трансплантата в подтаранное сочленение (Журавлев А. М. , Перхурова И.О., Осипов А.И., Горичев Б.М. Эквиноплосковальгусная деформация стопы у больных детским церебральным параличом и ее хирургическое лечение // Вестник травматологии и ортопедии им. Н.Н. Приорова. - 1994. - 2. - с.47-49). Недостатками данных способов является возможность применения у детей, начиная с семилетнего возраста, когда, как правило, уже имеются тяжелые деформации костей, смещение трансплантата в ранние сроки после операции, проникновения его в тело таранной или пяточной кости, что может привести к раннему рецидиву деформации. В то же время, целью данных операций является формирование необратимого костного блока на уровне подтаранного сустава, что неблагоприятно для дальнейшего роста стопы. Задача предлагаемого способа состоит в повышении эффективности лечения плоско-вальгусной деформации стоп у детей младшего возраста с ДЦП за счет стабильности фиксации в ближайшем послеоперационном периоде с последующим восстановлением подвижности в подтаранном суставе в течение 6-9 мес по мере рассасывания трансплантата. Использование моделирующей резекции пяточной кости в пределах хрящевой части позволяет применять данный способ у детей 3-6 лет, не вызывая дальнейших тяжелых нарушений роста костей стопы. Поставленная задача достигается тем, что в способе лечения плоско-вальгусной деформации стоп у детей младшего возраста с ДЦП путем открытого вправления таранной кости с костнопластической фиксацией таранно-пяточного сочленения предлагается производить моделирующую резекцию гипертрофированной хрящевой части пяточной кости, а фиксацию осуществлять чрескостно проведенным штифтом из замороженной кортикальной гомокости. Способ осуществляется следующим образом. The method is as follows.
Из разреза по наружному краю стопы от верхушки латеральной лодыжки до проекции кубовидной кости выделяются и Z-образно рассекаются сухожилия малоберцовых мышц. Производится капсулотомия подтаранного, пяточно-кубовидного и таранно-ладьевидного суставов. При этом становится виден гипертрофированный хрящ по тыльно-наружной поверхности переднего отдела пяточной кости, препятствующий вправлению таранной кости (чертеж 1, поз.1, 2). Скальпелем производится резекция этого участка в горизонтальной плоскости в пределах хрящевой части (чертеж 1, поз. 3). После этого таранная кость с помощью элеватора репонируется и сопоставляется с тыльной поверхностью пяточной кости. Достигнутое положение фиксируется шилом, проведенным через скелетированный участок подошвенной поверхности пяточной кости и таранную кость. Через канал в пяточной и таранной костях (чертеж 2, поз.1, 2), сформированный вторым шилом большего диаметра, импактором внедряется штифт из замороженной кортикальной гомокости (чертеж 2, поз.3). Длина штифта заранее рассчитывается по рентгенограммам и составляет 80% от суммарной высоты переднего отдела пяточной кости и шейки таранной кости. Сухожилия малоберцовых мышц сшиваются с удлинением.
