Наиболее близким (прототипом) является восстановление звучной речи у больных после резекции гортани путем рациональной психотерапии, дыхательных упражнений, дутья в губную гармошку для активизации мышц гортани, голосовых упражнений - произношение сонорного звука М, затем слов и фраз с глухими звуками Т, К, П (1). Поставленная задача решается способом восстановления звучной речи у больных после резекции гортани, заключающимся в проведении рациональной психотерапии, дыхательной гимнастики, магнитотерапии магнитным полем с помощью аппарата "Полюс-1" без теплового эффекта с индукторами цилиндрической формы диаметром 8 см, установленными на переднебоковую поверхность области шеи с двух сторон от средней линии, разными полюсами относительно друг друга с центровкой в точке вхождения возвратного нерва в течение 10 мин, на курс 10 процедур, причем первые пять процедур воздействуют переменным синусоидальным полем в непрерывном режиме, а в последующих процедурах - пульсирующим полем в импульсном режиме с индукцией магнитного поля, равной 17,5 мТл, и голосовых упражнений. Новым в предлагаемом изобретении является то, что перед голосовыми упражнениями проводят магнитотерапию магнитным полем без теплового эффекта с индукторами, установленными на переднебоковую поверхность области шеи с двух сторон от средней линии, разными полюсами относительно друг друга с центровкой в точке вхождения возвратного нерва в течение 10 мин, на курс 10 процедур, причем первые пять процедур воздействуем переменным синусоидальным полем в непрерывном режиме, а в последующих процедурах - пульсирующим полем в импульсном режиме с индукцией магнитного поля, равной 17,5 мТл, голосовые упражнения проводим с помощью произнесения звука "М" длительно, со слегка опущенной головой, одновременно воздействуя вибратором с плоской насадкой в точке вхождения возвратного нерва, попеременно с обеих сторон. В работе используются 2 вида токов: синусоидальный, дающий возможность создавать переменное магнитное поле, и пульсирующий - создающий однонаправленное магнитное поле. Больному в положении сидя накладывают два индуктора цилиндрической формы диаметром 8 см каждый. Устанавливают индукторы на переднебоковую поверхность шеи с двух сторон от средней линии, с центровкой в области вхождения возвратного нерва, относительно друг друга разными полюсами. Первые пять процедур воздействуют переменным синусоидальным полем в непрерывном режиме, а в последующих процедурах - пульсирующим полем в импульсном режиме с индукцией магнитного поля, равной 17,5 мТл. При появлении возможности говорить громким, но плохо модулированным голосом короткой фразы, больным предлагают произносить сочетания йотированных и гласных звуков. После чего переходят на речь восстановленным голосом в быту. Сроки восстановления звучной речи у больных после резекции гортани составили от 2 недель до 2 месяцев. В плане комбинированного лечения в предоперационном периоде больной получал химиолучевую терапию - 200 мг цисплатины и ФЭД=40 Гр. тормозным излучением бетатрона 22 МэВ. 2.03.1999 г. выполнено оперативное вмешательство в объеме трахеостомии, переднебоковой резекции гортани с эндопротезированием пористым никелидом титана. Швы сняты на 7 сутки. Заживление первичным натяжением. Трахеостома самостоятельно закрылась через 5 недель. Логовосстановительная терапия проводилась через 2 месяца после операции. У больного отмечалась полная потеря звучной речи - афония. В быту общался шепотной речью. Дыхание дезорганизованное, по данным спирографического исследования показатели внешнего дыхания снижены. Ларингоскопическое исследование: левая половина гортани неподвижна, правая - подвижна в полном объеме. При фонации голосовая складка по средней линии. Голосовая щель широкая.Example. Восстановление звучной речи проведено согласно предложенному способу. Рациональная психотерапия. Коррекция физиологического и фонационного дыхания с помощью комплекса дыхательной гимнастики. Активизация нервно-мышечного аппарата наружных и внутренних мышц оставшейся части гортани проведена с помощью аппарата переменного магнитного поля "Полюс-1". Индукторы цилиндрической формы диаметром 8 см устанавливались на переднебоковую поверхность шеи с двух сторон от средней линии относительно друг друга разными полюсами, с центровкой в области вхождения возвратного нерва. Первые пять процедур воздействовали переменным синусоидальным полем в непрерывном режиме, а в последующих процедурах - пульсирующим полем в импульсном режиме с индукцией магнитного поля, равной 17,5 мТл. Продолжительность процедуры - 10 мин, ежедневно. Курс лечения 10 процедур. На 20-й день приступили к голосовым упражнениям. Больной произносил звук "М" длительно со слегка опущенной головой. Одновременно воздействовали вибратором на точку вхождения возвратного нерва попеременно с двух сторон с частотой колебания 100 Гц. На третий день после выполнения этого упражнения появился грубый, плохо модулированный голос. Появилась возможность произносить слова с начальными фонемами Т, К, П. На 12-й день приступили к произнесению йотированных и гласных звуков. Далее больной перешел на общение в быту. В результате систематических занятий звучная речь достаточна для общения. При повторном спирографическом исследовании отмечалась нормализация всех параметров функции внешнего дыхания. При ларингоскопическом контрольном осмотре левая половина гортани неподвижна, правая голосовая складка подвижна в полном объеме. Голосовая щель широкая. При фонации правая голосовая складка заходит за среднюю линию и смыкается с рубцовой тканью оперированной половины гортани. Анатомическая травма приводит к дискоординации нервно-мышечного аппарата. Известно, что в спазмированной после операции мышце не могут проявляться условно-рефлекторные связи, поэтому необходимо их релаксировать. Магнитотерапия обладает противовоспалительным, противоотечным действием. Под воздействием магнитных полей улучшается микроциркуляция, стимулируются регенеративные и репаративные процессы в тканях, ускоряется прохождение нервных импульсов. Согласно предлагаемому способу проведено восстановление звучной речи после резекции гортани 12 больным. Преимущество предлагаемого способа по сравнению с известным способом-прототипом заключается в значительном сокращении сроков реабилитации и улучшении качества речевой функции.
