Claims (1)

Способ устранения комбинированного дефекта орбиты и верхней челюсти, включающий использование реваскуляризированного фрагмента гребня подвздошной кости, отличающийся тем, что производят остеотомию гребня подвздошной кости в дистальном направлении таким образом, чтобы внутренняя кортикальная пластика гребня подвздошной кости в каудальном направлении была длиннее на 2-2,5 см от резецируемого края наружной кортикальной пластинки подвздошной кости, пересекают сосудистую ножку, полученный фрагмент длиной 2-2,5 см внутренней кортикальной пластинки сгибают под углом 90°, аутотрансплантат переносят и располагают в области дефекта верхней челюсти с последующей фиксацией костного фрагмента в позиции альвеолярного отростка, фрагментом внутренней кортикальной пластинки создают нижнюю стенку орбиты, кожной порцией закрывают дефект полости рта, далее сосудистую ножку трансплантата проводят в подчелюстную область через подслизистый слой щечной области и формируют микрососудистые анастомозы между донорскими и реципиентными сосудами.A method for eliminating a combined defect in the orbit and upper jaw, including the use of a revascularized fragment of the iliac crest, characterized in that an osteotomy of the iliac crest is performed in the distal direction so that the internal cortical plastic of the iliac crest in the caudal direction is 2-2.5 longer cm from the resected edge of the external cortical plate of the ilium, the vascular pedicle is crossed, the resulting fragment is 2-2.5 cm long of the internal cortical the lobes are bent at an angle of 90 °, the autograft is transferred and placed in the region of the defect of the upper jaw with subsequent fixation of the bone fragment in the position of the alveolar process, a lower wall of the orbit is created with a fragment of the inner cortical plate, the defect of the oral cavity is closed with the skin portion, then the vascular leg of the graft is passed into the submaxillary region through the submucosal layer of the buccal region and form microvascular anastomoses between the donor and recipient vessels.
