Изобретение относится к медицине, а именно к сосудистой хирургии, и может быть использовано не только при посттромбофлебитическом синдроме (ПТС), но и при облитерирующих заболеваниях нижних конечностей. Известен также предложенный нами "Способ лечения облитерирующих заболеваний нижних конечностей и устройство для его осуществления" - прототип (заявка N 4663686/14 от 25.07.90 г., получено положительное решение из ВНИИГПЭ, опубликованное в N 39 - 92 г.), где для лечения окклюзионных поражений артерий предложена ступенчатая тренировка коллатералей с использованием:
1. Аппарата для тренировки коллатералей,
2. Манжетки для передавливания конечностей,
3. В качестве прототипа нами взято а.с. 1222275, заключающееся в лечении облитерирующих заболеваний артерий нижних конечностей ручной способ тренировки коллатериального кровообращения "Способ лечения облитерирующих заболеваний нижних конечностей". Целью настоящего изобретения является повышение клинического эффекта при лечении посттромбофлебитического синдрома нижних конечностей. Поставленная цель реализуется с помощью пережатия бедренных и подвздошных артерий. Аппарат для тренировки венозного кровообращения накладывается на больного, как указано, и прижимные пяты устанавливаются так, чтобы были расположены над проекцией верхней трети бедренных артерий у паховых складок. Затем на стороне поражения вен первые 5 - 6 дней передавливается пульсирующая бедренная артерия с таким расчетом, чтобы полностью закрыть просвет сосуда. Таких сеансов в сутки проводится 6 раз, а каждый сеанс состоит из 6 процедур (передавливания) продолжительностью 1,5 мин с перерывами 1 мин. В последующие 3 - 4 дня на том же уровне, только на здоровой стороне передавливается бедренная артерия, также в сутки проводится 6 сеансов по 6 раз, время сдавливания увеличивается с каждой процедурой от 2 мин до 4 мин, последующие 6 - 8 дней на той же здоровой стороне передавливается общая подвздошная артерия продолжительностью от 30 с до 30 мин с той же частотой как и выше. Только последние 2 дня перед началом сеанса (передавливанием подвздошной артерии) на пораженную конечность наматывают эластический бинт для передавливания подкожных вен (см. чертеж). Заключительные 4 - 5 дней на одном уровне попеременно передавливаются обе бедренные артерии в течение 2 мин. В день выполняется 6 сеансов и каждый сеанс состоит из 6 процедур (попеременно каждая бедренная артерия передавливается 6 раз). Все сеансы выполняются через разные промежутки времени. In the next 3–4 days at the same level, only on the healthy side the femoral artery is squeezed, also 6 sessions of 6 times are performed per day, the compression time increases with each procedure from 2 minutes to 4 minutes, the next 6–8 days at the same the common iliac artery is squeezed to the healthy side for a duration of 30 s to 30 min with the same frequency as above. Only the last 2 days before the start of the session (squeezing the iliac artery), an elastic bandage is wound on the affected limb to squeeze the saphenous veins (see drawing). The final 4 - 5 days at the same level, both femoral arteries are alternately squeezed for 2 minutes. 6 sessions are performed per day and each session consists of 6 procedures (each femoral artery is alternately squeezed 6 times). All sessions are performed at different intervals. The course of treatment is 22 to 27 days.

При посттромбофлебитическом синдроме нарушается гемодинамика не только в паравазальном пространстве, но и в вазо-вазорум (сосудах сосудов), что и является основной причиной превращения нормальной эластичной венозной стенки и их клапанов в фиброзную ткань с последующим нарушением функциональной и биологической деятельности сосудов. При ПТС нарушается функция не только венозных сосудов, но и артериальных и лимфатических в разной степени. С учетом патогенеза самой ПТС процедуры, проводимые при тренировке венозного кровообращения, в частности передавливания бедренных и подвздошных артерий, повышают ишемию всех тканей конечностей, что способствует продуцированию вазоактивных продуктов - лактат и аденозин, а также нуклеотидов, которые являются сильными расширителями сосудов и особенно коллатералей. Аналогичное происходит в пораженных венозных сосудах. Вместе с тем улучшается гемодинамика и микроциркуляция в самих стенках вен, что способствует сохранению функциональной и биологической деятельности венозных сосудов. Во-вторых, при тренировке венозного кровообращения происходит увеличение скорости и объема артериального и венозного кровотока в пораженной конечности, что несомненно способствует снижению вязкости, следовательно, улучшению микроциркуляции в тканях конечности; вместе с тем последняя способствует расширению сосудов, что имеет важное значение в регулировании гемодинамики в пораженной конечности. При тренировке кровообращения при посттромбофлебитическом синдроме имеет значение возникающий эффект Анрепа. Под действием усиливающейся (т.к. передавливаются сосуды) выталкивающей силы сердца повышается скорость и внутрисосудистое давление в магистральных и особенно коллатеральных сосудах, что также способствует расширению сосудов и улучшению гемодинамики в конечностях. Таким образом, в основе эффекта от тренировки кровообращения при посттромбофлебитическом синдроме лежат законы Пуайзеля, эффект Анрепа, накопление вазоактивных продуктов. Secondly, during training of venous circulation, an increase in the speed and volume of arterial and venous blood flow in the affected limb occurs, which undoubtedly contributes to a decrease in viscosity, and therefore, an improvement in microcirculation in the tissues of the limb; however, the latter contributes to the expansion of blood vessels, which is important in the regulation of hemodynamics in the affected limb. When training blood circulation in post-thrombophlebitic syndrome, the arising effect of Anrep is important. Under the influence of increasing (since blood vessels are squeezed) the buoyant force of the heart increases the speed and intravascular pressure in the main and especially collateral vessels, which also contributes to the expansion of blood vessels and improve hemodynamics in the limbs. Thus, the effect of blood circulation training in post-thrombophlebitis syndrome is based on the Poisel laws, the Anrep effect, and the accumulation of vasoactive products.

Пример. Больной Н.А., 56 лет (рис. 1) история болезни N 403/423 из Грозного поступил в клинику 16 апреля 1990 года с жалобами на боли в н/конечностях, особенно в правой. Болен с 1987 года, лечили неоднократно амбулаторно и в стационарных условиях. В 1989 году в Киеве оперирован по поводу посттромбофлебитического синдрома правой н/конечности - перевязка и пересечение коммуникантных вен с тромбоэктомией из бедренной вены (выписка). После операции через 1 месяц состояние больного ухудшилось. При поступлении - правая нижняя конечность больше в объеме (отекает), чем левая на 12 см (голень), на 10 см (бедро). На н/правой голени гиперпигментация, мацерация кожи на внутренней поверхности, трофическая язва размерами 2,5х2,5 см. Послеоперационные рубцы на бедре размерами 12х2,5, на в/з голени 12х1,5 см. Выраженная картина псевдоварикоза подкожных и кожных вен. Флеботонометрия подкожных вен позволила отметить - 300 мм вод. ст. На флебограмме выраженный псевдоварикоз кожных и подкожных вен, глубокие вены голени местами мешкообразно расширены, а местами не проходимы, подколенная вена резко сужена, бедренная вена непроходима. Больному проведен курс лечения тренировкой коллатерального кровообращения в течение 27 дней. Состояние улучшилось, отек в конечности спал, трофическая язва голени зарубцевалась. Выписан домой 14/V-92 г. в удовлетворительном состоянии. Example. On the phlebogram, pronounced pseudovaricosis of the skin and saphenous veins, deep veins of the lower leg are bag-like dilated in places and not passable in places, the popliteal vein is sharply narrowed, the femoral vein is impassable. The patient underwent a course of treatment by training collateral circulation for 27 days. The condition improved, the edema in the limbs was asleep, the trophic ulcer of the lower leg healed. Discharged home on 14 / V-92, in satisfactory condition.

Признаки лечения, отличительные от прототипа
1. Лечение посттромбофлебитического синдрома (ПТС) с помощью механического сдавливания бедренных и подвздошных артерий с определенным ритмом и протяженностью предлагается впервые. Время сдавливания артерии от 30 с до 4 мин, в день по 6 сеансов, состоит из 6 процедур. Вместе с тем, при лечении ПТС тренировкой венозного кровообращения пораженную конечность бинтуют эластическим бинтом (как указано выше) для сдавливания кожных и подкожных вен с целью повышения давления крови в глубоких венах пораженной конечности. Последнее способствует быстрому раскрытию коллатералей и магистральных вен и улучшению оттока венозной крови.Signs of treatment distinctive from the prototype
