Abstract
Description
The invention relates to medicine, namely to dermatology, and can be, in particular, used to treat trophic ulcers and burns in patients with leprosy.
(50%). A medicine is known from the practice of medicine for the treatment of trophic ulcers in patients with leprosy, containing methyluracil, 10 20% methyluracil ointment (Goloshchapov N.M. On the use of methyluracil in the complex treatment of perforated ulcers of the soles in patients with leprosy. / Questions of leprology and dermatology. Rostov-on Don. 1968, p. 116 117). The disadvantages of this tool are the long duration of treatment (up to 75 days), a low percentage of ulcer healing (50%).
Solcoseryl (Belda W. Treatment of plantar ulcer in leprosy with extract of deproteinized (solcoseryl)// International Journal of leprosy. 1967, v. 35, N. 1, р. 94(A). It is known to use for the treatment of trophic ulcers a local agent containing Solcoseryl (Belda W. Treatment of plantar ulcer in leprosy with extract of deproteinized (solcoseryl) // International Journal of leprosy. 1967, v. 35, N. 1, p. 94 (A ). The disadvantages of this tool include a small number of observations, long periods of treatment.
длительные сроки заживления язв (до 50 сут.);
The closest remedy to the proposed one is the remedy (in the form of an ointment), the constituent ingredients of which are rifampicin and vaseline-lanolin-based dimexide (Bragin V. S. Yushchenko A. A. Auth. St. USSR N 1196001). However, the known tool has the following disadvantages:
long periods of ulcer healing (up to 50 days);
the percentage of positive results did not exceed 70%
the percentage of relapse of ulcers reaches 40%
the possibility of developing local allergic reactions to the applied ointment.
The aim of the invention is to accelerate the healing of ulcerative skin defects and burns in patients with leprosy.
This goal is achieved in that the product as a medicinal substance contains zinc oxide, liquid paraffin and additionally the antibiotic doxycycline hydrochloride, with the following weight percentage of components (wt.):
Doxycycline hydrochloride 1.0 5.0
Zinc Oxide 5.0 15.0
Vaseline oil 10.0 20.0
Vaseline-lanolin base The rest.
The value of the ingredients indicated in the claims was chosen as the most optimal for achieving the purpose of the invention to accelerate the healing of trophic ulcers and burns in patients with leprosy. The content of doxycycline hydrochloride in an amount of 1.0 to 5.0 wt. provides the best epithelization of trophic ulcers and burns (in days). The content of zinc oxide in an amount of 5.0 to 15.0 wt. provides the best drying, astringent and disinfecting effect of the surface of trophic ulcers and burns. Vaseline oil in an amount of 10.0 to 20.0 wt. provides high-quality dissolution of doxycycline and obtaining, with further mixing, the components of a homogeneous ointment mass. Vaseline-lanolin base provides the necessary soft consistency of the proposed ointment form.
The method of obtaining funds.
The ointment is prepared extemporaneously from medicines approved for medical use (doxycycline hydrochloride, zinc oxide, paraffin oil, petroleum jelly, lanolin).
Description of the ointment: ointment of a yellowish color for external use.

All components known for their use in practical medicine are non-toxic and do not require special permission to use them. However, the proposed composition is new and its application makes it possible to obtain a single new positive effect, which consists in increasing the effectiveness of treatment for patients with leprosy by reducing the healing time of ulcers and burns, increasing the percentage of healing skin defects, reducing the number of relapses of ulcers.
The proposed tool can be made in any anti-lethargic institution of the country according to the available prescription, since each of them has its own pharmacy, which guarantees the quality of preparation.
The development of trophic ulcers in patients with leprosy is based on a complex of interaction of a number of factors, the leading ones being a specific lesion of the peripheral nervous system, vascular disorders, while secondary infection of trophic ulcers is only an aggravating and, moreover, not a necessary factor. The authors examined trophic ulcers in 112 patients with leprosy for pathogenic microflora, the results of which showed the presence of the latter in 32.1% and the detected microflora was sensitive to doxyclin only in 20%
The proposed ointment was successfully tested in the treatment of injuries (of various genesis) of the outer cover of nine-belted armadillos experimentally infected with human leprosy.
We present the data of a clinical trial of the proposed drug in patients with leprosy with trophic ulcers (Table 1).
72,0. Thus, the optimal wt. the ratio of the components of the ointment, ensuring the achievement of the goal, is: doxycycline hydrochloride 3.0; zinc oxide 10.0; petrolatum oil 15.0; Vaseline-lanolin base 72.0.
1. The results of comparative clinical testing of the proposed drug in patients with leprosy with thermal burns are presented in the following table.2. The control group received topical treatment with official Fastin 1 ointment.
Thus, the ointment containing the antibiotic doxycycline proved to be quite effective in the treatment of burns in patients with leprosy, helping to accelerate the processes of epithelization, reducing the time of treatment of patients.
3). The proposed tool was successfully tested in the clinic of the Research Institute for the Study of Leprosy, the Abinsky Leprosy, the Karakalpak Leprosy on 140 patients for 8 years (from 1986 to 1995). Below is a summary table of testing results (table. 3).
Thus, the above data confirm the effectiveness of the proposed remedies before known means, a significant reduction in the healing time of ulcers and burns, an increase in the percentage of healing.

Example 4. Patient I-va (source. Bol. N 1683). The diagnosis is lepromatous type of leprosy. Suffers from trophic ulcers for 12 years. Repeatedly treated with temporary improvement. On the plantar surface of the left foot there are 2 trophic ulcers with a diameter of 1 and 1.5 cm. Local daily use of an ointment containing 0.5% doxycycline, 3% zinc oxide, 5% petroleum jelly, 91.5% petrolatum-base, only led to reducing the size of ulcerative defects without a healing effect.
N 5. Example No. 5.
Patient K-in (source. Bol. N 2437). The diagnosis is lepromatous type of leprosy. Suffers from trophic ulcer of the left foot for 3 years. On the lateral surface of the left foot, a trophic ulcer is three by five cm in size. Daily local applications of an ointment containing 6% doxycycline, 20% zinc oxide, 25% petrolatum oil, 49% petroleum jelly-base, led to the complete healing of the ulcer within 60 days. Two months later, relapse of the ulcer was noted. Against the background of application, the patient periodically complained of skin itching.
Thus, the authors for the first time proposed a previously unproven remedy for the treatment of trophic ulcers and burns in patients with leprosy. Using the proposed tool, a positive effect is achieved, consisting in increasing the effectiveness of treatment for patients with leprosy by reducing the treatment time, increasing the number of healing skin defects, reducing the number of relapses of ulcers.
The proposed tool reduces the treatment time for patients with trophic ulcers and burns by an average of 6 days. in comparison with the known means (prototype).
The percentage of healed ulcers compared with the prototype increased from 70 to 90% i.e. 20%
The reduction in the number of relapses of ulcers decreased compared with the prototype from 40% to 14% i.e. almost three times.
The use of the proposed tool allows you to almost completely avoid the occurrence of allergic reactions.
The Institute sees the resulting positive effect of the proposed product not in the sum of the positive effects that are separately included in its composition of ingredients, but considers it to be due to the totality of the latter, taken in the ratio set forth in the claims.
