В настоящее время для лечения хронических вирусных гепатитов используются различные схемы лечения, предусматривающие использование различных доз препаратов интерферона (от 1 до 10 млн. ЕД), кратности (1-3 раза в неделю) и продолжительности лечения (от 3 месяцев до 2 лет). Основным недостатком существующих способов лечения является их невысокая эффективность (менее 50%). Наиболее близким к предлагаемому является способ лечения хронического вирусного гепатита, предусматривающий введение препаратов интерферона внутримышечно или подкожно в дозе 3 млн. ЕД 3 раза в неделю. Механизм противовирусного действия интерферона проявляется при его взаимодействии с клетками и заключается в подавлении репликации вирусов, стимуляции продукции эндогенного интерферона и иммуномодулирующем эффекте (Соринсон С.Н., 1998). The mechanism of the antiviral action of interferon manifests itself in its interaction with cells and consists in suppressing viral replication, stimulating the production of endogenous interferon and the immunomodulating effect (Sorinson S.N., 1998).

Технический результат при применении предлагаемого способа заключается в повышении эффективности противовирусной терапии при использовании умеренных доз интерферона с помощью непрямой лимфотропной терапии. Способ осуществляется следующим образом. Катетер вводится в круглую связку печени и фиксируется в ней при помощи лапароскопической техники под видеоконтролем. Препарат интерферона (реаферон или роферон А или интрон А) растворяется в соответствующем растворителе и вводится в катетер в дозе 3 млн. ЕД через день инфузоматом со скоростью 2-5 мл в час. Инфузии интерферона предшествует введение 5 мл 0,25%-ного раствора новокаина со скоростью 2-5 мл в час. В дни, когда интерферон не используется, катетер промывается 5-10 мл 0,25%-ного раствора новокаина, который также вводится инфузоматом с такой же скоростью. Лечение в таком режиме продолжается 14 дней, затем катетер удаляется, и пациент переходит на традиционную схему лечения по 3 млн. ЕД внутримышечно (подкожно) 3 раза в неделю. Катетеризация круглой связки печени и непрямое лимфотропное введение препарата интерферона повторяется через 3 и 6 месяцев. Общая продолжительность лечения интерфероном составляет 1 год. On days when interferon is not used, the catheter is washed with 5-10 ml of a 0.25% solution of novocaine, which is also injected with an infusomat at the same rate. Treatment in this mode lasts 14 days, then the catheter is removed, and the patient switches to the traditional treatment regimen at 3 million units intramuscularly (subcutaneously) 3 times a week. Catheterization of the round ligament of the liver and indirect lymphotropic administration of the drug interferon is repeated after 3 and 6 months. The total duration of treatment with interferon is 1 year.
Под нашим наблюдением находилось 12 больных с хроническим вирусным активным гепатитом (6 - с гепатитом С, 3 - гепатитом В, 3 - гепатитом В+С). Диагноз хронического вирусного гепатита был верифицирован лабораторными, патогистологическими, серологическими данными, а также ПЦР-анализом. Больным была проведена непрямая лимфотропная терапия одним из препаратов интерферона в дозе 3 млн. ЕД через день введением в круглую связку печени в течение 2-х недель. По сравнению с исходными параметрами через 2 недели лечения у больных отмечено снижение активности трансаминаз в плазме в 1,3-1,5 раза. Дальнейшее лечение в течение 3 месяцев больные получали в виде внутримышечных инъекций препарата интерферона в дозе 3 млн. ЕД 3 раза в неделю. Через 3 и 6 месяцев от начала терапии больным проводился повторный курс введения интерферона в круглую связку печени по той же схеме. Через 6 месяцев от начала лечения при биохимическом исследовании крови выявлены стойкая нормализация активности трансаминаз и признаки сероконверсии, а через 1 год - стойкая сероконверсия, нормальная активность трансаминаз, исчезновение интралобулярных инфильтратов и некрозов, то есть переход гепатита в неактивную фазу. Через 6 месяцев после окончания лечения только у 1 больного из 12 (пациент с хроническим гепатитом B+C) отмечены признаки обострения заболевания. Compared with the initial parameters, after 2 weeks of treatment, patients showed a decrease in plasma transaminases activity by 1.3-1.5 times. Further treatment for 3 months, patients received in the form of intramuscular injections of the drug interferon at a dose of 3 million units 3 times a week. After 3 and 6 months from the start of therapy, the patients were given a repeated course of introducing interferon into the round ligament of the liver according to the same scheme. After 6 months from the start of treatment, a biochemical blood test revealed persistent normalization of transaminase activity and signs of seroconversion, and after 1 year - persistent seroconversion, normal transaminase activity, the disappearance of intralobular infiltrates and necrosis, that is, the transition of hepatitis to the inactive phase. 6 months after the end of treatment, only 1 out of 12 patients (a patient with chronic hepatitis B + C) showed signs of exacerbation of the disease.

Больной Т., 44 года, поступил в клинику с диагнозом хронического гепатита С, активность II. Больному была назначена терапия рофероном А в дозе 3 млн. ЕД через день с введением препарата в круглую связку печени. Проведено 3 курса непрямой лимфотропной терапии интерфероном в круглую связку печени. Общая продолжительность лечения составила 1 год. Обследование, проведенное через 1 год лечения, не выявило признаков активного воспаления в печени, серологические маркеры репликации вируса гепатита С также были отрицательными. При контрольном обследовании через полгода и год после окончания лечения интерфероном не отмечено обострения заболевания, активность трансаминаз стойко нормальная, признаков репликации вируса гепатита С не выявляется, при морфологическом исследовании некрозов и воспалительных инфильтратов в паренхиме печени не обнаруживается.
