Изобретение относится к медицине, а именно к офтальмологии, и касается хирургического способа отслойки сетчатки с разрывами ее в заднем полюсе глаза в области макулы и зрительного нерва. Недостатками этого способа являются травматичность, техническая сложность и опасность доступа к заднему полюсу глазного яблока в связи с необходимостью проведения швов или выкрашивания склерального кармана для погружения пломбы в зоне крупных сосудов и зрительного нерва в узком и глубоком, практически в виде колодца, операционном поле. Эти манипуляции особенно опасны при миопической болезни, характеризующейся передне-задней растянутостью глазного яблока, истончением склеры и формированием стафилом различных размеров и выраженности. В ходе операции высок риск прорезывания швов, дислокации пломбы, перфорации склеры, кровоизлияний в оболочки глаза, нарушений кровообращения в сосудах зрительного нерва, сетчатки и сосудистой оболочки. Пломбажное вдавливание формирует постоянную или длительную деформацию заднего полюса глаза, приводящую к нарушению функции. К снижению ее приводит криопексия, диатермическая или лазерная коагуляция в области макулы. Технические трудности и осложняющие моменты приводят к удлинению операции и возрастанию ее травматичности. Блокирование разрыва осуществляют специально изготовленными из силикона или латекса экстрасклеральными надувными баллонами при раздевании формирующими овальное эластичное тело с седловидным углублением в концевой части объемом до 10 мл. Фиксируют баллон путем прижатия несущего катетера в меридиане разрыва П-образным или четырехстежковым рассасывающимся/кетгут, коллаген, викрил (швом, проведенным через склеру в 10-18 мм от лимба, обычно под местом прикрепления /или у переднего края) нижней косой мышцы. Баллон раздувают физиологическим раствором с антибиотиком до объема 2-7 мл. В ходе операции добиваются формирования офтальмоскопически видимого вдавливания, начинающегося от края диска зрительного нерва. После ушивания конъюнктивы катетер этим же швом дополнительно фиксируют. Состояние зрительных функций контролируют в ходе операции. В послеоперационном периоде типичен экзофтальм и в течение 4-5 суток заметен хемоз в нижнем своде. Срок пребывания баллона 17-24 суток (средний - 21 сутки). В обход старых лоскутов склеры сделан тоннель (около 10 мм шириной) к заднему полюсу глаза и в 3-4 мм от проекции макулы нанесены криокоагуляты средней степени. В меридиане 8-9 ч. к заднему полюсу глаза проведен баллон из силикона размером 16•3,5 мм (в раздутом состоянии в концевой части формирующий седловидное углубление). Баллон фиксирован к склере путем подшивания катетера четырехстежковым кетгутовым швом, наложенным в 10-16 мм от лимба. С помощью шприца баллон заполнен стерильным физиологическим раствором с антибиотиком в объеме 3,5 мл. Получено вдавливание полюса глаза с блокированием макулярного разрыва концевым отделом баллона. На 17 и 19 сутки после операции, учитывая прилегание сетчатки и закрытие ("замуровывание") разрыва, поэтапно удалены из баллона 1,5 и 1 мл жидкости (физиологического раствора), а на 21 сутки баллон снят. Сетчатка прилежит. Острота зрения правого глаза - 0,02 с коррекцией сферой -20,0 дптр = 0,2 - 0,3. По сравнению с известным способом лечения отслойки сетчатки предлагаемый способ лечения позволяет достигнуть у пациентов более высокого уровня остроты зрения при снижении травматичности и упрощении способа операции. Способ позволяет не травмировать зрительный нерв и сосуды вокруг него, отсутствует угроза перфорации стенки глаза, особенно вероятной в зоне задних стафилом и источений склеры, как следствие этого возникновение тяжелых осложнений. Благодаря предложенному способу удалось сделать меньший разрез конъюнктивы, не нужно делать разрезов век или их спайки, накладывать швы-держалки на все мышцы (и тем более их пересекать), достаточно фиксировать только нижнюю и наружную прямые мышцы, не нужен форсированный разворот глаза, обеспечивается надежный выход на зону поражения. Швы фиксируют катетер, несущий баллон. Проведение поверхностных склеральных швов идет в легко доступной зоне склеры. Sutures fix the catheter carrying the balloon. Carrying out superficial scleral sutures is in an easily accessible area of the sclera.
Не используется ретинопексия с воздействием на склеру в проекции макулы.
