Description
Дисплазия тазобедренных суставов это полигенно наследуемое заболевание, характеризующееся потерей соответствия между суставными поверхностями и ведущее к возникновению вывиха или артроза в суставе. Среди служебных пород собак дисплазия широко распространена, и при отсутствии мер борьбы с ней частота ее возникновения может достигать 60-70% (Белов А.Д. и соавт. 1994). Наследственно передаваемый фактор недоразвития сустава является пусковым моментом возникновения врожденного вывиха бедра или дисплазии. Дисплазия тазобедренных суставов является пороком формирования всех частей сустава (вертлужной впадины, головки бедренной кости, прилежащих мышц, сумочно-связочного аппарата), а также повреждением или недоразвитием его отдельных элементов. При отсутствии лечения дисплазия тазобедренных суставов приводит к серьезным дегенеративным изменениям суставных поверхностей, сумочно-связочного аппарата и, как следствие этого, стойким нарушениям функции пораженной конечности, проявляющейся, как правило, в неуверенной походке животного, слабости конечностей или полном отказе передвигаться. На почве дисплазии могут возникать деформирующие артрозы (Корж А.А. и соавт. 1987), остеохондропатии, варусное искривление шейки бедренной кости и пр. (Куценок Я.Б. и соавт. 1992). Hip dysplasia is a polygenically inherited disease characterized by a loss of correspondence between articular surfaces and leading to dislocation or arthrosis in the joint. Among service breeds of dogs, dysplasia is widespread, and in the absence of measures to combat it, the frequency of its occurrence can reach 60-70% (Belov A.D. et al. 1994). The hereditary transmitted factor of joint underdevelopment is the starting point for the occurrence of congenital hip dislocation or dysplasia. Hip dysplasia is a defect in the formation of all parts of the joint (acetabulum, femoral head, adjacent muscles, purse-ligamentous apparatus), as well as damage to or underdevelopment of its individual elements. If untreated, dysplasia of the hip joints leads to serious degenerative changes in the articular surfaces, the purse-ligamentous apparatus and, as a result, persistent dysfunctions of the affected limb, which usually manifests itself in an uncertain gait of the animal, weakness of the limbs or complete refusal to move. Due to dysplasia, deforming arthrosis can occur (Korzh A.A. et al. 1987), osteochondropathy, varus curvature of the femoral neck, etc. (Kutsenok Y. B. et al. 1992).

укорачивающей ступенчатой остеотомии бедренной кости (Митин В.Н. 1984),
межвертельных клиновидных остеотомий, позволяющих менять угол входа головки бедра в вертлужную впадину, с последующим проведением остеосинтеза Prieur W. D. Scartazzini R. 1980; Prieur W.D. 1990),
супраацетабулярной остеотомии таза по К.Chiare (1955) с созданием крыши вертлужной впадины и увеличением площади нагружаемых поверхностей головки бедренной кости и вертлужной впадины (Bohler N. et.al. 1985; Kruger K. 1991),
тройной остеотомии таза (David T. 1986; Schrader S.C. 1986; David T. Kasper M. 1991; и др.),

Способ осуществляют следующим образом. По наружной поверхности бедра делают разрез кожи и подкожной клетчатки в виде полукруга длиной 5-7 см, огибающий большой вертел. Тупым и острым способом разъединяют вдоль волокон глубжележащие слои мышц (поверхностная, средняя и глубокая ягодичные, грушевидная мышцы). Далее рассекают капсулу сустава, если она сохранена. Проводят пилу Джигли под большой вертел, опоясывая шейку бедра, и выполняют резекцию головки. Обязательным условием данного этапа артpопластики является обработка культи шейки бедра рашпилем. Резецированную головку извлекают наружу. Вторым этапом операции является иссечение мягкотканных наростов на вертлужной впадине и проведение шаровидной фрезой ее глубокого артроксиза (рассверливания), позволяющего тем самым создать "крышу" покрытия для будущей головки бедра.
