Известен способ мелодико-интонационной терапии речи, когда при восстановлении экспрессивной речи короткие (обходные) фразы вводятся в простую мелодию. При произнесении фраз больной производит рукой отстукивание ритма фразы. Поставленная цель достигается тем, что в качестве организующего и стимулирующего воздействия при речевой реабилитации используется экзогенно предъявляемый светозвуковой темпоритм. Светозвуковая стимуляция производится посредством работы стандартного фото-фоностимулятора при аппарате ЭЭГ 8S (ф. "Медикор"). Первоначальная частота стимуляции составляет 1 Гц. В темпе предъявляемой стимуляции больной читает специально приготовленные тексты (автобиографические, стихотворения и т. д. ), ведет диалог с врачом. Таким образом, заявляемый способ соответствует критерию изобретения "новизна". Известно техническое решение, в котором пир восстановлении речевых нарушений используется ритм мелодии, стихотворения, воспроизводимый больным (нанесением ударов кисти по плоскости) одновременно с произнесением текста, что, однако, не позволяет добиться достаточно эффективной реабилитации, особенно при лечении в поздние сроки от начала заболевания, что достигается в заявляемом техническом решении. This allows us to conclude that it meets the criterion of "significant differences".

определение частоты собственного двигательного ритма в здоровой (левой) руке, для чего электрод электромиографа ("Медикор") накладывался на m. Exstensor disitorum левого предплечья, записывалась электромиограмма движений кисти, производимых (нанесение серии ударов ладонью по плоскости) в произвольном темпе. Скорость движения ленты электромиографа 25 мм/с. Для обсчета брались 12 последовательно записанных на электромиограмме двигательных циклов. Средняя продолжительность двигательного цикла 31,2 ± 0,8 т. е. частота собственного двигательного ритма 1,25 Гц. В дальнейшем частота экзогенной светозвуковой стимуляции во всех случаях составляла 1,25 Гц;
отработка словосочетаний, используемых в первую неделю реабилитации, в экзогенном темпоритме (1,25 Гц);
чтение в экзогенном теморитме (1,25 Гц) простых, составленных врачом текстов из 6-8 предложений;
пересказ прочитанных текстов в темпе экзогенно предъявляемой светозвуковой стимуляции (1,25 Гц).Second week:

В дальнейшем для чтения, пересказа подбирались все более сложные тексты (стихи, статьи), расширялись темы и объем свободной беседы, производимой в темпоритме экзогенной свето-звуковой стимуляции с частотой 1,25 Гц. Занятия проводились ежедневно, кроме этого, больная самостоятельно готовила задания (читала и пересказывала тексты в темпоритме экзогенной светозвуковой стимуляции с частотой 1,25 Гц).Third week:

В результате 2-месячного лечения больная хорошо читает приготовленные ею тексты, ведет свободную беседу (предложения короткие, из 3-4 слов), из речи практически исчезли слова-эмболы. Речь намного свободнее при разговоре в темпоритме экзогенной светозвуковой стимуляции с частотой 1-1,25 Гц. С помощью данного способа пролечено 15 больных: 10 мужчин и 5 женщин в возрасте от 20 до 50 лет с остаточными явлениями травматической и сосудистой патологии: 7 больных с выраженной моторной афазией, больные общаются междометиями, словами-эмболами; 8 больных с умеренно выраженной моторной афазией, в общении которых преобладают междометия, слова-эмболы, предложения из 2-3 слов. Срок давности от начала заболевания 0,5 - 5 лет. Удовлетворительный эффект отмечается у 7 больных (1 больной с умеренно выраженной и 6 с выраженной моторной афазией): больные могут общаться короткими предложениями из 2-3 слов, хорошо читают подготовленные тексты. У 2 больных эффекта от лечения практически нет в силу снижения волевых процессов, отсутствия желания заниматься, вследствие более глубокого поражения мозговых структур и наличия энцефалопатии. Использование предлагаемого способа речевой реабилитации при моторных афазиях у взрослых по сравнению с существующими способами имеет следующие преимущества:
а) повышение эффективности реабилитации у больных, особенно при лечении в поздние сроки от начала заболевания;
б) возможность индивидуального выбора темпа экзогенной свето-звуковой стимуляции, соответствующего частоте "собственного" двигательного ритма больного. (56) Цветкова Л. С.
