Abstract
Description
The invention relates to medicine, namely to vascular surgery, and is used for post-thrombophlebitis syndrome and varicose veins of the lower extremities complicated by trophic ulcers.
Ч.2. -С.32-34/. The closest in purpose and technical solution is the "Method for the treatment of trophic ulcers of the lower extremities", including transdermal laser therapy / The use of lasers in surgery and medicine, M., Samarkand, 1988. Part 2. -C.32-34 /.
The aim of the present invention is to increase the clinical effect in the treatment of trophic ulcers of the lower extremities. The goal is realized using intraosseous radiation with a low-intensity helium-neon laser into the calcaneus of the affected limb. An LG-126 generator with a wavelength of 0.632 μm and an output power of at least 16-17 mW / cm 2 is used . The duration of exposure of intraosseous laser irradiation of blood for all patients is 1 hour 1 time per day. Only 6 sessions. For this, a trocar needle is used, proposed by R.P. Askerkhanov 1960 / cm. drawing/. The puncture site is the outer surface of the calcaneus and is determined by the point of intersection of two perpendicular lines, conventionally drawn along the lower leg and foot, a vertical, lowered along the posterior edge of the diaphysis of the fibula / and the outer ankle / and the horizontal, running in the middle of the distance between the apex of the outer ankle and the plantar margin heels / R.P. Askerkhanov, 1960 /. Puncture is performed after soft tissue anesthesia with a 0.5% novocaine solution, the trocar needle is inserted into the soft tissues to the bone, while the thickness of the soft tissue layer is noted. With slight twists of the trocar needle, the cortical layer of the bone breaks through, then it is introduced to a depth of 0.5 cm / to the thread /. After the introduction of mandrin, the needle with the handle is additionally screwed into the bone by 3-8 mm, depending on the thickness of the bone. The safety shield must be in contact with the skin. The mandrin is again carefully inserted into the needle fixed in the bone in such a way to pierce a possible obstacle in the lumen of the needle. Then, the needle is removed by turning it slowly backward by 0.2-0.3 cm, the mandrin is removed and a quartz-polymer monofilament is inserted into the needle / cm. drawing / until it comes into contact with bone tissue, which is also pulled back 0.2-0.3 cm to create free space between the needle and the conductor and the spongy part of the bone. After that, a LG-126 quantum generator with a wavelength of 0.632 μm and an output power of at least 16-17 mW / cm 2 is turned on . The duration of exposure of intraosseous laser blood irradiation for all patients was the same for 1 hour. Similar procedures are done once a day. The course of treatment is 6 sessions.
The advantage of intraosseous laser irradiation is:
1. The laser beam has a direct and longer lasting effect on the spongy part of the calcaneus, which is rich in blood vessels, which serves as the most distal place in the lower extremity beginning for the outflow of venous blood.
2. A laser beam with intrahepatic radiation reduces coagulating blood processes, improves macro- and microcirculation of blood in tissues, which contributes not only to the rapid healing of ulcers, but also to the restriction of the use of anticoagulants, physiotherapeutic procedures and intravenous infusions to patients.
3. With intrahepatic laser radiation, the laser acts simultaneously on the superficial and deep venous network of the lower leg.
All these factors improve macro- and microhemodynamics in the area of trophic ulcers, have a positive effect on blood rheology, improve the protective power of the body and local tissues in the ulcer area, have anti-inflammatory, stimulating, desensitizing, analgesic and other healing properties.
