Abstract
Description
Изобретение относится к медицине, а именно к хирургии сосудов головного мозга. The invention relates to medicine, namely to surgery of cerebral vessels.
Известно, что окклюзирующие процессы внутренней сонной артерии (ВСА) являются одной из основных причин развития ишемических нарушений мозгового кровообращения (ИНМК). В 18-40% случаев причиной ишемического инсульта является стеноз ВСА. [6, 10]. Статистические данные о том, что у 5-6% больных со стенозом ВСА, проявляющимся транзиторными ишемическими атаками в течение 6 месяцев, произойдет инсульт, диктуют необходимость активного лечения данной патологии. [11, 12, 15, 17, 18]. В настоящее время получены убедительные данные о преимуществе радикальных реконструктивных операций по сравнению с консервативным лечением. Так при длительном катамнезе (более 12 лет) после операции (каротидная эндартерэктомия) повторные ИНМК развиваются лишь у 8%, а без операции на фоне консервативного лечения - у 34% больных [3]. Важно отметить, что в подавляющем большинстве случаев (около 80%) при поражении ВСА наружная сонная артерия (НСА) остается интактной [8]. Это делает перспективным предлагаемый способ. [11, 12, 15, 17, 18]. Compelling evidence has now been obtained of the benefits of radical reconstructive surgery compared to conservative treatment. So, with prolonged follow-up (more than 12 years) after surgery (carotid endarterectomy), repeated IMSs develop only in 8%, and without surgery, with conservative treatment, in 34% of patients [3]. It is important to note that in the vast majority of cases (about 80%), when the ICA is affected, the external carotid artery (NSA) remains intact [8]. This makes the proposed method promising.

Резекция пораженного участка сосуда с последующим замещением его протезом из синтетического материала (лавсан, тефлон, дакрон, витлан) не лишена недостатков, так как организм реагирует на имплантированное инородное тело и в результате часто происходит тромбирование, обтурация просвета протеза. Ранние тромбозы сосудистого протеза (в течение первого месяца после реконструктивного вмешательства) наблюдаются у 19% оперированных, поздние - у 28% [5]. При этом проксимальный конец шунта анастомозируется с более мощной артерией, обычно с аортой, путем срединной стернотомии. В целях уменьшения травматичности операции в настоящее время применяется подключично-каротидное аутовенозное шунтирование (сегмент большой подкожной вены) [2]. Eastcott и соавт. в 1954 г. [9]. Недостатком данного метода является его травматичность, высокий риск ранних и поздних осложнений (кровотечение и эмболия из места анастомоза, тромбоз и рестеноз). Поставленная цель достигается тем, что формируют новую бифуркацию выше уровня окклюзии (стеноза) путем наложения анастамоза по типу "бок в бок" между внутренней и наружной сонными артериями. Преимущество предлагаемого способа по сравнению с прототипом состоит в том, что исключается работа на атероматозной бляшке и синокаротидной зоне, снижается риск эмболии и рестеноза, так как минимально травмирование интимарного слоя артерии, сохраняется проходимость ВСА первоначально. Способ осуществляют следующим образом. Производят линейный разрез кожи вдоль внутреннего края кивательной мышцы с заходом выше уровня угла нижней челюсти. Тупо и остро выделяют бифуркацию ОСА и ее ветви. Производят временное клипирование ОСА, ВСА и НСА. Ветви НСА проксимальнее клипса перевязывают. Выше уровня стеноза сближают внутреннюю и наружную сонные артерии и в прилежащих их стенках формируют овальные дефекты размером 8-10 х 3-4 мм. С помощью микрохирургической техники нитью 5/0 сшивают задний, а затем передний края анастомоза 14-16 узловыми швами. Последовательно снимают временные клипсы: с НСА, затем с ОСА и, наконец, с ВСА. Проводят контроль проходимости анастомоза, гемостаз. Послеоперационную рану ушивают. The method is as follows. Пример: больной П., 59 лет (история болезни N 17289-96 г.). Диагноз: церебральный атеросклероз. Стеноз устья левой внутренней сонной артерии. Нарушение мозгового кровообращения по ишемическому типу в левом каротидном бассейне. Заболел остро 9.10.96 г. При поступлении в нейрохирургическое отделение в неврологическом статусе выявлялись элементы моторной афазии, правосторонний умеренный гемипарез. При компьютерной томографии выявлен очаг ишемии в левой лобно-теменной области. На каротидной ангиограмме слева выявлен сегментарный стеноз устья внутренней сонной артерии до 80%. При ультразвуковой доплерографии средняя скорость кровотока по среднемозговой артерии слева снижена до 32 см/с, (норма 57±11 см/с [1]), справа - на нижней границе возрастной нормы. 1.11.96 г. под эндотрахеальным наркозом произведен разрез кожи вдоль внутреннего края левой кивательной мышцы. Тупым и острым путем выделены и мобилизованы общая, внутренняя и наружная сонные артерии. Визуально и пальпаторно в устье ВСА выявлена атероматозная бляшка протяженностью около 7 мм. Наружная сонная артерия интактна. На ОСА ниже бифуркации, на ВСА и НСА как можно дистальнее поставлены временные клипсы. Верхняя щитовидная и язычная артерии лигированы и пересечены. ВСА и НСА сближены и ротированы так, чтобы их прилежащие стенки были обращены кпереди. На расстоянии 25 мм выше бифуркации в стенках ВСА и НСА сформированы овальные дефекты размером 8 х 4 мм. С помощью микрохирургической техники под пятикратным увеличением на задний, а затем на передний края анастомоза наложено 14 узловых швов нитью 5/0. Снят клипс с НСА, затем с ОСА и, наконец, с ВСА. Гемостаз. Визуально и пальпаторно пульсация ВСА выше анастомоза хорошая. Послойные швы на рану. Длительность операции 1 ч 50 мин. После операции отмечен регресс неврологического дефицита: улучшение моторной речи, восстановление силы в правых конечностях до степени легкого пареза. При контрольной доплерографии скорость кровотока по среднемозговой артерии слева возросла до 58 см/с. При ангиографии 10.11.96 г. выявлен тромбоз стенозированного участка, ВСА заполняется через анастомоз. Example: Patient P., 59 years old (medical history N 17289-96). On the left carotid angiogram, segmental stenosis of the mouth of the internal carotid artery up to 80% was revealed. In ultrasound dopplerography, the average blood flow velocity along the midbrain artery on the left is reduced to 32 cm / s, (norm 57 ± 11 cm / s [1]), on the right - at the lower boundary of the age norm. November 1, 1996, under endotracheal anesthesia, a skin incision was made along the inner edge of the left sternocleidomastoid muscle. In a dull and acute way, the common, internal and external carotid arteries were isolated and mobilized. Visually and by palpation at the mouth of the ICA revealed an atheromatous plaque with a length of about 7 mm. The external carotid artery is intact. On the CCA below the bifurcation, on the ICA and NSA, temporary clips are placed as distally as possible. The superior thyroid and lingual arteries are ligated and crossed. ICA and NSA are brought together and rotated so that their adjacent walls are facing forward. At a distance of 25 mm above the bifurcation, oval defects of 8 x 4 mm in size are formed in the ICA and NSA walls. Using microsurgical technique, under a five-fold increase, 14 nodal sutures with a 5/0 thread were applied to the posterior and then to the front edges of the anastomosis. Clips were removed from the NSA, then from the OCA and, finally, from the ICA. Hemostasis. Visual and palpation of the ICA pulsation above the anastomosis is good. Layered seams on the wound. The duration of the operation is 1 h 50 min. After the operation, a regression of neurological deficit was noted: improved motor speech, restoration of strength in the right extremities to the degree of mild paresis. During the control dopplerography, the blood flow velocity along the midbrain artery on the left increased to 58 cm / s. When angiography 10.11.96, revealed stenotic thrombosis, ICA is filled through the anastomosis.
По данному способу оперировано 7 больных со стенозом ВСА. Срок наблюдения составил после операции от 2-х лет до 3-х месяцев. В указанной группе не отмечено повторных ИНМК, у всех пациентов наступил регресс дооперационного неврологического дефицита. При контрольной ангиографии через три месяца во всех случаях отмечен тромбоз ранее стенозированного участка с заполнением ВСА через анастомоз. Низкая травматичность способа позволила пациентам, часто с сопутствующей соматической патологией лучше и быстрее адаптироваться после операции, включиться в активный образ жизни.
