После обработки операционного поля осуществляют вокруг трахеостомы разрез кожи, который с одной стороны от трахеостомы дугообразной формы, а с противоположной - прямолинейной, т. е. дуга замыкается хордой. Последняя проходит по краю трахеостомы. Кожу, ограниченную дугообразным разрезом, отсепаровывают кнутри разреза, оставив при этом ножку по краю трахеостомы. В результате образуется лоскут на ножке, который после уменьшения в соответствии с размерами трахеостомы укладывают эпидермисом внутрь на трахеостому и подшивают к краю трахеостомы с противоположной стороны узловатыми швами. При этом образуется раневая поверхность. В дальнейшем мобилизуют кожу на стороне прямолинейного разреза, укладывают его поверх подшитого лоскута и фиксируют швами к коже на стороне выкроенного ранее дугообразного лоскута. При этом оба шва располагаются кнаружи от трахеостомы с противоположных сторон на различной глубине. Subsequently, the skin is mobilized on the side of the rectilinear incision, laid on top of the hemmed flap and fixed with sutures to the skin on the side of the previously arched flap. In this case, both sutures are located outward from the tracheostomy from opposite sides at different depths.

При прямой ларингоскопии надгортанник не изменен, голосовые складки несколько утолщены, подвижны, в подголосовой полости пристеночный рубец, просвет гортани для дыхания достаточный. Лечение. Под общим обезболиванием произведен окаймляющий разрез кожи вокруг трахеостомы: схема выкроен лоскут дугообразной формы, справа выполнен прямолинейный разрез (дуга, замкнутая хордой). Кожа, ограниченная дугообразным разрезом, мобилизована кнутри, в результате чего образован лоскут на ножке. После приведения его размеров в соответствие с трахеостомой лоскут уложен эпидермисом на трахеостому и подшит к краю трахеостомы с противоположной стороны. Затем мобилизована кожа кнаружи от линейного разреза и уложена поверх описанного ранее лоскута и раневой поверхности и подшита по краю дугообразного разреза к коже. В результате трахеостома закрыта двумя лоскутами, причем швы расположены с противоположных сторон от трахеостомы на различной глубине. Наложена асептическая повязка. Больной принимал кефзол по 750000 ЕД. 3 раза в день внутримышечно, ингаляции противоотечной смеси, десенсибилизирующую терапию. Послеоперационный период протекал без осложнений. Швы сняты через 10 дней. Рана зажила первичным натяжением. Выписан в удовлетворительном состоянии. При осмотре через 6 мес. дыхание свободное, голос звучный, на месте бывшей трахеостомы послеоперационный рубец. Treatment. Then the skin was mobilized outward from the linear incision and laid on top of the previously described flap and wound surface and sutured along the edge of the arcuate incision to the skin. As a result, the tracheostomy is closed by two flaps, and the seams are located on opposite sides of the tracheostomy at different depths. An aseptic dressing is applied. The patient took kefzol at 750,000 units. 3 times a day intramuscularly, inhalation of a decongestant mixture, desensitizing therapy. The postoperative period was uneventful. Sutures were removed after 10 days. The wound healed by primary intention. Discharged in satisfactory condition. When viewed after 6 months. free breathing, sonorous voice, postoperative scar on the site of the former tracheostomy.
