Abstract
Description
The invention relates to medicine, as well as to surgery, prevention and treatment of purulent wounds: extensive wound and burn surfaces, surfaces and cavities with high bacterial contamination.
There is a method of treating skin diseases, which consists in irradiating the affected area with ultraviolet (UV) radiation, infrared (IR) radiation and a small part of the visible radiation, moreover, the radiation has a linear spectrum, is characterized by low intensity and constantly in time (see US patent N 3818914, cl A 61
The disadvantages of the known method and device are the significant time of the treatment procedures (tens of minutes), the presence of undesirable side effects: photochemical and photobiological reactions (erythema, peeling, etc.) in the surface layer of the irradiated area, operating time in the air under the influence of UV radiation of high concentrations of ozone and other toxic gases.
These disadvantages are due to the use in the known method and device of continuous in time UV radiation of low intensity with a linear spectrum.
Closest to the proposed method in essence is a method of treating purulent wounds, including the introduction into the body of antibacterial drugs simultaneously with exposure to the wound of UV radiation in the range of 2200-2600 2200-2600 and infrared radiation in the range 52000-52600 52000-52600 within 20-30 minutes (ed. St. USSR N 1600790, class A 61
In the known installation, low-intensity mercury bactericidal lamps DRB-8 with a power of 8 W with a linear emission spectrum are used. The operating mode of the installation is continuous.
In principle, the known method and installation retain the disadvantages of the analogue, namely: due to the use of continuous UV radiation of low intensity with a linear spectrum, the duration of the treatment procedure and preparatory operations are significant and, in addition, there are negative side effects.

The implementation of the proposed method of prevention and treatment and the operation of the proposed installation are as follows.
2.Before irradiation, the wound is treated with a 3% solution of hydrogen peroxide, dried and then irradiated with pulsed UV radiation of a continuous spectrum with a pulse duration of not more than 2˙10 -3 s, a power density in the treatment area of at least 10 kW / m 2 and the total energy dose not less than 10 2 J / m 2 .
When using this method for the purpose of prevention after irradiation, the wound is sutured.
In the treatment of wounds after UV irradiation, a bandage is applied.
2. The required total energy dose, which is determined by the specific type of pathology and the state of the wound surface, is set using the switches of the
Образующаяся плазма с температурой 10000-15000 К и выше является высокоинтенсивным (плотность мощности УФ-излучения превышает 108 Вт/м2) источником широкополосного излучения, значительная доля которого приходится на УФ-область спектра (с длинами волн короче 400 нм).A powerful pulsed electric discharge in an inert gas filling the
Such discharge modes in the lamp are realized if the parameters of the discharge circuit satisfy the ratio presented in
The radiation flux of the
The use of high-intensity pulsed UV radiation of the continuous spectrum in comparison with low-intensity continuous UV radiation of the line spectrum allows, first of all, to significantly reduce the threshold energy doses of radiation necessary to achieve a preventive and therapeutic effect.
1) При высокоинтенсивном УФ-воздействии скорость подавления патогенной микрофлоры в ране значительно превышает скорость ее роста за счет собственности разложения и внешнего обсеменения. При низкоинтенсивном УФ-воздействии возможна обратная ситуация и лечебный эффект может отсутствовать даже при очень больших временах (или энергетических дозах) облучения;
This is due to a number of factors:
1) With high-intensity UV exposure, the rate of suppression of pathogenic microflora in the wound significantly exceeds its growth rate due to decomposition property and external seeding. With low-intensity UV exposure, the opposite situation is possible and the therapeutic effect may be absent even at very large exposure times (or energy doses);
2) With prolonged low-intensity irradiation, microorganisms adapt to UV radiation and, as a result, their sensitivity to ultraviolet radiation decreases. Moreover, it is known that in some cases, low-intensity UV radiation has an effect that stimulates the growth and reproduction of microorganisms.
3) With broadband irradiation (continuous spectrum) compared with exposure to monoenergetic radiation (i.e., radiation with a linear spectrum), the possibilities of adaptation of living matter to UV radiation are significantly reduced, which is associated with a multi-channel destructive effect on photon biomolecules with a wide energy spectrum.
4) Different types of microorganisms and biological tissues have different spectral characteristics of bactericidal efficacy and optical transmission, therefore, broadband UV radiation in the case of a wide variety of wound infections will on average have a greater penetration depth and lower threshold energy doses needed to achieve a preventive and therapeutic effect.
). The reduction of threshold energy doses for exposure to pulsed UV irradiation of wounds with continuous spectrum radiation and the high-intensity nature of such radiation lead to the fact that the duration of a preventive or therapeutic procedure is significantly reduced (from tens of minutes to several seconds), while the likelihood of negative side effects of UV effects of both photobiological (erythema, microcirculation disturbance, peeling, etc.), and photochemical nature (operating time of ozone, etc. )
