Abstract
Description
The invention relates to medicine, namely to surgery.
A method for treating eventration is known and applied in practice (patent 2045234 IPC A 61 B 17/03), in which U-shaped sutures are placed on each side of the wound through all layers of the abdominal wall except for the peritoneum in the direction inclined with respect to it. Every two symmetrically located sutures are removed through the skin near the wound and are fixed on the surgical bridge to fix the sutures, which allows you to adjust their tension. A method of treating eventration can also be used to prevent eventration.
In this method, the following disadvantages are inherent. Sutures and devices are used in all situations where there is a risk of eventing, however, this risk is realized only in 25-30% of cases, and in 70-75% of patients, suturing and devices is not justified enough and is accompanied by additional pain and increased consumption of painkillers funds. In addition, conducting U-shaped sutures through all layers of the abdominal wall, with the exception of the peritoneum, is technically difficult, traumatic and is accompanied by a large circulatory disturbance, which is unjustified for the purpose of prevention.
The purpose of the invention is to increase the effectiveness of surgical prevention of eventation and reduce its morbidity.
This goal is achieved by the fact that the operation is carried out in two stages. At the first stage, U-shaped sutures are made through the skin, subcutaneous tissue and the front walls of the vagina of the rectus abdominis muscles. When factors appear in the postoperative period that increase the risk of eventation, the second stage of the method is performed, which consists in fixing the U-shaped sutures on the surgical bridge.
The method is as follows. U-shaped sutures are applied parallel and symmetrically to the wound, through the skin, subcutaneous fat and the front walls of the vagina of the rectus abdominis muscles, deviating from the edges of the wound for at least 80 mm. Then, through each puncture point, the outer ends of the ligatures are additionally carried in the subcutaneous tissue to the edges of the wound with their removal through the skin to the outside, 30 mm from the edge of the wound. Further, the free ends of the ligatures without tension are connected with each other with a "bow" (stage 1). The surgical wound is sutured in the traditional way. In the early postoperative period, with the occurrence of factors leading to eventation (persistent postoperative obstruction, bronchopulmonary complications that occur with coughing, psychomotor agitation, and others), the ends of the ligatures of the U-shaped sutures are untied and are fixed with sufficient tension on the surgical bridge, which is transverse to to the wound (stage 2). In this case, it is possible to regulate the tension of the seams by moving the ligatures through the pins located on the surface of the bridge. U-shaped sutures and the surgical bridge are removed after eliminating the causes of the event, usually 10-14 days.
Example. Patient G., 64 years old, medical history N 2070, prevented eventuation during surgery for choledocholithiasis, obstructive jaundice. When suturing the surgical wound of the abdominal wall, 4 U-shaped sutures were applied, 2 on each side. Due to exacerbation of chronic obstructive bronchitis and increased cough on the 4th day after surgery, the ligatures of the U-shaped sutures are fixed on 2 surgical bridges with a tension sufficient to reliably fix the edges of the wound. Then, three times at intervals after 2 days, the tension of the seams was adjusted. The sutures from the skin were removed on day 11, and the U-shaped sutures and surgical bridges at 18 after stopping the cough and the appearance of positive dynamics during obstructive bronchitis. The surgical wound healed by first intention.
A method of preventing events is indicated during operations for peritonitis, at the height of gastrointestinal bleeding, intra-abdominal bleeding with damage to parenchymal organs, a high risk of bronchopulmonary complications in the postoperative period, as well as other conditions leading to disruption of reparative processes in the surgical wound.
The implementation of the method in two stages allows significantly less, by 55 - 75%, to use the device and reduce the use of painkillers. And conducting U-shaped sutures through the front walls of the rectus sheaths without trapping muscles and the back walls improves healing of the postoperative wound and, accordingly, reduces the treatment time.
