Abstract
Description
The invention relates to the field of medicine, gastroenterology, and specifically to methods for diagnosing helicobacteriosis.
Known methods for the diagnosis of Helicobacteriosis, allowing verification of Helicobacter pylori and consisting in sampling biopsy samples of the gastric mucosa with the following tests: studies of histological and cytological preparations of the gastric mucosa, urease test, bacteriological studies of the resulting pure Helicobacter pylori culture and detection of bacteria using phase-contrast microscopy. However, these methods have several disadvantages, namely: the morbidity of the study, caused by obtaining biopsy samples of the mucous membrane, and the duration of the analysis for 7 days, the shelf life of the sample for bacteriological studies is not more than two days.
Closest to the proposed method is a diagnostic method based on the preparation of a histological preparation from a biopsy specimen of the gastric mucosa and its further investigation. Its disadvantages are invasiveness, invasiveness and the duration of the method.
A new technical result - reducing trauma and reducing research time - achieves a new method for the diagnosis of Helicobacteriosis, which involves conducting esophagogastroduodenoscopy, collecting biological material from the antrum and fundus of the stomach, preparing drugs, identifying bacteria, and isolating a pure culture to determine antibiotic sensitivity, moreover, as biological material using parietal cavity mucus of the stomach in a volume of 0.4 - 0.5 ml.
The method is as follows: when conducting esophagogastroduodenoscopy, samples of parietal cavity mucus of the fundus and antrum of the stomach are aspirated with a volume of 0.4-0.5 ml using a hard catheter into a sterile syringe, followed by preparation of a cytological preparation from the mucus. After fixing the smear smear with 96% ethyl alcohol, it is stained with azure II-eosin for 20 minutes, while Helicobacter pylori is colored blue. Next, microscope under immersion.
To evaluate the activity of bacterial urease, a mucus sample is placed in urea medium and incubated.
If it is necessary to determine the sensitivity of Helicobacter pylori to antibiotics and to identify the coccal form of Helicobacter pylori, a pure bacterial culture can be isolated by plating the mucus on culture media.
If Helicobacter pylori is detected in patients with peptic ulcer and chronic superficial gastritis, if bacteria are found in the form of sticks of a typical C-shape, only a cytological examination can be performed.
The parietal cavity mucus of the stomach is selected as a biological material, because is the main habitat for Helicobacter pylori and bacteria can exist in the mucus in the form of sticks or cocci, in contrast to the epithelium of the gastric mucosa, where bacteria are most often presented in the form of sticks. The indicated volume of mucus is sufficient to prepare a smear and identify bacteria.
The study was conducted on 23 patients with duodenal ulcer and chronic gastritis, histological examination of biopsy samples of the gastric mucosa and analysis of smear smears from the antrum and fundus of the stomach were performed. A histological examination of biopsy samples revealed Helicobacter pylori in 15 patients (65%), a cytological examination of smear smears revealed Helicobacter pylori in 13 patients. The study time was one day, and with a cytological examination of a smear with detection of bacteria of a typical C-shaped form, one hour. The absence of Helicobacter pylori in mucus smears in the two examined patients may be associated with antibiotic therapy with trichopolum and amoxicycline. Mucus samples from three patients with diagnosed helicobacteriosis were used to obtain a pure bacterial culture to detect antibiotic susceptibility.
Based on clinical observations and studies, it can be concluded that the proposed method has the following advantages: reduced trauma when taking biological material and reduced complications while maintaining sufficient information in cases where peptic ulcer and chronic gastritis are caused by persistence of Helicobacter pylori. In addition, there is the possibility of longer storage of samples of biological material for bacteriological studies for more than two days. This diagnostic method can be used for screening examinations of the population (especially in children) due to the speed of obtaining results (rapid diagnosis within one hour) and the non-invasiveness of the study.
