Изобретение относится к медицине, в частности к травматологии и ортопедии, и может быть использовано для биостимуляции организма в целях устранения болевого, воспалительного, отечного синдромов, улучшения местного и общего лимфокровотока, регенерации тканей после артроскопичеких операций на коленном суставе. Известен способ эндоскопических операций на коленном суставе с применением различного дополнительного оборудования (электрорезектор, коагулятор), значительно повышающий возможность хирургического вмешательства (J.М. Прототипом является методика внутрисуставного лазерного облучения, которая применялась у больных с преимущественным поражением капсульно-связочного аппарата коленного сустава воспалительно-дистрофического характера (Берглезов М.А., Вялько В.В., Угнивенко В.И. "Инвазивные методы лазеротерапии в травматологии и ортопедии. Методические рекомендации". - М., - 1995. - 21 С.) Она заключается в пункции сустава тонкой иглой, через которую суставная полость заполнялась кислородом, затем пунктировали сустав иглой с широким просветом, через которую проводился световод. Под контролем светящегося через кожу пятна световод подводят к пораженному участку сустава (верхний заворот, область крыловидных связок). Для облучения используют свет гелий-неонового лазера с мощностью излучения на рабочем конце световода 10-15 мВт/см2. Процедуру повторяли через 3-4 дня. Общее число сеансов 4-6 продолжительностью 15-20 минут. Нами разработан новый способ внутрисуставной лазеротерапии во время эндоскопических операций на коленном суставе. После предварительной анестезии через передненаружный доступ в полость сустава вводится артроскоп, а после выполнения хирургических манипуляций через передневнутренний стерильный лазерный эндоскоп. В полость сустава вводится 30-40 мл смеси 1%-ного раствора лидокаина и 0,5%-ного раствора адреналина (в соотношении 200000:1). Под контролем эндоскопа определяется необходимое место воздействия и к нему подводится световод. Лазеротерапия проводится способом медленного сканирования выбранных участков в течение 3-5 минут на каждый (общее время не более 15 минут) лучом гелий-неонового лазера с плотностью мощности 20 мВт/см2. Наличие во внутриполостной жидкости адреналина позволяет избежать кровоточивости, что обеспечивает прозрачность среды на весь период лазерного воздействия.We have developed a new method of intraarticular laser therapy during endoscopic operations on the knee joint. Нами проведен сравнительный анализ результатов наружной и внутрисуставной лазеротерапии у больных с дегенеративно-дистрофическими заболеваниями коленного сустава после хирургического лечения с применением эндоскопической техники. Внутрисуставная лазеротерапия во время операции выполнена у 22 больных. У 28 больных в течение 7 дней после операции проводилась наружная лазеротерапия по методикам, разработанным в институте. В послеоперационном периоде у всех больных выявлялись снижение явлений синовита, менее выраженный болевой синдром, быстрые регресс неврологических расстройств и разработка движений в суставе по сравнению с больными контрольной группы (52 пациента), получивших традиционное консервативное лечение. При сравнительном анализе клинических и инструментальных (биофотометрия, лазерная доплеровская флоуметрия) данных обследования больных, различий в результатах внутрисуставной и наружной лазеротерапии не отмечено. Таким образом, адресная внутрисуставная лазеротерапия является эффективным способом профилактики послеоперационных осложнений эндоскопических операций на коленном суставе. Однократное внутрисуставное воздействие лазерным светом заменило целый курс наружной лазеротерапии, что позволяет рекомендовать ее к применению при артроскопических операциях. In the postoperative period, all patients showed a decrease in synovitis, less severe pain, rapid regression of neurological disorders and the development of joint movements compared with patients in the control group (52 patients) who received traditional conservative treatment. In a comparative analysis of clinical and instrumental (biophotometry, laser Doppler flowmetry) examination data of patients, there were no differences in the results of intraarticular and external laser therapy. Thus, targeted intraarticular laser therapy is an effective way to prevent postoperative complications of endoscopic operations on the knee joint. A single intraarticular exposure to laser light replaced a whole course of external laser therapy, which allows us to recommend it for use in arthroscopic operations.
