Изобретение относится к медицине и может быть применено для лечения и в комплексе оздоровительных реабилитационных мероприятий в системе социально-медицинской и психолого-педагогической реабилитации детей дошкольного, младшего школьного и школьного возраста с гиперкинетическими расстройствами, например, синдромом дефицита внимания и гиперактивности (СДВГ), с использованием музыки, избирательные жанры которой оказывают лечебно-оздоровительное влияние. Синдром дефицита внимания и гиперактивности в последние десятилетия рассматривается в качестве наиболее частых причин нарушения поведения, трудностей школьной и социальной адаптации у детей с нормальным интеллектом [О.Б. В структуре СДВГ большая доля приходится на дошкольников. С одной стороны, именно к этому возрасту полностью разворачивается клиническая картина, с другой в этом возрасте от ребенка требуется достаточное психо-речевое развитие для познавательной деятельности. Проявляется невозможность ребенка сосредоточиться на задании, склонность к перескакиванию с одних дел на другие без доведения их до конца, нерегулируемая чрезмерная активность, как двигательная, так и психическая и речевая, что нередко сопровождается задержкой моторного и языкового развития и отрицательно сказывается на успешности образования. МКБ-11 уже относит СДВГ к нейроонтогенетическим расстройствам. С неврологической точки зрения СДВГ рассматривается как стойкий и хронический синдром, для которого не найдено способа излечения. Аудио-музыкальную программу составляют из минорных по ладовой характеристике и тональностям музыкальных произведений 56-90 ударов метронома в минуту с частотным диапазоном музыкального звука от 400 до 600Гц и исключением резких диссонансных интервалов. Аудио-музыкальную программу представляют специально выстроенной схемой чередования музыкальных произведений, продолжительностью 2-4 минуты, воспринимающихся как спокойствие. Успокаивающий эффект достигают при помощи использования в аккомпанементе одинаковых ритмических фигур мелкими длительностями звучания нот, в том числе шестнадцатыми, и музыкальных произведений продолжительностью по 6 минут с присутствием мажорных включений, воспринимающихся как волнение, вдохновение. Чередованием в закономерном порядке контрастных музыкальных параметров тренируют нейро-гуморальные механизмы работы головного мозга. Занятия проводят под наблюдением специалиста реабилитационного центра в течение 30 минут детям 4-5 лет и по 60 минут детям 6-7 лет. Предварительно врач физиотерапевт/реабилитолог проводит осмотр пациента и определяют текущие параметры организма и путем измерений фиксирует показатели АД, частоты пульса, ортостатической пробы, данные заносятся в таблицу. Проводят модифицированный восьмицветовой тест Люшера. Модифицированный восьмицветовой тест Люшера заключается в выборе ребенком понравившегося в данный момент цвета из предоставленных цветных карточек, не ассоциируя его с посторонними предметами. Это позволяет определить психоэмоциональное состояние ребенка, выявляя не только субъективное отношение ребенка к цветовым эталонам, но также неосознанные реакции на них, тесно связанные с функциями гипофиза, определяющими настроение и общую психическую активность. На основе выбранных ребенком цветов устанавливают по методике В.М. Элькина зависимость между выбором ребенком цвета и соответствующей этому цвету музыкальной тональности, а также на предмет восприятия музыки. Данные используют для отбора детей для музыкотерапии и распределения их по группам на основании индивидуального подхода. В соответствии с группой назначается музыкотерапия, цветовая светотерапия и массаж по методу Су-Джок. Лечение выполняют в два этапа. Первый этап проводят в реабилитационном центре, где первоначально осуществляют локальную цветовую светотерапию, для чего наклеивают на зоны подушечек концевых фаланг больших пальцев ладоней один цветной пластырь, который крепится на время занятий, в день по порядковому номеру или по дню недели фиолетовый-лимонный-красный-красный-голубой-фиолетовый-оранжевый-фиолетовый по системе Диншаха. Ежедневно проводят чередование цветового спектра пластыря. Затем проводят массаж зон на ладонях поочередно правой и левой массажным шариком с шипами «Су-Джок» по методу М. Г. Пановой продолжительностью 5 минут с акцентом на цвет наклеенного пластыря. После чего во время интеллектуально-творческих обучающих занятий, требующих от пациентов активной мозговой работы, осуществляют рецептивную музыкальную терапию музыкально-психотерапевтического регулятивного воздействия аудио-музыкальной программой, составленной из музыкальных элегий. Аудио-музыкальную программу составляют из минорных по ладовой характеристике и тональностям музыкальных произведений 56-90 ударов метронома в минуту с частотным диапазоном музыкального звука от 400 до 600Гц и исключением резких диссонансных интервалов. Аудио-музыкальную программу представляют специально выстроенной схемой чередования музыкальных произведений, продолжительностью 2-4 минуты, воспринимающихся как спокойствие. Успокаивающий эффект достигают при помощи использования в аккомпанементе одинаковых ритмических фигур мелкими длительностями звучания нот, в том числе шестнадцатыми, и музыкальных произведений продолжительностью по 6 минут с присутствием мажорных включений, воспринимающихся как волнение, вдохновение. В процессе занятий педагог неоднократно обращает внимание ребенка на цветовой спектр наклеенного пластыря в словесной, возможно и в стихотворной форме. Аудио-музыкальная программа звучит фоном (35 Дб) для рецептивного восприятия. Чередованием в закономерном порядке контрастных музыкальных параметров тренируют нейро-гуморальные механизмы работы головного мозга. Занятия проводят под наблюдением специалиста реабилитационного центра в течение 30 минут детям 4-5 лет и по 60 минут детям 6-7 лет. Частоты тонов/звуков в аудио-музыкальной программе и частотные характеристики цветов, которые нанесены на пластырях для проведения цветовой светотерапии, совпадают. Перечисленные действия первого этапа проводят курсом 12 индивидуальных или групповых занятий ежедневно или через день, два раза в год в течение трех лет. Второй этап лечения выполняют в домашних условиях в той же последовательности и в том же объеме, что и на первом этапе. На зоны подушечек концевых фаланг больших пальцев ладоней наклеивают один цветовой пластырь в день по порядковому номеру или по дню недели фиолетовый-лимонный-красный-красный-голубой-фиолетовый-оранжевый-фиолетовый по системе Диншаха. Ежедневно проводят чередование цветового спектра пластыря, который крепится на время занятий. Затем проводят массаж ладоней продолжительностью 5 минут. После чего проводят интеллектуально-творческие обучающие занятия под рецептивную аудио-музыкальную программу, составленную из музыкальных элегий. Перечисленные действия второго этапа проводят не менее трех раз в неделю в течение трех лет. The audio-musical program is made up of musical works, minor in terms of modal characteristics and keys, 56-90 metronome beats per minute with a frequency range of musical sound from 400 to 600 Hz and the exclusion of sharp dissonant intervals. The audio-musical program is represented by a specially built scheme of alternating musical works, lasting 2-4 minutes, perceived as calm. A calming effect is achieved by using the same rhythmic figures in the accompaniment with small durations of notes, including sixteenth notes, and musical works lasting 6 minutes each with the presence of major inclusions, perceived as excitement, inspiration. In the process of classes, the teacher repeatedly draws the child's attention to the color spectrum of the pasted patch in verbal, and possibly in poetic form. The audio-music program sounds in the background (35 dB) for receptive perception. By alternating contrasting musical parameters in a regular order, neuro-humoral mechanisms of the brain work are trained. У получавших только музыкальную терапию аудио-программой «Элегия», число детей с улучшением было сопоставимо с основной группой: 85,4% (47 чел.), однако, общий суммарный показатель клинических симптомов снизился на несколько меньшую величину: от 103±2,24 до 30±1,2 баллов, за счет сохранения у ряда детей в структуре клинических симптомов более низких показателей уменьшения и редких признаков нивелирования симптомов, а именно: симптом импульсивности снизился на 16,2% меньше, чем в основной группе. Вероятно, это связано с отсутствием визуально-тактильного влияния цветовой светотерапии, как фактора усиления эффективности реабилитации. Проведение курса лечебной регулятивной музыкальной терапии аудио-программой «Элегия» и локальной цветовой светотерапии позволило уменьшить повышенную двигательную, речевую и эмоциональную активность, дало возможность проявиться сосредоточенности при выполнении работ, улучшить речевую моторику у детей с СДВГ. Лучшие результаты отмечались у детей основной группы, прошедших регулярно в течение трех лет музыкальную терапию как в условиях реабилитационного центра, так и в домашних условиях. Диагноз: Гиперкинетическое расстройство (F90.).Diagnosis: Hyperkinetic disorder (F90.).
Жалобы на частые головные боли, дискомфорт в области шеи, плохой сон, повышенную двигательную активность, импульсивность, невнимательность, конфликтность в социуме. Первые признаки заболевания заподозрены в 2-х летнем возрасте. Специфическую медикаментозную терапию не получал в связи с проявлением побочного действия психостимулирующего препарата в виде резкого возбуждения. В клинической картине исходно наблюдалась легкая отвлекаемость, забывчивость, мечтательность, неорганизованность, низкая концентрация внимания и сложность с выполнением задач. Чрезмерное беспокойство и возбужденность, повышенная двигательная активность, неусидчивость, нередко разрушительные действия в отношении сверстников, животных, игрушек значительно ухудшали качество жизни ребенка и его родителей. Нарушение речевого развития выражалось в том, что речь была настолько торопливая, что ребенок не успевал подумать, как выстроить предложения и это затрудняло процесс познания и общения. Симптомы заболевания проявлялись как в домашних условиях, так и в общественных (детский сад, реабилитационный центр) местах.Initially, the clinical picture was mild distractibility, forgetfulness, daydreaming, disorganization, low concentration and difficulty with tasks. Symptoms of the disease manifested themselves both at home and in public (kindergarten, rehabilitation center) places.
При общем физикальном обследовании отклонений не выявлено. В неврологическом статусе отмечались признаки легкой пирамидной недостаточности, проявляющейся неуверенностью при ходьбе; нарушением тонкой моторики; тестированием выявлено снижение показателей образного и вербально-логического мышления. Лабораторные анализы крови в норме. По ЭЭГ исходно доминировал альфа–ритм в диапазоне 7-9 Гц, что расценивалось как норма. При этом отмечены признаки незрелости лобно-таламических структур, где степень синхронизации альфа-колебаний низкая по сравнению с сигналами от передне-центральных отделов мозга. Зарегистрированы повышенные показатели альфа волн в отведениях левого полушария с фокусом в височных и затылочных отведениях. Эпи-сигналы не зарегистрированы. При допплерографическом исследовании церебральной гемодинамики скорости мозгового кровотока и индексы резистентности на интракраниальном уровне снижены. At the same time, signs of immaturity of the fronto-thalamic structures were noted, where the degree of synchronization of alpha oscillations is low compared to signals from the anterior-central parts of the brain. Increased levels of alpha waves were registered in the leads of the left hemisphere with a focus in the temporal and occipital leads. Epi-signals are not registered. In Doppler study of cerebral hemodynamics, cerebral blood flow velocity and resistance indices at the intracranial level are reduced.

В условиях реабилитационного центра проведено шесть реабилитационных курсов за три года музыкальной терапии аудио-программой «Элегия» и локальной цветовой светотерапией во время интеллектуально-творческих занятий и массажа по Су-Джок в количестве 12 раз за курс на фоне общей реабилитационной программы (энтеральная кислородотерапия, массаж, ЛФК, гидротерапия, психолого-педагогическая коррекция, курсовая терапия витаминно-минеральным комплексом «Супрадин» через каждые 4 месяца по 20 дней). В домашних условия пролангирована рецептивная музыкальная терапия аудио-программой «Элегия», локальная цветовая светотерапия и массаж Су-Джок во время развивающих игр и занятий и в последующие годы подготовительных заданий к школе не менее 3-4 раз в неделю непрерывно в течение трех лет. Изначально проведено обучение родителей методическим навыкам проведения занятий на дому.In the conditions of the rehabilitation center, six rehabilitation courses were conducted over three years of music therapy with the audio program "Elegy" and local color light therapy during intellectual and creative classes and massage according to Su-Jok in the amount of 12 times per course against the background of a general rehabilitation program (enteral oxygen therapy, massage, exercise therapy, hydrotherapy, psychological and pedagogical correction, course therapy with the Supradin vitamin and mineral complex every 4 months for 20 days). At home, receptive music therapy with the Elegy audio program, local color light therapy and Su-Jok massage during developmental games and classes and in subsequent years of preparatory tasks for school were prolonged at least 3-4 times a week continuously for three years. Initially, parents were trained in the methodological skills of conducting classes at home.
Первый год реабилитации показал достоверное (р<0,05) снижение клинических симптомов СДВГ: гиперактивность уменьшилась на 23 балла, невнимательность на 33 балла от исходного. Ребенок стал более спокойным, уменьшились избыточные движения, речевая продукция стала более понятной, размеренной. Эпизоды импульсивности сохранялись. После второго года реабилитации заметно улучшилось поведение: ребенок стал прислушиваться к обращенной речи и стараться точнее выполнить инструкции, заметно уменьшились эпизоды перебивания взрослых при разговоре (оценочная интеллектуальная функция). Признаки импульсивности стали единичными, чаще в виде резких движений, агрессии уже не проявлял. К концу третьего года реабилитации логопедом подтверждено речевое развитие, соответствующее возрасту. Тестовыми пробами подтверждена психологическая устойчивость. Эхоэнцефалографические отклонения оставались минимальными, а допплерографические показатели краниального и экстракраниального кровотока соответствовали возрастной норме. Достаточная усидчивость и спокойное поведение позволили хорошо подготовиться к школе в подготовительном классе и ладить с сверстниками.The first year of rehabilitation showed a significant (p<0.05) decrease in the clinical symptoms of ADHD: hyperactivity decreased by 23 points, inattention by 33 points from the baseline. The child became calmer, excessive movements decreased, speech production became more understandable, measured. Episodes of impulsivity persisted. After the second year of rehabilitation, behavior improved markedly: the child began to listen to addressed speech and try to follow instructions more accurately, episodes of interrupting adults during a conversation (evaluative intellectual function) noticeably decreased. Signs of impulsivity became isolated, more often in the form of sudden movements, he no longer showed aggression. By the end of the third year of rehabilitation, the speech therapist confirmed age-appropriate speech development. Test samples confirmed psychological stability. Echoencephalographic deviations remained minimal, and Doppler parameters of cranial and extracranial blood flow corresponded to the age norm. Sufficient perseverance and calm behavior made it possible to prepare well for school in the preparatory class and get along with peers.
При ежегодных контрольных осмотрах невролога медикаментозная специфическая терапия не назначалась в связи с положительной динамикой клинических симптомов.At annual follow-up examinations by a neurologist, specific drug therapy was not prescribed due to the positive dynamics of clinical symptoms.
