Рассмотрим использование навигационного устройства на примере пациента с компрессионно-оскольчатым переломом тела ThVIII позвонка.Let us consider the use of a navigation device on the example of a patient with a compression-comminuted fracture of the ThVIII vertebral body.
Для каждого транспедикулярного винта формируется изображение и определяется траектория проведения, создается карта разметки. В карте отмечается: А - расстояние между точками введения винтов (Фиг. 5); по расстояниям KR и KL подбирают длину винта (Фиг. 6); В - расстояние от горизонтальной линии в аксиальной проекции, проходящей через дужку вводимого позвонка (при сечении дужки 15 мм) до параллельной линии, проходящей через точки введения винтов в аксиальной проекции (Фиг. 5); CR и CL - расстояние от точки, находящейся на средней линии, проходящей через центр тела позвонка и центр дужки (при сечении дужки 15 мм) на высоте 70 мм до точек, находящихся на перпендикулярной линии в месте пересечения с траекторией введения винтов с двух сторон в аксиальной проекции (Фиг. 7); DR и DL - расстояние от точки, находящейся на средней линии, проходящей через центр тела позвонка и центр дужки (при сечении дужки 15 мм) на высоте 120 мм от центра дужки до точек, находящихся на перпендикулярной линии в месте пересечения с траекторией введения винтов с двух сторон в аксиальной проекции (Фиг. 7); Е - расстояние от точки, находящейся на высоте 150 мм от перпендикулярной линии, проведенной через дужки вводимого и соседнего позвонков (при сечении дужки 15 мм) до точки, находящейся на линии введения винта на той же высоте в сагиттальной проекции (отклонение винта в сагиттальной проекции) (Фиг. 8); F - расстояние от точки на дужке вводимого позвонка, находящейся на линии, проведенной через дужки вводимого и соседнего позвонков (при сечении дужки 15 мм) до точки введения винта на той же линии в сагиттальной проекции (дополнительно для поясничного отдела, так как дужки в поясничном отделе имеют выступающий острый край, где более четко можно отметить данную точку) (F) (Фиг. 9). На основе проведенной мультипланарной реконструкции создается сводная таблица с расстояниями на примере ThVII позвонка (Фиг. 10). По сформированной карте разметки данные с таблицы переносятся на навигационное устройство. Винтами (8) ослабляются ползуны (7) на проставках (6). Выставляются расстояния CR и CL на нижней, DR и DL на верхней проставках и винты (8) затягиваются, таким образом формируется угол конвергенции. С помощью гаек (14) на регулировочном винте (13) устанавливается расстояние (Е) (от центра соединения регулировочного винта до центра ближайшей регулировочной гайки), и тем самым получаем угол наклона в сагиттальной плоскости. Согласно выставленным расстояниям (CR, CL, DR, DL, E) вставляются метчики (11) в отверстия (9) в ползунах (7), далее устройство ставится на горизонтальную плоскость для выставления расстояний А и В. Причем, при точной установке угла конвергенции, выставляя расстояние А, должны получить расстояние В и наоборот. С помощью винтов (10) по выставленным расстояниям (А, В, CR, CL, DR, DL, E) метчики (11) фиксируются на ползунах. В собранном виде навигационное устройство накладывают на дужки двух соседних позвонков (ThVI, ThVII), располагая его строго по центру этих позвонков (как и проводилась разметка), чтобы метчики свободно расположились между дужками (в данном случае ThVI-ThVII) (Фиг. 1). В поясничном отделе в местах прижатия основания устройства к дужкам позвонков появятся условные точки соприкосновения на дужках. При перемещении устройства вдоль позвоночника от дужки ThVI к дужке ThVII метчики упрутся в костное препятствие дужки ThVII позвонка, т.е. метчики укажут на рассчитанные точки введения винтов на дужках ThVII позвонка. Для устойчивости устройство прижимается к позвоночнику и фиксируется стержнями к основанию остистых отростков. Стержни заводятся по резьбе и ограничены на введение до средней линии позвонка. Ослабляются крепления винтов (10) на ползунах, и метчики по заданной навигационной траектории с двух сторон вводятся в тело ThVII на 10-15 мм. Для уверенности положения метчиков однократно проводится рентгеноконтроль в двух проекциях. Метчики через 45 мм резко утолщаются для предотвращения случайного ятрогенного прохождения глубже. При сохранении навигационных траекторий продолжается формирование каналов для винтов, ориентируясь на длину каналов по расстояниям KR и KL (Фиг. 5); при необходимости щупом исследуются сформированные каналы. Затем в устройство вставляются винты на центральных штангах необходимой длины (с учетом расстояний KR и KL) и вкручиваются в подготовленные каналы.For each pedicle screw, an image is formed and the insertion trajectory is determined, and a marking map is created. Based on the performed multiplanar reconstruction, a summary table with distances is created using the example of the ThVII vertebra (Fig. 10). According to the generated markup map, data from the table is transferred to the navigation device. The screws (8) loosen the sliders (7) on the spacers (6). The distances C R and C L on the bottom spacer, D R and D L on the top spacer are set and the screws (8) are tightened, thus forming the angle of convergence. Using the nuts (14) on the adjusting screw (13), the distance (E) is set (from the center of the connection of the adjusting screw to the center of the nearest adjusting nut), and thus we obtain the angle of inclination in the sagittal plane. According to the set distances (C R , C L , D R , D L , E), taps (11) are inserted into the holes (9) in the sliders (7), then the device is placed on a horizontal plane to set distances A and B. Moreover, when precise setting of the angle of convergence, setting the distance A, should get the distance B and vice versa. Using screws (10) at the set distances (A, B, C R , C L , D R , D L , E) the taps (11) are fixed on the sliders. When assembled, the navigation device is applied to the arches of two adjacent vertebrae (ThVI, ThVII), placing it strictly in the center of these vertebrae (as the marking was carried out), so that the markers are freely located between the arches (in this case, ThVI-ThVII) (Fig. 1) . In the lumbar region, in places where the device base is pressed against the vertebral arches, conditional points of contact will appear on the arches. When the device is moved along the spine from the ThVI arch to the ThVII arch, the taps will hit the bone obstruction of the ThVII arch, i.e. the markers will indicate the calculated screw insertion points on the pedicles of the ThVII vertebra. For stability, the device is pressed against the spine and fixed with rods to the base of the spinous processes. The rods are threaded and limited to the midline of the vertebrae. The screws (10) on the sliders are loosened, and the taps are inserted into the ThVII body by 10-15 mm along the given navigation trajectory from both sides. To ensure the position of the taps, X-ray control is carried out once in two projections. Taps thicken sharply after 45 mm to prevent accidental iatrogenic penetration deeper. While maintaining the navigation trajectories, the formation of channels for the propellers continues, focusing on the length of the channels in terms of distances K R and K L (Fig. 5); if necessary, the formed channels are examined with a probe. Then screws are inserted into the device on the central rods of the required length (taking into account the distances K R and K L ) and screwed into the prepared channels.

После СКТ исследования выполнена мультипланарная реконструкция позвоночника с занесением данных в карту разметки. Анестезия -эндотрахеальный наркоз. Послойный задний доступ к поврежденном отделу позвоночника. Для винтов в LI и LIII позвонки данные с карты разметки последовательно переносились на навигационное устройство. Навигационное устройство с закрепленными данными для LI позвонка опускалось в рану на дужки LI, LII позвонков, смещалось в сторону LI позвонка, пока метчики не упирались в костное препятствие дужки LI позвонка; стержни на П-образном основании вкручивались с двух сторон в основание остистого отростка LI позвонка; ослаблялись винты на проставках, которые фиксируют метчики, и одновременно с двух сторон метчики погрузились на глубину до 2 см; выполнен ЭОП контроль в 2-х проекциях и убедились в том, что траектории проведения метчиков совпадают с рассчитанными; дальнейшее формирование каналов метчиками на 45 мм (до расширения метчиков); метчики удалены и по той же траектории через навигационное устройство по ходу сформированных каналов нарезана резьба; выкручены стержни на П-образном основании и в навигационное устройство вставлены винты 50 мм на центральных штангах; с прежними параметрами навигационное устройство винтами возвращается в рану, где кончики винтов ориентируются в сформированные каналы, и стержни вкручиваются в прежние отверстия; по заданной траектории вкручиваются винты; ослабляются винты на проставках, и навигационное устройство удаляется. Затем данные с карты разметки переносятся на навигационную систему для винта LIII; устройство помещается в междужковый промежуток LIII-LIV, и манипуляции повторяются как и для позвонка LI. После введения винтов выполнен ЭОП контроль (3-й по счету). Наложена репозиционная система, произведена тракция позвоночника по оси и получены значительное расправление тела LII позвонка и исправление кифотической деформации позвоночника. Через заранее собранные муфты на наружных продольных штангах репозиционной системы под ЭОП контролем введены редукционные винты, и дальнейшими манипуляциями с гайками на центральных штангах полностью устранена деформация передней стенки позвоночного канала. Транспедикулярные винты с двух сторон соединены между собой отмоделированными фиксирующими штангами. Демонтирована репозиционная система, и рана послойно ушита с дренажами. Асептическая повязка. К концу первой недели отмечался регресс неврологических нарушений. Рана зажила первичным натяжением, и через 11 дней пациент переведен в отделение медицинской реабилитации.After the CT scan, a multiplanar reconstruction of the spine was performed with the data entered into the markup map. Then the data from the markup map is transferred to the navigation system for the LIII propeller; the device is placed in the interarticular space LIII-LIV, and the manipulations are repeated as for the LI vertebra. After the introduction of the screws, an image intensifier tube control was performed (3rd in a row). A repositioning system was applied, axial traction of the spine was performed, and a significant straightening of the body of the LII vertebra and correction of kyphotic deformity of the spine were obtained. Through pre-assembled couplings on the outer longitudinal rods of the repositioning system, under image intensifier tube control, reduction screws were introduced, and further manipulations with the nuts on the central rods completely eliminated the deformation of the anterior wall of the spinal canal. Transpedicular screws are interconnected on both sides by modeled fixing rods. The repositioning system was dismantled, and the wound was sutured in layers with drains. Aseptic bandage. By the end of the first week there was a regression of neurological disorders. The wound healed by first intention, and after 11 days the patient was transferred to the medical rehabilitation department.
