Разрезом по наружной поверхности верхней трети бедра обнажают подвертельную область, через которую с помощью сверла диаметром 1 см производят черезшеечную туннелизацию до субхондральной зоны головки. В образованный канал вводят предварительно взятый ауто- или аллотрансплантат. Периферический конец последнего фиксируют крючкообразно загнутой спицей Киршнера, которую выводят за пределы раны. Во избежание чрезмерного выстояния к концу дистракции периферического конца трансплантата целесообразно первоначальное заглубление его во внутрикостном канале на 1-2 см относительно края входного отверстия. После ушивания раны накладывают трехсекционный аппарат Илизарова: три спицы проводят через подвздошную кость с закреплением их на полукольце, две - через верхнюю треть бедренной кости на полукольце, две - через средний отдел бедра на кольце. Конец спицы, фиксирующей трансплантат, закрепляют на фиксаторе-тягунке на одном из нижних колец аппарата. Боли появились 8 лет назад и резко усилились в последние месяцы. Физиотерапия и анальгетики перестали приносить облегчение. Наблюдалась атрофия мышц бедра (в средней трети - на 4 см). Движения в тазобедренном суставе в сторону сгибания - 180-90о, при отсутствии отведения (приведение возможно на 150о) и ротации. Рентгенологически определялась уплощение бедренной головки с уплотнением ее структуры и резким сужением суставной щели. Через разрез по наружной поверхности верхней трети бедра поднадкостнично обнажена поверхность подвертельной области, через которую с помощью толстого сверла сделан канал в шейке и головке бедра, в которой заведен костный трансплантат. Периферический конец последнего фиксирован крючкообразно загнутой спицей Киршнера. Наложен трехсекционный аппарат Илизарова с проведением трех спиц через подвздошную кость, двух спиц - через верхнюю треть и двух - через нижнюю треть бедра. На аппарате дано одномоментное растяжение области тазобедренного сустава с силой около 15 кг, что обеспечило раскрытие суставной щели до 0,6 см. Конец спицы Киршнера, связанной с трансплантатом, закреплен на фиксаторе-тягунке среднего полукольца аппарата Илизарова. Операционная рана ушита. Через 3 недели начата дистракция трансплантата по 0,25 мм два раза в сутки.
