Больному с установленным диагнозом местного перитонита на почве гангренозно-перфоративного холецистита проводят общепринятую предоперационную подготовку. Под общим интубационным наркозом разрезом в правом подреберье вскрывают брюшную полость, удаляют гангренозно измененный желчный пузырь, гнойную полость дренируют тампонами (1) с сорбентом (активированным углем), размерами 10×200 мм в количестве 5, дренажной трубкой, размером 5-8×230-250 мм, которая расположена в центре между ними, предварительно устанавливают в оболочку (3) из эластичного проницаемого материала. Дренажную трубку (2) подключают к электроотсосу (4). Брюшную полость послойно зашивают до устройства. Больному проводят общепринятый комплекс послеоперационного лечения. Через сутки меняют тампоны путем последовательного их извлечения. Смену тампонов проводят в течение 5 суток до полной очистки гнойной полости от бактерий и токсических веществ, после чего устройство удаляют из брюшной полости.A patient with an established diagnosis of local peritonitis on the basis of gangrenous-perforated cholecystitis is given generally accepted preoperative preparation.
